What Causes Blood In Urine, Vaginal Bleeding And Constipation?
Question: Hello. I am 60 years old and in very good health most of my life. My last period was 5 years ago. Last Oct. I had a VERY stressful week, and when wiping after urinating, there was a very light pink on the toilet paper. (One other thing, I have been constipated most of my life with sometimes a LOT of straining to have a BM.) The next day, I had a difficult BM and when I wiped after urinating at that time, there was red blood. I saw my internal med dr. the next morning. She did a rectal, that was fine, and a pelvic exam. She saw a small amount of blood around the cervix. Ovaries/uterus felt fine at that time. Pap in July was ok. She couldn't tell where the blood was coming from. She also noticed vagina was atrophied. Because I don't have insurance, she gave me the option of having a transvaginal ultrasound. I finally saved enuff to have it this Sarurday. I am VERY worried. I also wanted to mention that a couple times this week a few times I had a very mild dullish ache in my pelvis. The bleeding back in Oct was just spotting after that, lasting 5 days. Nothing since. I have gone from 150 to 200 lbs in 2 1/2 years after my father's passing. I am 5'7. What could I be dealing with?

I have read your query. As you have mentioned that, the pap smear was suggestive of normal cells and you have not yet done the trans vaginal ultrasound. That is more necessary.
See you had constipation also but the rectal examination showed no bleeding point, that has been ruled out.
Usually after the menopause, the estrogen and the progesterone are lowered, because of which the vagina is atrophied and starts bleeding.
Other causes that may cause bleeding or spotting are the overgrowth in the uterus like fibroid or polyp with bleeding points (as you have mentioned of pain in the abdomen. Or something hard may have rubbed against the vaginal tissue.
I would also suggest that you get a repeat PAP smear along with the ultrasound and also investigate for the HPV infection. Please go to gynecologist and get the treatment under her guidance.
Since you are on meds for anxiety and depression, it could have resulted in weight. I am sorry that you lost your father, but life has to move on.
You please try to relax and join some yoga exercises. And will also help in weight reduction

Thank you for getting back to me. I am dieting and exercising now. What do you think my chances are of any type of gynecological cancers? As in endometrial, ovarian, etc.? At this point after 6 months, wouldn't I have experienced more symptoms as in pain, bleeding or discharge? No need for the HPV infection. I know this for sure. Thank you

Since you had only one episode of bleeding there are very rare chances that you may be suffering from cancer. That is like one in hundred chances around the age of 50 to 60 years.
After menopause, the lining of the endometrium in the uterus starts getting atrophied. It is usually less than 3 mm in thickness. But if the woman is on hormonal replacement therapy or has endometrial cancer this lining is more than 5 mm. That is why we are doing the ultrasound so that we can measure the thickness of endometrium and decide the further line of treatment.
In case there is an ovarian cancer there will be an enlargement in the size of the ovaries. But I must tell you that ovarian cancer usually occurs in the younger age group.
There is one more thing these investigations are followed by further more tests for confirmation.
So it is better that you get the ultrasound done before thinking much further
And may be the bleeding may be due to only atrophic vagina or some polyp that is richly supplied by blood .

I have already told you that the chance of cancer with one episode of bleeding is very rare . Then i must tell sometimes due to a disease like leiomyoma which is benign and presents with heavy bleeding. This can only be assessed by investigations and cellular histology .
For your reassurance I must tell you that only 10 percent of the woman who present with postmenopausal bleeding can have cancer. And furthermore the chances increases with age. That means that chances of cancer are more in woman around the age of 80 years than around the age of 55-60 years . It is important to reassure women that only 10 per cent. Come what may, take monetary help. Please get the ultrasound done.
