Posted on Thu, 14 Jul 2016
Question: Hello doctor, I am 26 years old woman got married on Feb'15, I had got irregular periods on the month of XXXXXXX and Feb 2015 and have got detected with hypothyroidism in March 2015. From April 2015 to till date , I am taking Thyronorm 50 mcg each day morning on empty stomach and I am also taking duphaston 10 mg from day 22 to day 27 from the mensus of each month as advised by the gynecologist. and i am taking folxt and vitamin D3 capsules (coecoral) along with that since my vitamin d level is only 5 units , got to know on recent blood test taken before three months .
I have been trying to get pregnant from November 2015 onwards, but couldn't get pregnant. hence consulted a gynecologist recently, and I have been advised to get folicular study at day 14, but follicle did not rupture and i am not ovulating. but i have eggs on both the right and left ovaries they said.

I would like to know the reason why i am not ovulating. Can continuous intake of duphsaton prevent ovulation?? They have advised me to take meprate twice daily for 8 days (medroxy progestrone acetate) this month for bleeding. Is it safe to use in conjunction with thyronorm 50 mcg tablet ?

Thanks.

Hello and welcome to HCM forum. Thanks for writing int o us with your health qyiery . I have gone through your query and understand your concerns. Problem in ovulation is not due to duphaston as you are taking it only for 5 days before periods for withdrawal bleedjng ( day 22-27 of periods) . I would like you to have your TSH free T3 and T4 levels done, as many a times hypothyroidism is cause for an ovulation. Also you have to take more vitamin D whether you go to sun for 20 min or take vit D 60000units XXXXXXX such as Tayo K once weekly. Also there is no problem in taking meperate with thyronorm. Get back with Thyroid report , if they are normal then we will run other tests to see for your hormonal status as to why ovulatiinnis not proper. Thanks. Yesterday, we took thyroid test. TSH is showing 3.80 mcIU/mL. LH is 9.27 mIU/mL, FSH is 5.11 mIU/mL, Prolcatin 12.50 ng/mL. Do you need free T3 & T4 as well?

Brief Answer:
Ovulation induction advised

Detailed Answer:
This blood test is quite suggestive of polycystic ovaries and thus you need to get an organised infertility treatment in the form of ovulation induction drugs and follicular monitoring.
The problem in rupture of eggs can be taken care of by ovulation induction and also injection for growth and rupture of eggs.
I basically practice infertility so in case you have any query, please feel free to ask.

Is there a way to restore LH and FSH so that follicle grows, ruptures and releases an egg normally other than using ovulation induction drugs? When I took TVS last year, we saw an egg got released,so seems like between the 7-8 months something happened due to which ovulation is not happening I think. We are not in a hurry to have a baby right away. So, Is there any alternative to these drugs, such as any nutrient diet etc helping ovulation?

Brief Answer:
Natural precautions advised

Detailed Answer:
Alternatives by natural means are not going to change the overall tendency but weight loss is a very important and proven method of improvement of the problem.
Also indulge yourself in some good active lifestyle like gym, swimming or sports.
Take good amount of milk, green leafy vegetables and fruits which will give you protein and antioxidants.
Since you are not in a hurry for the baby you can very well try these measures for 5-6 months and get follicular monitoring done along.
All the best
