Question: For Dr Rovena Murati
I hope you will be fine dr,
I have some questions i hope you will answer same like before.
As i told you i was feeling pain in my arm and tremer or twitchings also, that was finished in two days , but still i feel a pain sometimes in arm, also in both arms i feel someties A pain like tired feeling, what could be the reason?
As you know my doctor gives me a medicine cobal, that you told me its used for B12 deficency, and its aslo used for neuropathy , my question is ,as per my medical reports i dont have b12 deficiency, and if i dont have b12 deficiency then the only possible reason is nuropathy? Because after the twitching problem doctor told me that you should check your sugar level, and you told me that diabeties dont have such early symptoms , then kinldy tell me what could be the possible reason of my muscle twitchings and vribration type feelings? Because i am tired now with these fearfull thoughts, because every time i thought i have nurological problem suddenly hiv comes in my mind. I knw my risk is less but my symptoms are real that gives me depression,

Hello XXXXXXX

I am sorry that these symptoms are causing you such problems with your daily life.

First let me explain some things.

First ,I have not excluded Vit B12 deficiency as a possible cause for your symptoms. The blood test that you have done only can tell if you have anemia or not.
Vit B 12 deficiency causes anemia ,so since you are not anemic ,we can assume that you don't have a severe vit B12 deficiency.
This doesn't exclude a mild form of it or the early onset of Vit B 12 deficiency ,because in this stage anemia is not present but other symptoms of muscle twitching, vibrating sensations or numbness ,tingling sensations in legs and arms etc.
To know it for sure you have to test the vit B12 level.
For this reason your doctor prescribed you Vit B12 supplement.
If these symptoms were due to Vit B12 deficiency ,they will improve some weeks after you start taking the supplement.

If there have been some weeks on Vit B12 supplement and the symptoms haven't improved ,then it is less likely that they are related to Vit B12.

It is true that having high level of blood sugar can cause muscle cramps ,tingling and numbness in feet or hands ,but this happens when you have long term uncontrolled high levels.

Since this muscle twitching ,muscle pain and weakness is persistent you may need to consult a doctor (neurologist if possible ) for a detailed examination.
I would suggest to test your thyroid function test (TSH level) and you should get tested for HIV so you can stop worrying about that.
A doctor will do a complete physical examination ,evaluating the nerves and muscles function.
If necessary the doctor will do some specific tests to evaluate muscle and nerves:
-A nerve conduction study (NCS)
-Electromyography

It is very unlikely that these symptoms are related to HIV and you should stop worrying about that.
I really would suggest to see a neurologist. If the doctor excludes any possible neurological condition as a cause ,this will help to manage your anxiety and the symptoms will improve.
The doctor may also prescribe you an anxiolytic to help with the anxiety and stress.

Let me know ,how can I help you further.
Take care.

Thanks a lot for your detailed reply
I will check dr but i was planning to test for sugar level because my doctor told me to test after one month and it will be next week. After that i will test for thyroid .
Kindly give me answer of these questions
If i dont have diabeties and hiv as a cause of these nurological problems then how can i have these problems? Means what could be the reason of these nurological problems?
Pain in my arms and muscle twitchings or tremer could be due to flue? Now its reduced or finished, as i told you before i was feeling like flue from last 5 or 6 days and like mild fever but i was not having fever , now these feelings are reduced but still i have slughtly, is it possible that arm pain and twitchin could be due to this reason? And anxiety and stress increased these problems?
Stress and anxiety can cause nurological problems? Or the problems that i have like muscle twitching like pulse , that can come anywhere in body.

Hello again XXXXXXX

A viral infection can cause muscle pain or body aches so your symptoms of muscle pain in arms.A viral infection can also cause muscle cramps.
If the symptoms persist even after the viral infection is cleared up ,then your symptoms are not related to it.

As I have told you ,anxiety and stress are the most common causes for such symptoms. If all other possible causes are excluded then anxiety might be the cause.
In this case the doctor will prescribe you a treatment for anxiety and see if the symptoms improves.
Stress and anxiety can not cause neurological problems

The most common causes for neuropathy are diabetes, vitamin deficiency ,some specific neurological condition (mostly with unknown causes) ,thyroid dysfunction .Neurological problems refer to specific neurological condition ,where the peripheral nerves are affected .The cause mostly is unknown ,although some have autoimmmune causes.

Take care.

Thanks for reply dr.
You mean if i am not feeling muscle pain after some days then it means it was due to flue? Or some other viral infection?
But the muscle twitching i was feeling even without flue condition.
You know all the sympyoms which i have discused Now dr kindly tell me considering all of my symptoms from muscle twitchings , sugar level, stomach problems, rashes, pimples, mouth canker etc which i have from last some month means on and off condition. What you think which disease i have more chances to have?
I want to inform you one thing that my stomach is very much improved now a days means from last two weeks.

and what you think my symptoms related to autoimmune disease? Does my test which i have done indicates this dieases or not?

Hello again XXXXXXX

Yes,if your symptoms were due to viral infection ,they will improve once the infection is resolved but I don't think your problems are due to flu because you had such symptoms times before the infection.

