Suggest Treatment For Symptoms Of Gonorrhoea Infection

Question: Hi, I ended up with a late diagnose of Gonorrhea today at 6.5wks after exposure from a newly acquired partner after it was missed at 9 days post testing. Basically, my partner and I engaged on brief unprotected oral and protected sex. After the encounter a couple days later found my partner's ex had cheated on her and so 9 days later I decided to get checked out in case after hearing the news since I also had developed a very mild jock itch rash that morning in my left thigh crease and had scrotum itching with no rash there which was probably from shaving and stressing over the situation which the doctor looked at and said it was nothing and to use creme from cvs for treatment. Anyhow, hsv1, hsv2, syphilis, gon, clam tests all came back neg so didn't think much of it, and until wk 3 didn't have any std symptoms, but felt like I had a hemorrhoid at wk 3. Waited a week and added more fiber in my diet and it sort of improved, but started freaking out thinking worse case situation because this seemed very unusual as I never had one of those and went on a search for possible stds which in that week my jock itch came back but this time went from my left crease out to my inside thigh which was probably from pretty extreme stress/ sweating at this point and doctor prescribed an anti-fungal creme which it improved in a few days (4.5wks). Then on week 4.5 until now is when things really got weird I started feeling funny icy hot feelings in my the front of my knees, top of my ankles, shins, top and inner thighs, and had weird itchy rashes pop up on my arms that stayed and forehead briefly (less than 1 hr) and my scrotum and anus itched as well the hemorrhoid symptoms. Went to the doctor and he had no answer other than poison oak for my arms / jock itch for my scrotum after mentioning the std possibility, which at this time I was freaking out thinking hsv 2 from the icy hot feelings / hemorrhoid from research, so at 6wks (42 days) got tested for everything to get a decent hsv2 result to find results at 6.5wks being hsv1, hsv 2, syphilis, and clam were all neg and gon was positive. After 8hrs post treatment now everything seems to be already improving quick, but what I like to know does this seem only like a gon infection or hsv2 possible because with as much as I've been freaking out I would have thought this would have triggered a very noticeable outbreak? My partner is getting tested/ treated for gon asap to know forsure, but just like to get an understanding on all this here since this is all surreal to me being this is only my second sexual partner and she got a full panel test (hsv 1, hsv2, gon, clam, syphlis) at the start of the year all negative a month prior to ending her relationship with her ex and always used condoms for vaginal sex but not oral. Also, one more thing to add, after being informed about the cheating ex I had been checking myself constantly for anything and never had anything hsv notable (eg rash with blisters, ulcers, or superficial red dotted pimples) and all was seen by a professional provider eg jock itch symptoms described and also an ingrown hair 1wk after shaving at wk 3.5 on my scrotum (4.5wks post exposure) which was basically a painless single red hard bump formed on a hair follicle until I fussed with it but even then it only lasted a couple days.

To your questions:

Further, I would like to know if you have take test for HIV ?

-Yes, missed that but initial blood test included HIV and also did RNA quick test both negative.

Additionally, I would like to know in case if you have taken any medications for your symptoms.

-Yes, was treated with 250mg cefRIAXone (ROCEHINE) injection and azithromycin 1000mg oral for Clam and gon.


One additional comment I like to add on the rash on my arm and forehead, which is this most likely was poison oak since I had returned from being out of town and my dogs were running in the bushes and I had petted them after. Also, cortizone creme prescribed got rid of it in a day or so which is typical of the other 100 times I've gotten it.

You are prescribed the right kind of antibiotics for Gonnorrhea and Chlamydia.

Also Cortisone cream is rightly prescribed for the extremity rash.

Since the quick test for HIV is negative, you need not bother much and you seem to be safe zone.

Additionally, I would like to prescribe oral pills if antifungal medications like Terbinafine considering the residual jock itch on groin.

I prescribe 250 mg of the above drug to my patients for 14 days with such symptoms.

Please check with your physician if he shares my view and if prescribe these pills to you.

I will follow up about the antibotics. In the meantime I was prescribed both cortimozal creme 1% and nystatin creme for the rash, which would be best for treatment until then?

I will follow once seen. Also, wanted to note that post 24 hr treatment I have fairly signifcant rectum pain that's shown little improvement now since initial treatment. One question I have on this is based on my symtoms of icy hot feelings in my legs which is inline with tenditous, and also I had unusual rashing on my fingers and one on the backside of my arm that worried me as it didnt present well as poison oak and got me wondering based off of my late diagnosis if this was a dismentic infection since I also did felt alost sick in da before treatment. So, I was wondering if based off of this would you think this and if so if I am under treated and should I wait or seek immediate additional treatment.

Hi since treatment I have had pretty bad diarrhea that has been somewhat dark, now 48hrs with only a little improvement. Do you have any suggestions for this? Also, I plan to get checked monday to have a urine culture as well as blood work done since this was not done prior treatment for status of the infection.

Diarrhoea can be a self limiting side effect of antibiotics used in the treat of Chlamydia and Gonnorrhea.

Further investigations are needed only if diarrhoea does not resolve in next 48-72 days.

But in case if you are willing to take up tests which you mentioned, I also advise you to get a complete stool culture and sensitivity testing in case if diarrhoea does not resolve after 72 hrs.
