Question: Hi

I have been taking primolut for a period of 10 days. The first time I took it 5 mg once a day the doctor realized the pharmacist made a mistake I was supposed to take it 3 times a day. So after 5 days I started taking it 3 times a day so 15mg altogether.

It was supposed to be to stop some heavy bleeding that had gone on for 3 weeks without stopping.

They also took a sac which showed a 5cm ovarian cyst on the left ovary, I also have a fibroid that has been there for years without causing any trouble.

They took blood tests which came out normal.
I was getting really bad heart palpitations dizziness and light headedness, but no anemia showed on the blood test.

The primolut has made me feel better and the bleeding although not completely stopped is now light spotting.

I went back to the doctor today as its my last day on primolut and she said I don't need to take them anymore but she hasn'r re checked my blood, she won't take a pap smear because she won't until I stop bleeding completely.

Im worried that I will start bleeding again and my progestorene levels will drop again my estrogen will rise again and so will mu blood pressure.

I will be travelling alone with my child on an 8 hour flight in three weeks and worried about bleeding or blood clots.

Can you please give me some advice? Im very worried

I was hoping the doctor would follow up with some contraceptive pills to keep my hormone levels balanced but she told me at my age its not convenient????

Thanks for writing to us with your health concern.
Around your age, you are peri menopausal, and this age is often associated with hormonal imbalance.
The hormonal imbalance can lead to irregular bleeding.
I wish you would upload the scan report and images, I would like to assess the fibroid and cyst that you mention, and whether they are contributing to the bleeding.
Are you sure the fibroid has never caused any bleeding issues ?
Have you had problems with hypertension, and are you on any medication for the same ?
The palpitations and dizziness is also caused by hormonal imbalance mostly.
Have those symptoms gone since ?
Even if hormonal imbalance is causing these transitory symptoms, yes, the cyclical progesterone therapy is given for atleast 3 months, to stabilize your system.
It does not make sense to stop the progesterone out cold, and then take nothing.
Ideally progesterone is given every month for 10 days for 3 months temporarily till you ease out into menopause.
Since you are travelling after a few days, please ask your doctor to supplement cyclical progesterone for you.

thank you for your reply. It all starte dvery suddenly, actually after a series if events that were very stressful to me. Exactly 2 months ago I was ok, blood pressure as normal 68 over 120. Perfect health, perfect hormone balance. Never bled with my fibroid. The suddenly after I sprained my leg badly it shocked me to be home unable to move, then I experienced another emotionally taumatic event and BOOM! I started getting head aches , light headeness and a period that wouldn't end! For the first time ever in my life. two months prior evrything was perfect.

Im in very good physical health, I don't smoke don't drink, Im married,Im not overweight and I eat a good healthy diet of mainly beans fruit and vegetables, never any junk food.

The radiology report says Ovaries are normal, cyst measuring 30x22mmin posterior wall ET-10mm
Large intramural fibroid 7x7 3x5 cm

No free fluid

Oh I forgot to mention that today I started bleeding again even with the primolut pills! Im so worried... Would I be ok for tonight? I worry I will bleed too much.

Here are the documents so far, it doesn't look like the fibroid is causing too much trouble. Two years ago I had a uterus inspection with a microscope while I was under anaesthesia and it was clear for cancer. I saw Dr Svigos in Adelaide Australia.

Dr Svigos at that time thought it would be better to leave the fibroid as it wasn't causing any symptoms except the uncomfortable bladder incontinence.

Is it possible to menstruate while taking Primolut tablets?

What would you suggest be my next step?

Since Im going to be travelling a lot this year, I would prefer to take hormone tablets for 6 months and see how things go. What would you suggest?

Hello again.
Well, I have gone through your report.
All of them are essentially normal.
It sounds like hormonal disturbance secondary to the trauma that you mentioned.
When was your last Pap smear ?
What is your height and weight ?
Maybe the dosage of progesterone is not enough for you.
ALso, are you on medication for the hypertension ?
You might need an increased dosage of progesterone.
IF you bleed inspite of that, you can try taking Trapic - MF ( tranexamic acid and mefenamic acid ) combination.
It is just possible that the fibroid, that was asymptomatic all these years, is not causing some bleeding, that might be happening.
Even if that is the case, surgery is a last resort, since you are nearing menopause.
So medications can be tried.
Since travelling is on the cards, it is best to have a long term option.
Rather than deal with the titrating of progesterone and other tablets, an excellent option is insertion of the Mirena intra uterine device.
You can have it inside, and it controls such bleeding without worrying about tablets and oral medications.
I would suggest trying the Mirena first.

