Question: Hi,

I am 57 year old lady, married with two kids. I have been suffering from XXXXXXX piles and gatritis for past few years. I have colonoscopy which was normal. I get frequent stomach infections.

Currently, suffering from hyperacidity, weakness, and pain in the anal muscles. I have a lot of burning sensation in the anal area. I take stz bath but things are not improving. Please relieve me of this pain

Thanks for writing in.
From your description you seem to be distressed by the symptoms.
I will answer all your health issues sequentially.
Regarding the gastritis and weakness. The anemia is due to poor absorption of various nutrients as well as the frequent use of antibiotics.
I would suggest the following for these problems.
1. Stop antibiotics.
2. Increase the consumption of natural antacids namely bananas and cucumbers. Ensure you take a banana at breakfast and after dinner.
3. Start a medication by name of sucralfate in the dose of one tsf three times a day after meals.(3 weeks)
4 start cap omeprazole in dose of 20 mgms one cap at bedtime.(3 weeks)
5. Increase the intake of XXXXXXX leafy vegetables as much as you can. Exclude XXXXXXX containing products.
6. Avoid spicey and hot foods including alcoholic beverages.
As a result of these measures your symptoms of hyperacidity ,gastritis and constipation will come under control.
For your anal symptoms I would suggest.
1. Application of local anaesthetic jelly to the anal region before passing stools.
2.After the act of defecation wash the area with soap water and then take a seizt bath for half an hour.
3. For seizt bath add half a XXXXXXX of magnesium sulphate powder and 2-3 tsf of dettol to the warm water in the tub. This will reduce the edema as well as offer antiseptic effect. Continue this for 2 weeks.
4. Cremaffin 1 tablespoon at bedtime.

Regarding weakness I would advise you to get a CBC (complete blood count) done.
Once the hemoglobin levels are available then I can advise specific food additives to correct it. Iron preparations at this point of time would aggravate the acidity issue. So I would advise you get the aforementioned symptoms under control . Thereafter in the second phase we can concentrate on improving nutritional parameters as well as improving the immunity of the gut so that you do not suffer from frequent infections and feel energitic as well.

Hi Doctor,

I am taking Sufralcate and sitz bath as directed but od no relief.

My concerns
1. Whatever I eat gets turn into gas , I get feelings of stomach bloating. As a result of which, I experience pain in my rectum. This pain is intensified more when I pass motion . A small lump comes out while passing motion with pain and I take sitaz bath which gives temporary relief.

2. I had been taking flagyl for quite a while. It reived my anal pain but all that was temporary.

3.I have been suffering from gastric and hyperacidity for the past 10 years. Doctors ptrescribe antacids, Omez and stool softener. It does not help my pain.

4. I dont get proper sleep . I have to take Lonazepo 25 every night to get proper sleep. I am anxious about my health and fear surgery. I require releif from this.


I have uploaded my blood test and stool test reports. I am aneamic. Please help.

I am 57 years old, 47 kg, underweight and 5 ft in height.

I have seen the reports sent by you. From the reports it is evident that you have a low hemoglobin. there is no evidence of any infection . The stool report does not cantain any evidence of blood in stools. So overall no significant problems.
Now regarding your surgical problems I will answer all your doubts sequentially.
1. It appears to be a prolapsed or a third degree pile. As you are experiencing pain there is a likelihood of it being thrombosed. This would then require surgical intervention as it requires to be removed.
2. For the gas issue I would suggest carminative tablets like Tab Dimol to be taken in dose of 4to 6 tabs at bedtime. Note they have to be chewed.
3. Stop taking flagyl. It will not help in your case now given the impression that the piles are prolapsing.
4. You can continue with the stool softner but do not take antacids for a long time. Instead just use sucralfate in dose of 1 tablespoon after every meal.
5. I understand that the thought of surgery is distressing to any one. But do not worry. With the advent of excellent anaesthesia methods pain control is not a problem at all. You will hardly experience any problem if you are to undergo surgery. Do not take anxiolytic medications for a long time. they are addicting which is very bad for health in the long term course.

Hi Doctor,

Symptoms :

I am taking Sufralcate and also applying that anaesthetic jelly regularly as directed.
After taking Sufralcate - it gives me relief for a while, but when I usually wake up after sleep often get sour taste in my mouth and if in case by mistake I eat heavily, I also get vomiting tendencies.
Decrease in the appetite is fairly common, ( prefer not to have breakfast and dinner, just forcefully make myself eat)
Tongue is white colour coated, often get mouth ulcers twice in a month - usually take Vitamin B capsules to get relief. It takes 6-7 days to get relief from that pain
I often get heart burns accompanied by burning sensation on the top portion of my head after having dinner( or if in case I have eggs, chicken, milk or oily food)
After having dinner, I usually get bloating tendencies - abdominal regions usually remains very hot.
I started getting rectal bleeding since November ,2011 . It happens once in four months( 1 or 2 drops of blood flow - Colour- bright red, Never get blood in my stool, it usually happens in the afternoon)
Often feel heaviness on my head after waking up in the morning( do not get proper sleep during night , always get very light sleep )
Questions:

1) According to the following symptoms mentioned above, I kindly request you to suggest me the remedies for the same?

2) Major concerns: How to get relief from sleeplessness during nights ?How to increase my appetite? Why is this rectal bleeding happening if there is no infection?

From your symptoms I would suggest you to undergo upper GI endoscopy just to be on the safer side.
Once endoscopy rules out any abnormality then I would suggest lifestyle modification.
Colonoscopy has already been done and has ruled out any colonic lesion.
For the rectal problem I would suggest you to undergo surgical intervention which will cure your rectal problem.
Sleeplessness is due to stress of all these diseases. Once the surgical issues are addressed the sleep problem will automatically be resolved.
For improving appetite exercise is required. A walk for 30 minutes will ensure improved appetite.

Thanks for replying!! As suggested I am ready to undergo GI endoscopy. I have my concerns regarding the rectal bleeding ,

My Questions are :-

Why does this rectal bleeding happen once in four months? ( Specially during afternoon )

If there is no infection in the colon, why is this rectal bleeding happening? What is the reason?

What is the specific test for detecting this problem, what surgical intervention is required for the same?

I am suffering from sleeplessness past 20 years. I rarely get proper sleep once in a blue moon .I am sure because of sleeplessness, I am facing all these issues, how to come out of this problem?

Hi,
The rectal bleeding is due to piles. As and when there is a change in the rectal pressure there is bleeding. This is quite a unpredictable phenomenon.
Infection is the last cause for rectal bleeding.
A proper per rectal and proctoscopic examination by a surgeon would be the best diagnostic investigation.Excision of the pile would be the treatment of choice assuming it to be piles.
Insomnia is a complicated issue to treat. It will require opinion of a sleep specialist.
