Question: Dear Doctor
Last year, my GGT reading is 150. I don't drink alcohol, I put nuts, seeds in my oats in morning breakfast daily. I take lean meat, vegetables, fruits & drink lot of water daily. I have been taking supplement (Thisilyn Daily Cleanse) - which contains 1,443 mg fiber proprietary blend, 180mg Milk Thistle extract, 130mg Artichoke extract (leaf), 130mg tumeric extract (root) & 65mg dandelion (root) for 6 months and my GGT reading is still high at 103. Please advise what other better & economical supplement I can take, or what else I should do to lower my GGT faster. Thank you XXXXXXX

Brief Answer:
Some more tests are needed.

Detailed Answer:
Hello,
Thank you for asking on HCM!
First of all i would suggest you to find the cause of high GGT level. Alcohool consumption is the main cause of course and you have excluded it.
To judge further you need to check the whole liver function test panel such as ; ALT, AST, ALP, and bilirubin.
You have attached some test results but i would kindly ask you to do it again because it results empty.
It will be a pleasure to give you an appropriate answer regarding to high GGT level after the above results.
Kind regards,
Dr.Mirjeta

Brief Answer:
The cause of liver enzymes elevation should be found.

Detailed Answer:
Welcome back,

From the results reported i noticed that not only GGT is high but also SGPT and total bilirubine are slightly elevated.

Having into consideration the lipid profile, a steatosis of the liver may be the cause of these abnormalities but a viral hepatitis should be exluded. If you have Diabetes and are overweight, the risk factors for hepatosteatosis increase.

If you were a patient in my clinic i would recommend these tests;
-HbsAg ( to exclude hepatitisB)
-AntiHCV (to exclude hepatitis C).
-An abdominal ultrasound would be helpful too.

Without having a proper diagnoses we can not recommended to treat the GGT level with herbal or alternative medicine.
Looking forward to help you further.
Kind regards,
Dr.Mirjeta

Dear Dr Mirjeta Guni

I have NO diabetes issue, Blood glucose reading has been stable for the last 2 years at 85mg/dl
I am NOT overweight either, my BMI 23 kg/m2 (ht 1.66m wt. 63.4kg)
I exercise regularly, at least 3 time/week (jog, swim & gym)
Very conscious of my food intake, regular meals - no fatty meat, little sugar, salt, etc. (except do take protein drinks due to gym training)
I DO NOT smoke nor drink alcohol
Family history - None in my family (parents & 6 siblings) has any liver disease or sickness

1)HbsAg (to exclude hepatitisB) Year 2015 result - hepatitis Bs Antigen NON-REACTIVE, Hepatitis Bs Antibodies REACTIVE 145

2)AntiHCV (to exclude hepatitis C) - very unlikely to have hepatitis C (please refer to my Year 2015 blood report attached)

3)An abdominal ultrasound would be helpful too - Done in year 2015 Result: Liver is normal in size & echogenecity. There is no focal hepatic lesion
Gallbladder is unremarkable. The wall is not thickened & there are no gallstones seen. Intrahepatic bile ducts are not dilated, common duct measures 2mm.
The kidneys are normal in configuration and echogenecity. The right kidney measures 9.2 cm & the left 10 cm in bipolar length
There is no focal renal abnormality. No hydronephrosis & no renal calculus detected.
Spleen & imaged pancreas are unremarkable.
The tail of the pancreas is obscured by bowel gas. Overall comments: No abnormality detected

Since year 2005, I go for health screening once every 2 to 3 years, all the test are normal except borderline results for Cholesterol 207mg/dl (year 2016) and Triglycerides from 258 (year 2015) to 164 mg/dl (year 2016)

The only result that troubles me are the Liver Profile as follows:

Yr2005 Yr2008 Yr2012 Yr2015 Yr2016
1) Bilirubin (mg/dl) 1.9 2.5 1.9 1.69 2.3
2) Alkaline Phosphatase(mu/ml) 124 123 144 145 122
3) SGPT (ALT) (mu/ml) 38 35 38 66 48
4) SGOT (AST) (mu/ml) 26 34 25 36 30
5) GGT (IU/L) 90 67 105 150 103

In view of the above results, I have taking supplement Thisilyn Daily Cleanse since the beginning of this year. Despite that, the reading only show marginal improvement. Hence, would appreciate your expert medical advice and appropriate measures to take & recommendations.

Thank you very much

Best Regards XXXXXXX

Brief Answer:
There are other possible causes that should be excluded.

Detailed Answer:
Hello again XXXXXXX

From your report viral hepatitis that is the most common cause of liver damage is excluded.
The ultrasound does not show enlarged liver or steatosis. Anyway GGT, ALT and bilirubine level persist high.

You should talk to your treating physician about other causes of liver inflamation such as autoimmune hepatitis, hemochromatosis , Morbus XXXXXXX etc.

Another abdominal ultrasound to see the if there is any progression would be of help.
Wish you good health, feel free to discuss further.

Kind regards,
Dr.Mirjeta
