What Do My Lab Test Reports Indicate?

Question: My wife ( 30 years) had a throat pain 4 days back with light fever. She was prescribed Levofloxacin 500mg for 5 days and Azeethromymcin 500 for 3 days. After 2 days Fever went way and one day was without fever but yesterday she had fever of 102 degree. Then went away with paracetamol 650 MG. Fever is not constant. She has terrible running nose, cough and headache. Throat pain is gone. But High cough and blocked nose is very high. She was asked to to blood count, dengue and malaria. Dengue and malaraia were negetive. Her WBC count are slight low and hemoglobin was low. Platelette count is perfectly normal.Neutrophills is slightly higher than normal. RBC count is also normal.
Only WBC count is slight low and Hemoglobin is low (8.7gms/dl). MCV and MCH is low. She is prescribed IRON and Zinc medicine. She has weakness and headache, cough and blocked nose.

Can you please help what to do ?

I appreciate your concern for your beloved wife!
I have gone through the history and the attached report of lab tests done on September 6th, 2016.
She seems to be suffering from infection of the respiratory tract with iron deficiency anemia.
I would advise the following:
1. Let her complete the course of prescribed antibiotics.
2. Take Paracetamol tablets every 6-8 hrs if fever does not subside
3. To take oral antihistamines like Levocetrizine for the relief of cough and blocked nose.
4. She must also be encouraged to take steam inhalation, nasal decongestant drops for the blocked nose.
5. To continue the iron and multivitamin medications for her anemia.
6. To take meat, eggs, milk, green leafy vegetables to correct her low hemoglobin level.

Hope the suggestions given would be useful for her problems.

Thanks a lot for your quick reply. I appreciate detailed and most satisfactory answer. Glad that you study my reports and case in detailed. You took my tension away for a while.

Meanwhile in tension and in worry i was asked to test Widal typhoid for her and urine test. Urine test was negative and Widal test came positive showing 1:160. But my old aged uncle is also MD medicine said this report is useless and to early as there is no symptoms. He also said she cant have typhoid. Also she has not other symptoms of typhoid excepts fever irregular ( not constant).

Other thing i want to ask how long iron + zinc tablets will work to normalize hemoglobin level ?

Thanks for your reply.
1. First and the fore most, the Widal test is not a diagnostic test for typhoid fever.
A titre of 1:320 or more along with symptoms of fever, abdominal pain, constipation, loss of weight, red spots over the skin, coated tongue, etc is suggestive of typhoid fever. Beside a false positive Widal test can differentiate between a current infection from the past infection, it can also be due to the vaccination taken previously.
2. For the correction of anemia, the iron+zinc tablets should be taken initially for 3 months. An adequate or successful treatment shows an increase of 1 gm of Hb/dL at the end of first month. Thereafter, the treatment should be continued for another 3 months to replenish the body stores of iron.
3. Besides, every effort should be made to find out the cause of anemia like bleeding, inadequate diet, diseases of the stomach or intestines, etc and if detected should be treated accordingly.

thanks again for reply and answer.

she had another fever of 101.5 degree yesterday at the same time she had day before yesterday. Fever went away with paracetamol 650mg dosage. Then in morning to evening she feels absolutely OK without any complaint. But after evening she feels headache and fever comes. Also feels heavy eyes feeling. This has been going since last 3 days. Exactly at 6 to 8 pm fever comes once at time. It goes away in an hour with paracetamol then comes after 24 hours.

In report WBC count is not that much low..then why this infection is not being treated? Also other reports like RBC count and platlete count also normal. Only netrophils is slightly higher. How long will take to treat this infection?

Is this because if low hemoglobin?

1. Usually recovery from such types of infection would take 5 - 7 days.
2. Low hemoglobin may cause body ache, weakness or dizziness.
3. Give her Paracetamol every 8 hr, let her take plenty of oral fluids.
If fever doesn't subside, get her examined by her treating physician for further evaluation.
