Periods Started After Unsuccessful Pipelle Procedure. How Would UTI Influence The Current Period?

default
Posted on Mon, 13 Aug 2012
Question: apart from what I put in my previous query, I have started my period after the unsuccessful pipelle procedure , on Monday 16th July and I still have it, generally a lot less, but this time is proper blood discharge.i have got a UTI infection at the moment and I also have problems with my BP. On 1st Aug I am provisionally booked for the endometrial biopsy, hysteroscopy and +/_ XXXXXXX coil. On the latest scan, they found a thickness of the endometrium of 11 mm , a calcified( asymptomatic fibroid) 4.5 -5.3 cm) and my uterus being retroverted. previosuly I had 2 children, one born by caesarian, and the second one by natural birth, after the latter I have been stitched. At the age of 47 I have missed a period just like that, then they came normally for the whole year, but then the subsequent yaers I have been diagnosed with mennhoragia, due to irregular bleeding and one period(at age of 49) which did not stop, until taking Utovlan-it lasted for about 2 weeks, before the GP put me on Utovlan pills, which from the leaflet, can give side effects including spotting and irregular bleeding., not sure whether this could have caused irregular, prolonged bleeding alone or with other factors.. In XXXXXXX 2011, I was referred to my GP to have a scan, in view to being diagnosed with mennhoragia. On the scan , it showed a bulky retroverted uterus , and a polyp at the fundus of 1.5 cm big. They offered me to have a polypectomy, hysteroscopy, endometrial biopsy and +/- XXXXXXX coil, but said no then, as I was really scared of the procedure especially if they proposed to do it under general anaesthetic. A year later( on 16th July 2012) they performed another scan , where they didn't see a polyp( their theory is that it could have been shed through bleeding), a calcified fibroid(the one I have mentioned before) and a thickness of the endometrium of 11 mm.(that was before I started my period on 16th July-before that periods were quite heavy, the first 3 or 4 days, and the last period I had was on 2nd March 2012, then had no bleeding at all until mid XXXXXXX ish, then it started with very light flow, on and off, sometimes it carried on for 3 days for ex. then stopped for a few days, then started again, having absolutely no pattern at all. It's only after the pipelle, on 16th July that I started the period I mentioned before.
I am usually a healthy, active, v. stressed out person, leading a v. busy life, in the past I have breastfed my first born for 4 months, then the second child for 3.5 years, and gradually gave up. feeling awful at the moment,as due to my hypertension, I am not so sure whether I can withstand any general anaesthetic to have the procedure. My BP went sky high yesterday and the day before, 170/111 the highest, then it went down like a yo-yo, went to the GP out of hours yesterday and the GP said it's high, but not high enough to be kept there(they expected something like 250/150).. Anyway, I am feeling unwell at the moment, as the doctor put me on BP tablets, a diuretic(Bendroflumethiazide), 2.5 mg once a day, and Trimethoprim(200 mg twice a day for 3 days), today 22nd July is the last day of the above antibiotic) for my UTI infection. One positive thing is that now I don't have those pains that I experienced lately, around the kidneys area as well in the pelvic and abdominal area. For 3 days while I was taking these antibiotics and BP tablets, I felt quite unwell, nausea, lost appetite, sour taste in my mouth, headaches, now and again, and experienced shivers now and again. I used to take vitamins and supplements up until today, at the same time as taking these medications for BP and UTI.
My questions are:

How would UTI influence the current period that doesn't seem to stop?
Is it a good idea to take iron supplements to make sure I'm not anaemic?
If my current BP cannot be controlled by the time the procedure is scheduled, how am I gonna manage a general anaesthetic?
Is there anything I can take to stop this bleeding? i'm not keen on Utovlan to use every time the bleeding doesn't stop.
What can you suggest about the thickness of the endometrium 11 mm?(Surely that would have started to thin out a bit or maybe not)

Note: Since I started these BP tablets, I have changed them twice so far starting from mid-May and , with headaches, the doc then decided to switch to a diuretic(Indapamide 2.5 mg), which gave me v. unpleasant nausea. So I have now switched to Bendroflumethiazide 2.5 mg, But when i started the latter, I started to take the antibiotic Trimethoprim as well, to clear the UTI urine infection ( I am not sure which one of them gave me all these shivers, nausea, headaches, loss of appetite, and today I started to feel feverish as well( and tendency to get a bit hot, even though I haven't got fever as such.

hi XXXXXX,

welcome again,
Thanks for the query.

1. UTI usually will not cause menstrual irregularity, your continuous bleeding may be dysfunctional uterine bleeding due to hormonal imbalance that may not be related to UTI.

2.yes, you can take iron supplements as you are having continuous bleeding,

3.as your BP is not too high as told by your doctors this much fluctuations can be generally effectively managed by the anesthetists, you don't worry about it.

4.as your bleeding seems to be because of hormonal imbalance better to take Utovlan which is a progesterone that can balance the hormonal deficiency, if you have any problem with the brand you can change to some other progesterone and along with that you can go for antifibrinolytic drugs.

