Suggest Treatment For Severe Itching On The Body

Question: my daughter is suffering from itchy skin all over her body for the past two days. Her first symptoms were a few bumps on her hand and legs when she got up in the morning which looked like mosquito bites. There were a few mosquitoes near her bed also. Afterwards, she began to feel itchy all over her body and later areas of swelling and redness were seen all over her body. We tried a few home remedies but none of them gave much relief for more than a few minutes.

Next day, our paediatrician prescribed the following medicines:
1. Rantac syrup(75 mg/5ml)-3ml twice a day before food for 5days.
2. Atarax syrup(10mg/5ml)- 4ml thrice a day for 3 days
3. Kidpred(Prednisolone sodium phosphate syrup)(5mg/5ml)- 8ml twice a day for 3 days
4. Calora lotion for applying on affected areas.

I would like to get my following queries answered urgently.
1. Now she has temperature also. She didn't have that when we visited the doctor. Is it ok to give paracetamol at night now? I plan to give hydroxyzine syrup after an hour or two.
2. If it is ok to give paracetamol, could you please suggest dosage suitable for her?

3. My daughter is 5 years old, weighs 16 kg. Is it ok to give Prednisolone at this age in the prescribed dosage?
4. Is Ranitidine syrup given in combination with Prednisolone?
5.she had throat infection before one week. she has her first loose tooth. Do any of these have any impact on her present condition?

Hi... I understand your concern. By what you say I feel that this could be an urticaria or hand foot mouth disease.

Skin problems or best diagnosed and treated after seeing them directly, so I suggest that you upload some images of the child, so that I can guide you better.

Coming to your queries one by one -

1. Paracetamol can be given in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

You can give hydroxyzine syrup even simultaneously with paracetamol.

2. You can give Paracetamol which is available as syrup with strength 250mg per 5 ml - at 5 ml every 4 or 6th hourly depending on temperature as suggested above.

3. The dose of prednisolone suggested is correct and you can give it at this age. But if it is urticaria it will be useful. If it is hand foot mouth disease prednisolone is not indicated.

4. Ranitidine is given because of its anti allergic properties in this case, please use it. It can be used in combination with Prednisolone.

5. The sore throat and the loose tooth, do not have any correlation with present condition.

Hi,
I had been giving hydroxyzine syrup(thrice a day) and ranitidine syrup(twice a day before food) from Thursday evening.
The swelling in her face -on her cheeks and on her forehead, is almost gone.
The itching has reduced to a great extent. Still she complains about itching a few times a day; but after applying calamine lotion, the itchy feel goes away.
The area covered by red patches have decreased, but the patches are still present all over the body, except on her face.
She is still getting slightly high temperature by the time for the next dose of hydroxyzine syrup and temp drops to normal range within half an hour.

We had been asked by our paediatricianto come for a follow up today, but the doc s on leave today. So we are confused what to do today- Wait for the doctor till Monday or consult some other doctor?

I would like to know if her condition is improving as expected after administration of these medicines.
I'm attaching a few photos of her skin condition now. Is further treatment necessary? Will we have to do any blood tests for further treatment?
