What Does My Lab Test Report Indicate?

Question: hi
i had an us done and the impression says Fatty Infiltration no focal lesion, should i be worry about metastasis, i just had left nephrectomy done las Feb 2016,

i also have generalized itching, what is the next step for me

Metastasis refers to the spread of cancer from one part of the body to the next either through direct spread (organs in contact with each other), through the blood vessels or through the lymph nodes.

Cancer is not mentioned in the report.

According to what is said above, there is evidence of fat build up in the liver (fat is infiltrating/getting into different parts of the liver).
This is not related to cancer but can cause different problems

If the fat build up continues, it can cause the liver to become inflamed.
This can go onto liver damage and in some cases cirrhosis/scarring of the liver so that it cannot work well.

Liver disease can cause itching though other causes should be ruled out like allergies, infection (Eg scabies, fungal ), kidney disease, thyroid disease,

It would be best to treat the fatty changes seen. Fatty liver can be due to alcohol use or inappropriate diet/lack of activity.
If due to alcohol would be best to decrease

If due to diet/lack of activity- would need to alter diet to increase fruits and vegetables, decrease sodas/artificial juices, decrease unhealthy carbs (turned into fat and stored)

NEXT steps: (a) make sure liver is only possible cause for itching, may need to see dermatologist or your GP if dermatologist not available
(b)need to alter diet/activity or decrease alcohol use if this is an issue

Do you have any comment on the other impression related to the rigth kidney?

I had a renal US done on May 18 and it was clear (no focal masses) how come in 2 weeks they see a nodule or somethinf?

So definetely not metastasis from Renal Cancer (which i had), how accurate is the US ? If it was mets the US would have detected correct?

Can you please explain me the meaning of no focal lesion? Means no tumors and no mets?

No more imaging necessary to rule out
anything?

I jist had blood work for the tyroid and it was normal also blood work for kidney was ok (creatinine and gfr within range)

How do i check my metabolism to make sure is not the reason of the fatty liver

HI

In terms of the kidney they state that there is an abnormal area (said hypoechogenecity) that is likely the scarring and cyst seen on a previous MRI but with the ultrasound would not be able to see as clearly

This means that on a previous MRI, they noted these changes but they cannot see it as clearly on the ultrasound (MRI shows things better than ultrasound)

Scarring (like scar tissue formed after a fall or injury) is noted at the cortical aspect (top of the kidney)
This may be due to previous infections or trauma.
A cyst is also seen , cysts are not abnormal in the kidney and in most cases cause no problem

NO focal lesion means no specific mass , no single mass/lesion seen. No mets mentioned according to this result.
This means that there is not likely to be any cancer since this would cause a mass to be seen.

Metabolism would not usually affect the liver but you can do liver function blood tests to see if the liver is working well.
This would be ast, alt, alkaline phosphatase, bilirubin, ggt, proteins

One more concern, how accurate is the US to diagnose Fatty Infiltration and what is the likelihood to confuse Metastasis by Fatty Infiltration

Sorry about the delayed reply

Ultrasound of the abdomen is the most common way used to diagnose fatty liver at the moment.
The appearance of fat and cancer lesions would be different on imaging. Mets would likely have a nodular appearance (distinct lesions seen)

Fat would have an irregular appearance, irregular borders

Ultrasound technicians would be able to distinguish the two.

Are there any other blood work test that I can do for the liver?, I don't understand why it went from normal on my US on April 8 2016 to fatty infiltration on XXXXXXX 3 2016. - Do you think that is possible in such as short period of time?

my concern is because the US is showing fatty infiltration but most or all of the blood work parameters are within range.

As you mentioned , all of the above results are within normal range this means that though there is fatty infiltration, it is not yet affecting the function of the liver which is good.

Diet changes and exercise would then help to prevent progression to the point where liver function is affected
