What Causes Excessive Sweating And Nausea?

Question: Hi
this is a bit long but I`ll try and give you the whole picture. I`m having bad early morning sweats and nausea. The sweating is so bad that I have to change my night clothes as I`m soaked through. The last few days have been extremely bad and the nausea is lasting all day.
I was diagnosed with Graves in May and had my thyroid ablated in October, I`m now underactive and taking 75 mcg levothyroxine- my next blood test and an increase in meds is in a week`s time. Also I`ve been seeing my doctor to withdraw from temazepam but am not due to see him for a few days. He thought the nausea and sweating may be withdrawal symptoms and prescribed me 15 mg temazepam and 4 mg diazepam to take with one cyclizine hydrochloride every night - and the anti sickness pills again twice during the day. This has reduced me from 20 mg temazepam. I`m just finishing my second week of the withdrawal. To begin everything was ok, I had minor sweats and nausea but expected some withdrawal. I also take 2 tramadol for back pain with paracetamol - and then more paracetamol during the day as I need them. I had a bad nausea day y`day so had an early night with the temazepam etc..woke an hour later to more nausea! Went back to sleep and woke this morning at 5.30 am, took the levothyroxine and 30 minutes later the sweats and nausea began. I`m getting the usual underactive symptoms, joint pain, dry hair , sluggish- yet I`m having diarrhoea too. My endo is happy for me to increase my levothyroxine after my next blood test, based on the result and symptoms. My GP has looked at all my blood results and my TSH is 6.00 (0.35- 3.50) T4 14 ( 8-21) - apart from that he says everything looks fine. I`m getting concerned that this could be something else that`s serious

I meant to add, I used to take tramadol for pain before Graves was diagnosed but after the thyroid ablation I wasn`t in so much pain and withdrew them down to 2 per day as opposed to 6 or 8 a day. I noticed last week I had taken 4 to 6 tramadol daily again because of the thyroid joint pain. I reduced them down to 2 again yesterday- wondering if they are the cause? I haven`t taken any tramadol today. I believe the levothyroxine is helping get some thyroid function back. I have only been taking the 75 mcg for 3 weeks but my endo is happy for me to have an early blood test late next week with the view to raising it to 100 mcg.

I have read your detailed history and I can understand your problem. You have been diagnosed with Graves Disease and used to take Tremadol for pain. Later thyroid ablation was done and you were advised to take 75 ug of Thyroxin. You are also on withdrawal from Temazepam and are on 15 mg of Temazepam and 4 mg Diazepam. Now you have intense sweating and nausea especially in morning hours. Due to pain recently you have started taking Tremadol again. Your current Thyroid levels are TSH 6 (0.35-3.5), T4 (8-21).

Now first of all I would like to tell you that high TSH denotes hypothyroidism. Although you are taking 75 ug of Thyroxin still TSH is coming in higher range so probably the doses are not sufficient. I would advise you to get your Functional T3, T4 levels in next visit so that exact concentration of Thyroid hormones can be found. Poor thyroid function is probably one factor that is potentiating the nausea. You are also having symptoms like joint pains, dry hair, sluggishness which could be explained by low thyroid levels. But the diarrhoea and sweating are also there. These symptoms are probably occurring because of Thyroxin doses. Due to peripheral effects of thyroxin you are having such symptoms.

You are withdrawing from Temazepam and nausea can occur due to withdrawal but you have been prescribed Temazepam again and you suffered mild withdrawal symptoms and you have also associated co-morbid features that are potentiating nausea. So as per my opinion the withdrawal of Temazepam is main reason of your nausea, sweating and anxiety. I will suggest you to consult your Psychiatrist again to increase the doses of temazepam to original doses till your thyroid functions become stabilised. Due to associated problems you are experiencing a little severe withdrawal.

Other reason for your severe nausea is Trmadol. Tremadol is associated with severe nausea. You have started taking Tremadol again and this has aggravated severe nausea.

So just to sum up-

Poor thyroid along with thyroxin doses, tremadol are aggravating the nausea. This nausea is potentiating the withdrawal of Temazepam and you are having severe nausea and night sweats. I will advise you to consult with your doctor to increase the dose of temazepam at least till your Thyroid get stabilised. This should improve your nausea, sweating and anxiety if any.

Thank you for your reply,
I intend to quit the tramadol, will I need to taper them? I have taken 2 today and the nausea seems to have calmed a bit.
I realize I`m hypothyroid and have a blood test booked for later this week but I`ve only been taking my dose of 75 mcg levothyroxine for 3 weeks- is it too soon to raise this to 100 mcg.

Yes to quit the Tramadol you have to reduce the doses of tramadol gradually over 6-8 weeks with about 20-25% reduction each 2 weeks. But as you are currently having other co-morbid conditions so I would advise you to hold the reduction for some time now.

Yes as you are having some symptoms of hypothyroidism and the TSH came high so as per my opinion the doses should be increased to 100 ug per day.

So please see your doctor and also get your tests done. Discuss the dosage ideas I have given you and see about the gradual tapering of the Tramadol. Right now, don't shock your system with any drastic changes.
