What is Local anaesthetic?

A local anesthetic (LA) is a drug that causes reversible local anesthesia, generally for the aim of having a local analgesic effect, that is, inducing absence of pain sensation, although other local senses are often affected as well. Also, when it is used on specific nerve pathways (local anesthetic nerve block), paralysis (loss of muscle power) can be achieved as well.

Clinical local anesthetics belong to one of two classes: aminoamide and aminoester local anesthetics. Synthetic local anesthetics are structurally related to cocaine. They differ from cocaine mainly in that they have a very low abuse potential and do not produce hypertension or (with few exceptions) vasoconstriction.

Local anesthetics are used in various techniques of local anesthesia such as:

  • Topical anesthesia (surface)
  • Infiltration
  • Plexus block
  • Epidural (extradural) block
  • Spinal anesthesia (subarachnoid block)

I had a knee replacement 14 months ago, I still have significant pain, I had Pt for a total of 16 weeks. My surgeon now wants to aspirate for cultures, cell count and synovosure. I am told they do not use an anesthetic when they insert the needle to obtain the fluid. Why won't they give me a numbing agent?????

I'm having a colonoscopy next week. I have sleep apnea due to pain meds so I'm having MAC anesthesia. I looked that up and it looks likely I'd be awake for the procedure. That is unacceptable! I'd rather have general anesthesia if they're worried I'll stop breathing. I've had versid before and it didn't work and I'm already on a fentanyl patch. Hello I have been talking to a Dr. Chandrakant in XXXXXXX about my problem and I was wondering if I could get a second opinion on the answer that he gave me. He was very kind and I felt that he may have even given me a good solution but I don't know if I made myself understood about how reluctant and afraid of getting another operation that I am. I am a slow healer and each operation has either made me worse off than I was before or took me so long to heal that I lost a year of my life with each operation I just don't know if I can handle another unsuccessful operation. You have my ok to read our correspondence.

Sorry if my ipad messed my first paragraph but it is a new device to me.

Does this sound like a simple and correct path for me to take? Every surgeon that has operated on me has told me that the recovery would be simple as a few weeks to a month at the most! Only to have me right back where I started from or worse off than I was before a year later!

One surgeon (urologist) said that he had never seen anyone have the problems and mass of dead sperm in the (epididimis) canal only to call me back a few weeks later to tell me that he operated on someone else that had the same large amount of dead sperm cells and he told me he was fine. He also said that the 2 injections that stopped the pain ment that I was likely a good candidate for the reverse vasectomy clearing up my problem and that the shots would only relieve my pain for a few hours. Yet the surgeon that specializes in pain after vasectomy said that the shots would last a week or so and that would tell us if they should operate.

Why does one surgeon say that the nerve block would only last a few hours and the other surgeon says that it would last a few days to a few weeks. This is very confusing. Are there different kind of nerve blocks that will deaden the pain in the scrotal area? Why does one say a few hours and the other say a few days?

Is the operation that Dr. Chandrakant suggested to me really that common and simple? And to what degree of numbness do people experience? Will I loose 10% of feeling 20% ...just what kind of numbness can I expect in that area if I have the nerves cut in the inguinal canal?

Thank you if someone who has dealt with this specific problem has time to answer. I would so much appreciate a definitive and coordinated answer, but if it is not, i need to know.

Have injured upper back for 10 yrs. When i done it felt crack and fizzing up down spine for 2 secs. Could not move left arm for 1 month very painfull.
