What Causes Heavy Snoring, Difficulty In Breathing, Enlarged Tonsils And Irritation In Ears?

Question: Hello,

I need your help please, we live in Scotland and my son is going to be 2 years old this December. For several months he has been suffering from bad snoring and difficulty in breathing while asleep, big tonsils size, and high wax volume and irritation in his left ear.

For long months here is Scotland since he moved here since last March (he used to live in Egypt since he was born untill last March), he has been catching diseases specially when he started going to the nursery, he got the croops twice and was given steroid to easy it down, also he was always coughing most of the time, temperature high with other symptoms like less eating or throwing up on a regular basis, to the point that he was prescriped at least 4 courses of antibiotics (Amoxicillin) in a period of 5 months.

But during all this time no real diagnosis of why all these symptoms were showing was identified and we were told that it is normal viruses in the air, and his ear has an increased amount of wax but that is normal and it will go away eventually.

Now he is back to Egypt and we decided also to perform some examinations on him, the following was discussed with the doctor:

1- his tonsils were found out to be big in size, first the option to reduce its size with laser surgery was discussed, but then a blood test was performed and it was found that (Streptococcus) level is high (please refer to attached blood test report), the doctor said this might cause him (Rheumatic heart disease) and he recommended to remove the tonsils ASAP.

2- his adenoid was checked by x-ray and found out that it needs to be removed also the cartilage was found to be in big size that needs treatment, and the doctor said it should have been treated long time ago, but unfortunately no one diagnosed it (please refer to attached x-ray report).

3- an ear test was done (please find attached report) and the doctor said there is high volume of wax and volume of water in the ear that also needed to be treated long ago, but now we will try drops in his ear for ten days otherwise if it doesn't get better then they will have to insert some kind of tube to reduce that water in his ear.

Please I need to know what is your opinion looking at the attached reports, as now I am in front of two completely different opinions between the doctors in Scotland and in Egypt. I don't know if here in scotland these issues were ovelooked and underestimated and not treated accordingly, or is the doctor in Egypt exaggerating about the current condition.

Your help is very much appreciated as now I am very worried about my son.

Hi...I understand your concern. It could be recurrent upper respiratory infection, mostly viral along with large tonsils which is causing upper airway obstruction and sleep disordered breathing.

I do not see an images of the reports. It would be really helpful to explain further if you can upload them.

Questions:
1. How many days per month does he cough or feel breathless or snore or have continuous sneezing and runny nose?
2. How many nights per month does his sleep get disturbed due to above symptoms?
3. Does he feel breathless when he runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough and this cold and runny nose always associated with fever?
7. Does he have day time sleepiness due to disturbed sleep at night due to big tonsils and obstructive symptoms?
8. Does he have an elongated face?
9. Just see his oral cavity and check whether his tonsils are so large that they are touching each other?
10. Currently are you in Egypt or in Scotland?

Please get back to me with the answers to above questions and I will give my opinion based on both information and reports.

Apologies for the delay in the answer, but I was having some issues with the application on my phone.
Please find below the answers to your questions:
1- for the snoring and difficulty of breathe while asleep I would say 80% of the month, it is also visible in his difficulty to eat since he needs to breathe from his mouth. The coughing and the runny nose I would say around half of the month. We had a period where he was continuously coughing with fever of almost a month and a half after he started going to the nursery.
2- his sleep is disturbed due to the above almost half of the month.
3- No.
4- No, not only seasonal changes.
5- for Asthma I used to have it (his father) when I was young until now I sometimes suffer from it, but it is not a family history for me, it is something I gained when I was young, having said that can I inherit him this, since it is not genetic rather than something I did gain when I was young and no one else from my family had it ?
6- the cough is not always associated with fever.
7- Yes.
8- I believe not, but I have also sent a photo to the attachments email so you can have a look.
9- yes they are touching each other.
10- he is now in Egypt.

This is upper airway resistance syndrome - needs intranasal steroids

Hi....this is upper airway resistance syndrome or sleep disordered breathing.

If I were your pediatrician my suggestions would be this -

1. Intra nasal steroids (Flomist)....2 sprays in each nostril for 4 weeks.

2. Tablet Monteleukast 4mg once daily in evening daily for 4 weeks.

3. Take a video of his mouth breathing and snoring today. Take a repeat video again after 4 weeks. You will find a noticeable difference.

But all these are prescription drugs and the technique of intra nasal steroids has to be taught.

If no response then we can go ahead with surgery.

Hi.. I have reviewed the attachments and I again reiterate the same suggestion of going ahead with a 4 week treatment of Intranasal steroids and then decide on the next step of management.

Your kid is having slightly elongated facies and it is tell tale marker of upper airway resistance syndrome.
