Suggest Treatment For Hair Fall

Question: Hair at Birth-> less hair
Hair shaved twice at the age of 1month and 7 months, and hair grew up very healthy, thick and silky.
I used to suffer from cold often and sometime throat pain due to which had lot of medicines for sinusitis and farengitis. Menses started at the age of 10 yrs 8months. Initial two months the menses did not start again and had to see doctor often till 1 yr. It was normal often till 17-18 yrs of age when my hair fall problem also started. But more problems started at the age of 21yrs when I was having 2 periods in a month. A test detected a no. of simple cyst which was cured with 3 months contraceptive pills. Everything was fine till the age 26 after marriage when I had late menses and doctor said it was because of I-pill. It really seemed so, because it was regular and normal till 27yrs, i.e., august 2013. I was transferred, and stayed in place where I used to be very depressed all the time. The water there was not good and some kind of chemicals was added like bleaching power,etc. I started having severe hair fall and dandruff, itchiness,etc. Visted a dermatologist on 23rd sep 2013. He prescribed some medicines which did not work and made my condition worst and I started having very less and brown discharge since november. Menses problem is still there along with the severe hair fall. Only improvement is in my mental condition because I am happy with my posting now.

Thanks for writing to us.

After going through your history and your reports, I feel that you need to have some more investigations like a complete hormonal profile, trans-vaginal sonogram, serum DHEAS levels, AMH count, test for insulin resistance and serum Prolactin.

Your reports are mostly normal except for the high ESR, which may be non-specific as a single investigation; and also the slightly lowered platelet count, which needs to be repeated. As your TSH levels are on the high normal side, it would be better if you can get the free T3 and free T4 levels done and if necessary test for anti-thyroid antibodies.

Though a definite conclusion can be drawn based on all the reports, I tentatively feel that you could be suffering from polycystic ovarian disorder. The menstrual irregularity, hair fall and the history of ovarian cysts is suggestive. Please do not worry, as once the condition and the specific hormonal defect is diagnosed, it is very easy to treat this condition and response is almost always good.

Please see a specialist in hormone disorders and also a gynecologist for further evaluation and management. Please maintain a healthy weight, exercise regularly and maintain a healthy diet. Try to do some relaxation exercises to de-stress yourself. After all, there is more in this life to enjoy than to worry and we only live once.

Thank you for replying mam, but can you please guide me if I don't see any doctor here but get help from this forum? Can you please give me an idea of the cost involved in the tests mentioned above.

Hello,

Thanks for writing back.

You need to get an estimation of LH, FSH, DHEAS, AMH, Prolactin, glucose tolerance test, glycosylated hemoglobin, urinalysis and complete blood picture; a trans-vaginal sonogram is also needed.

I think you should be able to get an endocrinologist and a gynecologist in Dibrugarh to help you with your investigations and further management.

The approximate cost of all these tests together may amount between Rs. 2500-3000, based on the lab.