Thank you very much for your help, it all does make sense to me and youre right i tlooks like my body is functioning well but the stress aswell as my age seems to have triggered the estrogen to go out of control.

The doctor told me to go on only one primulot tablet a day for 2 weeks but you re right if anything it should be higher but worth trying Minera much gentler on th system.

JUst as your body had to adjust to a lot of shocks during menarchae, it has to do this as the systems wind down at menopause.
These are small hiccoughs, and with due patience and meticulous care, these can be managed.

Dear Dr XXXXXXX XXXXXXX

I think you re a very special lady yourself, its funny I feel as if Im a teen again going through these hormonal changes. The odd thing is that it happened so quick one minute I was just normal XXXXXXX then WHAM!

My blood pressure is back to normal again it was 70 over 130. I weigh 60 kilos and Im 1.70m tall so not overweight. Its as if my hormones are going for a fun roller coaster ride. I wonder if its due to stress.

Im glad everything will settle back again, hopefully soon. I asked my mum about all this and she was no help as she didn't experience any symptoms nor did my grandmother, and my grandmothers side from my dad , she died os diabetis at 50 so I don't know as I wasn't even born.

I sure didn't expect this shock to my system, I've always had everything more or less under control and been so fit! Life...its an interesting mystery tour.

Im off to my home country in Spain in a couple of weeks but sure I will definately ask you if I get worried again. Is panic part of hormonal changes? Im certainly panicking a lot ha ha ha

Yes, it is strange sometimes and uncanny, how we bond with complete strangers !
The menopausal transition is fraught with such ups and downs, it is truly a hormonal roller coaster ride !
But then it stabilizes in a short while, you just have to hang on !
The BP is fine now, and you are otherwise quite fit and without any complications, so dont worry, this should be okay.
It could be the fibroid causing issues, or the hormonal fluctuations, but menopause will ease both.
Meanwhile talk about the Mirena insertion.
Did the bleeding stop, or is it under control.
Do not get off the Primolut without another alternative in place, else you might face heavy bleeding.
Once the bleeding eases off, remember to get a Pap smear done.

The bleeding hasn't subsided even after taking Primolut 3 times a day, Im starting to feel very light headed. I went to emergency here in Doha and after an internal examination she just thought I should be on Primolut once a day.

If I go to once a day Im going back to heavy bleeding... so frustrating!

I would like to take something to keep me going until I get to MAdrid and see a gyneacologist there


I wish I could stop the bleeding for a while and also find out what is causing the bleeding. What a puzzle.

Going back to Primolut once a day is only going to increase the bleeding.
Did a OB - GYN see you ?
And what was her logic of tapering off the dosage ?
And you should get your hemoglobin levels checked too , with heavy bleeding you might be getting anemic and losing too much of blood.
That should be looked into.
You can try taking Trapic - MF three times a day in addition to the Primolut N 3 times a day.
Heavy medication I know, but desperate times call for desperate measures.
You need to get the bleeding under control somehow.
I now am starting to think - I hope it is not your fibroid acting up.
Hormonal imbalance does respond nicely to Primolut thrice a day.
And you have not had a Pap smear, I hope there is nothing on the cervix that is causing such heavy uncontrolled bleeding.
These are all possibilities that I am talking about.
Of course I cannot be certain unless I examine you.
A last desperate resort is having a diagnostic and therapeutic D and C - cleaning of the uterus, it will scrape off the lining temporarily, leading to stemming of the bleeding, and will yield some endometrial tissue too, that can be sent for a biopsy to know what is happening .
This procedure is also combined with a Mirena insertion at the same sitting.
I think this could be done, if adding Tranexamic acid does not help.
Please get the hemoglobin done, you might need iron supplements, you cannot just bleed on this way.
