Detailed Answer:
Hi Mr XXXX.
Thank you for posting your lab results and queries @ HCM.
I have gone through your lab results and understand your concerns,
before discussing about abnormal lab values,
I hope that you tested for H.pylori after eradication therapy. Furthermore, you did not mention "Metronidazole" , though its also prescribed with Clarithomycin and Omeprazole for eradicating H.Pylori.
H.pylori eradication requires "TRIPLE drug THERAPY".
Next I would discuss the abnormal lab values one by one.
1. Slightly High ALT (72.0) and very mildly High AST (43.0) and
High Triglyceride level
Firstly, the AST/ALT high values should NOT be of much concern as values twice the upper limits (>80) usually reflect acute liver injury.
However, if you have a history of HepB and/or HepC infection then you need to visit treating physician to get some tests done:
HBsAg (if not vaccinated against HBV) and antiHCV OR
HCV RNA PCR (if history of HCV)
Furthermore, "abstinence from Alcohol" is strictly advised and a very low fat diet and increased Triglyceride levels are maybe injurious to liver.
If your NON-Alcoholic and HBV and HCV are ruled out from your any previous lab results (not displayed here), then drug induced liver injury maybe a cause. Your using drugs to reduce weight and for a skin allergy. Discuss the drug dosage with treating physician.
2. High Blood glucose value(104)
the blood glucose level is slightly high. I hope you have done 8-hour fast or more, before doing the glucose test. if yes, then you might need a Oral glucose tolerance test (OGTT) and also a low sugar diet. Discuss with treating physician
3. Low T4 level.
at the earliest, Next step should be "testing of TSH" (thyroid stimulating hormone)
if TSH comes back high then your suffering from hypothyroidism and may be needing Thyroid hormone therapy (oral); dose subjected to your tSH levels.
Primary thyroidism is also a cause of weight gain
4. Low phosphorous and Vitamin D
You need vitamin D supplements straightaway.
Try to ingest Fresh cow's milk everyday and ingest mutton liver - fried in less oil twice or thrice a week.

I have some specifications to provide you with. My doctor did not prescribe me Metronidazole.He found the infection after the endoscopy in preparation for my gastric sleeve surgery.Regarding HVB and HVC I did the test recently and they were negative, and I do not consume alcohol at all.Also I would like to mention that I did not fast before my blood drawn in fact I had breakfast that morning with cereals and milk.

Firstly, regardless you were prescribed metronidazole or not, i advise you to get Urea Breath Test (for H.pylori) done as a follow up examination to be sure that H.pylori has been eradicated.
Secondly, if you were NOT fasting and had your blood glucose check, then there is NO need for OGTT.
Lastly, primary sclerosing cholangitis (PSC) is very unlikely indeed as most of others test are normal especially the bilirubin level is NORMAL. in PSC patients bilirubin is dramatically elevated.
Since your NON-Alcoholic and HBV and HCV are ruled out then drug induced liver injury maybe a cause. Your using drugs to reduce weight and for a skin infection. Discuss the drug dosage with treating physician.

Firstly, regardless you were prescribed metronidazole or not, i advise you to get Urea Breath Test (for H.pylori) done as a follow up examination to be sure that H.pylori has been eradicated.
Secondly, if you were NOT fasting and had your blood glucose check, then there is NO need for OGTT.
Lastly, primary sclerosing cholangitis (PSC) is very unlikely indeed as most of others test are normal especially the bilirubin level is NORMAL. in PSC patients bilirubin is dramatically elevated.
