What Does My Thyroid Test Result Indicate?

Question: I have been experiencing pain & tenderness under my chin at the top of my neck, a lot of fatigue, dry ski (psoriasis), brain fog and extreme foot pain. I saw my primary dr and she check my thyroid levels:
TSH 1.07 (range 0.40-4.50)
Free T4 0.9 (range 0.8-1.8)
Free T3 2.8 (range 2.8-4.2)
Since everything fell within "normal' range but I was continuing to have symptoms, she ran some more labs and everything came back normal with the exception of the following:
Now, TSH is 0.79 (range 0.40-4.50)
T4 is still 0.9 (range 0.8-1.8)
Vitamin D, 25-OH Total 22 low (range 30-100)
ALT 31 high (range 6-29)

Good day,
Noted your symptoms.
The pain under the chin is unlikley to be due to thyroid issues. Both the thyroid function tests are normal as well.
You may have to do an ECG ( EKG) of the heart.
How are your periods ?

Vitamin D is low and You will need Vitamin D replacement . However, i must say that even Vitamin D deficiency will not cause the symptoms which you mentioned.

You may also have to do a blood glucose test and cholesterol profile.

I did have an EKG done during my last annual visit but the PA did not do it this year for some reason.

I no longer have a period because I had a partial hysterectomy about 5 years ago due to uterine fibroids.

As far as I can see on my lab report, under the comprehensive Metabolic Panel, the Glucose was 89 (range (65-99) and the cholesterol profile as follows:
Total Cholesterol 198 (range 125-200)
HDL 55 (range > or +46)
Triglycerides 81 (range <150)
LDL 127 (range <130)

With that said, I didn't mention in my original post to you that 2 years ago, I had a benign tumor removed. It was suspected to be in my lung and after a 5 hour surgery, it was found to actually be embedded in my diaphragm. As a result, about a quarter of my diaphragm was removed. I had a long recovery because of the size of it and it could not be done laproscopically. Around January of this year (1 1/2 years post-op), some of my current symptoms (with the exception of the foot pain) began and my pulmonologist saw me and did an xray in his office and said my lung looked fine and that a shadow at the base of my lung he saw was probably just the mesh where they reconstructed my diaphragm. I never did anything about it just like I ignored the original tumor since I was initially told it was a calcification and that it was not something I had to worry about but just to keep an eye on it. Now, I know I might sound paranoid but given these new lab results, should I be worried?

Hi, thanks for the reply. Your blood works are fine. With this additional history, i must think of 2 other issues.
1) you had a hysterectomy and we dont know the current status o your obaries.
It is worth checking a blood level of FSH and Estradiol ( fasting, between 7 to 9 AM) which will tell the status of your oavies. Since you have no periods, we dont know the status of oavries. Low levels of estrogen ( estradiol) can cause some of the sympoms like dry skin and anxiety.

2) You could also be suffering from little bit of anxiety.

3) Irritation of diaphragm from the mesh could be partly responsible for the symptoms becuse when you have irritation of diaphragm, you tend to have pain or discomfort in the shoulder region and rarely in neck area.

It is worth repeating your EKG again this year and if that is normal, please stay relaxed.

I thank you for your response. I am calm and any anxiety you might sense is because I don't want to be caught off guard. I take a proactive approach to my health and simply want to explore if my vitamin d levels can be what is affecting my overall feeling. Last year my level was at 29 and now it's at 22. My symptoms started after that last check up.

Vitamin D deficiency

Detailed Answer:
Vitamin D is made under the skin on exposure to sunlight and it is also present in very few foods like oily fish. Deficiency of vitamin D cause body ache, muscle weakness and weak bones.

Low Vitamin D level has been claimed to cause number of disorders including depression, diabetes , arthritis etc. Most of these are based on observations and not proven yet. Many such claims are suspected to be driven by pharma companies to boost sales of vitamin D.

Endocrine society and many other endocrinology authorities have recommended Vitamin D replacement only for bone health ( not to prevent diabetes , depression, arthritis etc )

Vitamin D level of 22 ng is low, but not too low. Less than 10 is considered as deficiency. Levels between 20-30 is insufficiency.

It is unlikely that Vitamin D lnsufficiency has caused your symptoms.
As I mentioned earlier, it is worth doing an FSH and Estradiol level to check your ovary status

It is good to take vitamin D replacement to keep Vitamin D levels above 30 ng/dl. 15 to 20 minutes of sunlight exposure is also important to prevent further lowering of Vitamin D in future.

Thank you. Please start the Vitamin D replacement after meeting your primary care doctor and also take sufficient dietary calcium ( low fat milk, milk products).
As I mentioned earlier, sunlight is good to improve the vitamin D levels and it gives a feeling of general well being. Wish you good health
