yes I am sexually active, I did have contact with a commercial sex worker, no fever, I did smoke at the time. I was also taking two types of amoxicillin and another antibiotic

The white tongue and gastric irritation could be
1. Secondary to smoking.
2. Fungal infection can lead to a white tongue. However I am sure your doctor would have examined and ruled it out.
3. Vitamin B complex deficiency

Here is what I suggest

1. Stop smoking
2. Drink plenty of water.
3. Avoid liquid or semi solid food
4. Take vitamin b complex everyday for a month
5. If it does not settle down I recommend a tongue scraping for histopathological examination

I have had a lot of gas burping my doctor said it's gerd so I took the meds he gave me and heartburn went away but my penis head feels itchy no discharge and negative std and hiv

I could not make out much with picture you sent me as the focus was mainly on the throat and not the tongue.

Itchy head of the penis could be because of inflammation of the head of the penis. Candidial infection of the penis can cause a itchy penis however your doctor would have ruled it out. The second common reason would be bacterial infection. Please make sure you clean the penis thoroughly with soap everyday to prevent infection. I also recommend you to send a urine routine to rule out urethritis.
For now I recommend Tab Levocetrizine twice daily to bring down the itching. Keep the are dry and clean at all times