I am glad that your stomach symptoms are improved and you are feeling better.
I don't think that all your symptoms are related or indicative for a specific condition.
Means the stomach symptoms were due to acid reflux. Not eating healthy for some time and the stomach problems can have caused a mild Vit B12 deficiency.
Another possible cause for such symptoms can be anxiety.

If the muscle twitching and vibration sensations persist even after taking Vit B12 supplements or after the blood sugar is under control ,then the cause of muscle twitching need to be investigated.
If thyroid function is normal ,then you may need to consult a neurologist.
He can do some specific tests to understand if is there any problem with muscles or nerves.

If all other causes for such symptoms are excluded and muscle twitching persist and are continuous the a neurological condition (motor neuropathy) should be taken in consideration.Usually these condition have an autoimmune etiology.

Si I don't think that your symptoms are related to any autoimmune disease but if every other possible cause is excluded this should be taken in consideration.
Your tests doesn't indicate any autoimmune disease.

Try to relax.Managing anxiety is very important.

Take care.

Hello XXXXXXX

It makes sense that the doctor will order only random blood sugar or fasting blood sugar because HbA1c reflects the values of the sugar in blood for the last three months. So you may need to test again HbA1c after 2 months to evaluate the level of blood sugar in last three months.

I will be waiting for the result of your test.
Hopefully everything will be fine.

hy.. i have received my Random blood sugar test result. it is 146.4 mg/dl
and dr told me that its normal. now i will try to do test of thyroid . such type of things give me more stress that if i dont have blood sugar problem then why i was having rash. ? u told me that cobal also not a issue of rash . then the things becomes more complex for me. Mouth canker, pimples and rashes which i was having before , still i could not found the reason. now my stomach is improved , my feeling of hot feet and hands also finsihed , and my arm are also not feeling pain.
my problem is doctor will not prescribe me test of thyroid on my demand. i was trying to explain doctor that suggest me thyroid test. Doctor just examine my throat and neck and he thought i have no thyroid problem . doctor told me to continue the cobal and come after one week. i am trying to test for thyroid . i will check new doctor tomorrow or in upcoming days.

Hello XXXXXXX

I am glad that your blood sugar test was normal.
Since your symptoms seem to be pretty much improved you don't need to worry so much.

All the symptoms you have mentioned pimples, skin rash ,mouth sores are minor problems that doesn't mean to be related or caused from a condition.
Everyone can have mouth sores. They are more frequent if you have anxiety problems.
The skin rashes always have been very localized and have resolved in few days. They are not indicative for any problem.

You should stop thinking that these symptoms are related to any disease.

You should try to manage your anxiety.

The only problem that may require evaluation in your case ,is muscle twitching,if they persist and don't improve with cobal.
In this case evaluation would be necessary to determine if the cause is anxiety or a medical condition.

Dr.Rovena

yes doctor you are right , but i will try to test for thyroid and i will inform you about the results.
dr is it possible that from last near about 25 days i was trying and strict diet , means i am not taking any sweet dish and also doing more exercise , is that could be a reason of normal sugar level.? my question is , is it possible that before i was having these problems due to high blood sugar?
how mouth sores can come with only anxiety or stress?
and i am agree with you my rash was only for some days , but i have persistent pimples. thats also a fear factor. i am taking a 500mg cobal daily one tablet from last 25 days. but still no effect. i dont have a constant twitching all the day, but i may feel any time anywhere. sometimes for a longer time i dont feel anyting. but the fear remains all the time. specially these vibration feelings.

Hello XXXXXXX

Yes,the normal level of your blood sugar most likely is due to the diet changes you have maid.
A strict diet and regular physical activity are very important in controlling the level of blood sugar.I would suggest to continue these diet changes (no sweets,no sugar,no fatty ,processed foods, more vegetables and fruits) and exercise regularly.
As I have told you ,a high level of sugar can trigger skin acne. You should know that stress and anxiety also can trigger acne (pimples).During stress body increases the production of a hormone called cortisol that can trigger acne formation.
Regarding the mouth sores, stress is a major factor in getting them.It is possible to have recurring mouth sores without an obvious cause ,more since you don't have other symptoms that would suggest an underlying skin condition.

I would like to know the result of the thyroid test.

Hello XXXXXXX

The level of TSH 0.5 is within normal values. It is very unlikely that your symptoms are related with this value of TSH.

Now ,try to relax and give it some more time .Continue with cobal and see what happens.
If the muscle twitching is persistent consult a neurologist .
If pathological causes are excluded ,then it might be idiopathic muscle twitching and the cause is unknown.

Take care.

one more thing. as you know i was having a high sugar level before. does that could be a reason ? means i am talking about the rash on hand could be due to hight sugar level at that time?

Detailed Answer:
Hello XXXXXXX

I don't think a calcium test is necessary since you have been tested before and the level was normal. You can repeat the test just to be sure it is normal.

As I have told you before ,a high level of blood sugar can cause skin problems like rash and pimples so it is possible that the rash in your hand was related to high blood sugar.

You don't need to worry so much for the skin rash you had.It is not of concern and was just a common skin rash.

Let me know if you need further help.

All the best,
Dr.Rovena
