What Causes Vaginal Bleeding With Blood Clot And Whitish Matter In It?

Question: Hello, I have a question.
I'm two days into my period and I just passed a blood clot that was thick and had some whitish clump in it. It was kind of rubbery. Is this normal? I haven't had sex in 2 months. And wasn't pregnant. This is the first time it happened.

Did you get this period after few days delay? What was the date of your previous menstruation?

Such white tissue with heavy bleeding suggests abortion. If you get Beta hCG test done within a day or so, it can tell you whether this was early abortion. Even after abortion, it takes about a week for this test to become negative. However, since you did not have sex in last two months, this possibility is very remote one.

The other possibility is the complete lining of the uterus (endometrium) getting expelled out as a big chunk. In such case also, usually there is heavy bleeding with pain in abdomen.

Would you also inform me whether there was any pain associated with this bleeding?

Since you did not have sex in last two months, the second possibility seems to be more likely. The best option is to submit the white tissue you have expelled for histopathological study in the laboratory. This will conform the diagnosis.

For the endometrium expelled out en mass, no further treatment is required. These are solitary events and do not repeat. If it repeats again, you will have to report to the Gynaecologist. However, I am again insisting on the histopathological examination of the tissue.

This tissue may be expulsion of small polyp from the uterus or the cervix also. Its laboratory examination only will clarify the dilemma. Please upload the reports of the histological study of the tissue.

Yeah its been two months or longer since the last time i had sex, also, i have had some irregular periods. I recently had one at the beginning of the month, it was super light and was brown and lasted a mere 2 days. This period seems more like my normal heavy flow minus the blood clot. I usually have clots but its never been like that.

I did have heavy bleeding (still do) and pain was present before the release of the clot. Afterwards i felt normal and small cramps here and there but nothing too painful. Just the normal bloating. I did think it may have been the uterine wall just clotted up. If it happens again I will save it and visit my obgyn as so send it for testing.

Dear Kagel,

Good you informed about the regularity and heaviness of your menses. This could be fibroid in the uterine wall or cavity, ovulatory menorrhagia or Polycystic Ovarian syndrome.

So the possibility of passage of some polyp gets strengthened. The very fact you are having cramps in the abdomen suggests that this is fibroid or ovulatory menorrhagia or epimenorrhagia.

I feel your thorough work up is necessary. This white tissue seems to be blessing in disguise to that you thought of seeking for medical opinion. Please get following investigations done:

1. Utrasonography to find out any tumour in the uterus or uterine cavity. This will also show the presence of endometrium in the uterine wall.

2. Hysteroscopy (hysteromicroscopy or endometrial biopsy for histopathological study). This will show any anatomical defect inside the uterine cavity and also the study of endometrium will reveal the endometrial pathology and hormonal balance or imbalance.

3. Blood examination for FSH, LH and Prolactin. Based on the reports the other hormonal profile may be requested for. Thalssemia could be complicating your bleeding problem. Please get an opinion of haematologist.

4. Get your complete haemogramme done. You might have lost lots of blood in this process.

If you are still bleeding, please take some measures to control the bleeding. To start with, you may take Tab. Deveri for 5 days (may require for more number of days) which will bring the bleeding under control. You may have to take the pills for longer durations depending on the control of bleeding. After stopping the tablets, you will get bleeding again after 4-7 days, which should be normal in quantity. However, if this bleeding is due to fibroid, then the bleeding may be heavy again. In that case you may have to take further steps, may be curetting of the uterine cavity or stronger hormonal injections. Please report to your Gynaecologist for this.

Irrespective of haemoglobin levels, please start some iron tablets and take good amount of proteins in diet. If you take non-vegetarian diet, meat and eggs will be good for you.

I would be rather keen on viewing the images and the reports after these investigations. So please upload them for my review.
