Suggest Remedy For Excessive Hair Fall

Question: Unusual Hair Fall & allergy related issues.Attached are pics of my hand & scalp.

I have visited 2 different dermatologists ,they have suggested either surgical removal of pilar cysts on scalp,or leave it to its state. They did not guarantee of non occurence of cysts at same place again in future,so i decided not to go for surgery. After going through experiences of different people online started using Neutrogena T Gel. One of my pilar cysts (which was the 1st one i got to know 6 yrs back) slowly its size got reduced almost to the tiny size.

We shifted to different apartment community where found very hard water. I noticed more hair fall &my 6 yr old girl's hair also started falling and thinning. But tried using purified water for shower for my daughter. There was not much hair fall but have noticed slow regrowth.

Now,in XXXXXXX 2015 we moved to XXXXXXX from Minnesota(Snowy Place). Its been almost 5 months but my hair still fall regularly, The new growth of hair is much thinner from which my scalp is visible more than usual.

I visited dermatologist couple of months back,she suggested Rogainne foam for Women for 4 months, if it doesn't show any result ,she would then prescribe tablets. I've read online that people who have used Rogainne , experienced more hair fall initially and later some of them had hair growth but once stopped using it hair started falling again.

I truly appreciate your help & perfect solution in this regards.

Since we were in very cold place, 4 years back a small rash started on my right palm ,which gradually spread through my fingers and almost full palm. I've started using Aveeno moisturizing lotion. I'vd had different visits with different dermatologists,used
1. Hydrocortisone
2.Triamicinilone
3. Corticonozole

But no use. Here in this new state visited dermatologist ,she said it is hand eczema, it could be heriditary too & suggested Triamicinilone 0.1%. Its just temporary relief. I'm tired of visiting doctors again & again. I've noticed 1 thing that if i avoid dish washing & water related works ,i dont have itching. But i cant b away from putting my hands in water as i have 2 small kids & cant stop routine household work.Kindly suggest perfect solution in this regards.

I have gone through your query in detail and I have also reviewed the Images.

You seem to have Female Pattern Androgenetic Alopecia and I feel that Rogaine (minoxidil) is the right treatment choice for you.

Female pattern androgenetic alopecia is gradually progressive. This type of hair loss in females is androgen dependent.
Androgenetic hair fall in females is characterized by increased hair shedding and thinning mainly from the center (frontal, vertex and mid) of scalp.
There is decrease in volume of scalp hair due to hair shedding & miniaturization of hair follicles.
In androgenetic alopecia the duration of anagen phase of hair growth cycle decreases i.e hair does'nt grow as long as it used to, before it ultimately sheds and are also finer than before and therefore cover the scalp less and less well.
Thinning and hair loss manifests as widened central part width, initially and in later stages by diffusely visible scalp.

Treatment options in this type of hair fall are... topical minoxidil and oral antiandrogens like finasteride, spironolactone, oral contraceptive pills etc.
Finasteride and spironolactone are contraindicated in those females who are planning or could get pregnant while on these.
I suggest you to continue minoxidil/rogaine and one of the oral antiandrogens can also be started after discussing with your dermatologist.
Some people may initially notice an increase in hair shedding while on minoxidil but this is transient i.e during the first 2-4 weeks and usually subsides on continued use.

Besides, in the image I can also see some flaking on scalp and therefore I suggest you to use an over the counter antifungal shampoo e.g Nizoral which is 1% ketoconazole, every alternate days.

Regarding your second concern related to condition affecting your right hand I suggest you to upload an Image. I have not received images of your hand.
I will like to rule out a possibility of fungal infection/ tinea since the condition affects only one hand!! Hand eczema's are usually endogenous and therefore bilateral.

Sorry please find attached pics of my right hand palm , pics before & after applying Aveeno moisturizing lotion. It gets very itchy sometimes. I've noticed when its itchy very tiny bubbles(poppy seeds like) form red in color.Thank you.

Tinea mannum; oral and topical terbinafine

Detailed Answer:
Hi.

I have reviewed the Images.
I suggest a possibility of fungal infection i.e tinea mannum rather than eczema. Moisturizers won't help much.
I suggest Tablet Terbinafine 250 mg once daily for 4 weeks. Topically you may use Terbinafine 1% cream twice daily for 4-6 weeks.
These are prescription antifungals and therefore you need to talk to your doctor for the needful.
I am sure these would be effective for you.
An over the counter oral antihistamine e.g cetrizine 10 mg once daily will provide you symptomatic relief from itching.
