Question: Regular blood test showing BMI 38, Fatty acid level 55- up from 17. Almost diabetic- 45. Feeling execessive tiredness. Shortness of breath. Asthmatic as well.

Based on the brief history and the results of the lab tests given you seem to have developed Insulin resistance that has resulted in an increased fatty acid level and low glucose.

1. The normal body mass index should be in the range of 18.5- 25 kg/m2
Since your BMI is 38 (high) it is suggestive of Obesity Class II.

2. Increased fatty acid level and decreased blood glucose level is due to Insulin resistance.

Regarding the Insulin resistance, let me explain you in detail:

Insulin is a hormone produced by the beta cells of pancreas (an organ located behind the stomach) and release it into the blood. With the help of insulin, cells throughout the body absorb glucose and use it for energy.

When blood glucose levels rise after a meal, the pancreas releases insulin into the blood.
Insulin and glucose then travel in the blood to cells throughout the body.
The main action of insulin is to increase the uptake of glucose into muscle, fat, and liver cells thus decreasing the blood level of glucose.
It also stimulates the liver and muscle tissue to store excess glucose.
It also lowers blood glucose levels by reducing glucose production in the liver.

In insulin resistance, muscle, fat, and liver cells do not respond properly to insulin and thus cannot easily absorb glucose from the bloodstream.

One of the consequences of insulin resistance in the adipose tissue is that excess fat is released into the bloodstream resulting in increased fatty acid levels and also stored by liver other organs (skeletal muscles).

Owing to my busy schedule I could't attend to your query on time.

Insulin resistance (decreased insulin sensitivity) increases fatty acid levels in the blood. The increased delivery of free fatty acids to the liver results in increased built up (storage) of fat in the liver. It may also occur due to excess intake of dietary fat.

Fatty liver is highly correlated with the metabolic syndrome which may increase the risk of type 2 diabetes and atherosclerosis (diseases of the heart and blood vessels).

You need to have periodic consultation with the physician and to rule out other possible causes of weight gain such hypothyroidism or other hormonal diseases.

You also need to have certain lifestyle modifications such as:

1. Avoid high caloric diet such as sweetened beverages, ice creams, chocolates, etc.,

2. Avoid fatty meals and red meat - You can take beans, lentils and soy — and lean meats. If you like fish, try to include fish twice a week. Limit salt and added sugar. Stick with low-fat dairy products. Eat small amounts of fats,

3. Increase the intake of plant-based foods, such as fruits, vegetables and whole-grain carbohydrates.

4. Regular aerobic exercise at least 30 minutes/day is the most efficient way to burn calories and shed excess weight, any extra movement helps burn calories.

5. You may need weight loss medication such as Orlistat if the above measures fail to yield a good response.