5.the endometrial thickness of 11mm could be because of endometrial growth under influence of estrogen with no limiting effect of progesterone, that may be cause for dysfunctional uterine bleeding, so with progesterone supplementation your bleeding can stop.

Hope I have answered your query. I will be available to answer your follow up queries. If you are satisfied with all my answer, please rate the answer after closing the discussion.

Hello, Thanks for your answer

still need to ask:1. These antifibrinolytic drugs or even Utovlan or equivalent, are they giving any side effects like development of malignancies or something nasty like that?Usually HRT is known to have breast cancer side effect and others. Obviously depends on the woman.
2. Because my endometrial biopsy has been scheduled on 1st August, I asked the doctor then about Utovlan, and she said it's not good for long-term. Are there any equivalents to Utovlan?(any names if possible)
3. Now I am concerned about UTI , as it makes me feel unwell, weak and hot(had to check my temperature, but haven't got any atm). It doesn't stop me from doing the daily activities but it makes me less energetic and a bit unwell. At the moment I am on an antibiotic for one week, to clear this infection(it's a penicillin based antibiotic called Co-Amoxiclav 250/150 mg, hopefully it will work, as the previous antibiotic that I was on for 3 days didn't work,it made me unwell and had to go to see a doctor yesterday about it and I was prescribed this new antibiotic. Last night I was shaking all over and was extremely anxious and couldn't go to sleep till 3 am. Any advice on that?
The doctors said it's because the UTI. Can UTI give such symptoms of unwellness, fever tendency weakness, lack of appetite,as I normally experience good appetite, which I do have usually?

Thanks

hi XXXXXX,

1.HRT usually contains estrogen and progesterone that may sometimes increase the risk of few malignancies but Utovlan contains progesterone only that is usually not related to malignancies and antifibrinolytics also not associated with these.

2.any medication for long term is not good, alternatives to Utovlan are medroxy progesterone containing drugs like deviry, meprate or other drugs like regesterone etc but anything to be used with doctor's advice only.

3.you can continue the antibiotic, take plenty of water, if you are having fever better to go for anti-pyretics.
it is true that UTI can cause these type of symptoms, that may decrease after completing the dose, but if your anxiety, shaking etc recurs after taking next dose better to check for your sensitivity to that drug with doctor's advice.
if you do not find relief with this treatment you can go for urine culture and sensitivity once and take treatment accordingly.

Thanks for your answer.
1.Sorry, what did you mean by anti-pyretics, earlier on in your answer?

2.Actually, when i measured my temperature all day, it was normal, doctor checked it yesterday and it was 36.9 Celsius, so he said it was normal. and yet I do feel warmer than usual. Does it mean the start of a hot flush?because last night when I had the shivers, I experienced a bit of sweating on my legs mainly.

3.When I gave the doctor yesterday a urine sample, because I was still bleeding a bit, it was red, which diluted a bit as the sample was left to settle. But the doctor didn't seem worried, he said he could see signs of infection. Is there anything I should be concerned with?

4. And lastly, if I take Utovlan, when should I start taking it and for how long? My period now has started on Mon 16th July after the pipelle test which was unsuccessful.

Thanks

hi XXXXXX,

1.anti-pyretics are the drugs which are used to decrease the fever,

2.your feeling of warmer, shivering and sweating all are more probably because of UTI.

3.technicians will be aware of these common problems and they calculate and give results after correcting these parameters only,

4.as you are not having regular periods and having continuous bleeding you can start now with your doctor's advice, you can take for one week by that time your procedure date will come.

Wish you Good Health, If you are satisfied with my response please rate the answer after closing the discussion.

One more question I wanna ask you:

Actually my period has stopped last night,Tue 25th july. I took another sample, took it this morning. When the nurse dipped the stick into it(even though it was a clear lighter, pale yellow), she said she saw blood in the urine.
My previous urine sample taken, while I was still bleeding from my period), showed repeat test, and blood in urine, and white cells(pus), but not sure about the concentration, if applicable. From that moment, it just worried me, as I know blood in urine is not good.
The doctor then asked me to wait a few days to let the urine settle, then take another sample then.

Question 1: What likelihood is it for the blood in urine to be worrying in the two samples?(one sample was taken while i was still bleeding and the second one this morning, after a shower, after period has stopped).

Question 2: How come in the previous urine test result, the UTI was not confirmed and they think it might be a viral or some kind of infection somewhere else, due to the pus?

Thanks

hi XXXXXX,

1.normally blood in urine may indicate upper or lower due to contamination by blood from vagina it will create problem in diagnosing UTI,
may be after stopping your periods also your sample might have shown few RBC which are not visible to naked eye, that may detected by stick.

2.normally to detect UTI because of bacteria the number of bacteria should be more than 1,00,000/ml of one organism should be there, may be your urine analysis didn't show that much number of bacteria,
