Is Creatinine Level Of 0.9 A Matter Of Concern?

Question: I have just had the following blood exams. Aldolase, ANA screen IFA, CBC and DiFF w Platelets, comp metabolic Panel. Free T3, Free T4, Hemoglobin A1C HIV, IMM ADM ONE VC, Immunoelectrophoresis Urine, Lyme Disease, Methylmalonic Acid, serum, Pneumococal POLYSACC, rheumatoid Factor, Sed Rate, TDAP Vaccine, Thyroglobulin Antibodies, Thyroid Peroxide, total CPK, TSH/Thy Stimulus Hormone, Urinalysis R and M, Urine Culture. The only ones I am waiting on are the Protein Electrophpresis Serum And Urin. This test for Multiple Myloma, this bothers me. All my blood counts on blood work 3 weeks ago was normal. My creatanine was 1.34 on that test but it is .9 on this test. Please evaluate.

Happy to hear that all your tests came out normal and you are awaiting for UPEP (Urinary protein electrophoresis) and SPEP (serum protein electrophoresis).

Multiple Myeloma patients usually presents with pain in ribs or vertebral column and continuous kidney failure, lethargy, weigh loss, low platelets with low heamoglobin, high sedimentation rate, positive RA factor and moderately high CPK level.

You must have told this doctor about pain in shoulder and arms or weight gain so he did all these tests to rule out PMR (poly myalgia rheumatica) has high aldolase and cpk level with positive ANA.Weight gain for hypothyroidism (T3 , T4 , TSH, thyroglobulin Abs, thyroid peroxidase, CPK). But as all your tests came normal so you dont have these diseases.

UPEP and SPEP show us two bands of M protein and G protein. M protein is for multiple myeloma which is a aggressive cancer of bones. G protein is for waldenstorm disease which is a sistern disease of multiple myeloma and is usually benign.

In both these disease the hall mark is continuing kidney failure and your kidney fresh creatinin is 0.9 which is completely normal + your CBC with differential of platelets are normal and you have no complaints of multiple myeloma as I have told you in the start. So in my experience your UPEP and SPEP will come normal.

Your urine culture was done to see any urinary tract infection that caused transient high creatinin, but as your fresh crestinin is normal with no fever so this rules out this disease as well.

So I assure you that all the signs of your previous tests show us THAT YOU DO NOT HAVE MULTIPLE MYELOMA.

I saw the 3 files of tests attached to your query. Your tests are totally normal , this is strange.Why did he tested you for UPEP and SPEP.Even a non-doctor can tell you that your tests are totally normal by seeing the reference range.

Why did he tested you for myeloma. What were the symptoms you had, that you told him. Doctors treat the symptoms and people not the tests. So tell in detail about your symptoms

This is a new doctor and I know he wanted to test everything, because I told him I have anxiety, mainly because of the many doctors that were trying to determine what was causing my abdominal pain. My endoscopy in January did find Gastritis, but no H pylori, and the search to find the cause began. Upper GI, abdominal ultrasound, Ct scan, MRI, MRCP, Hida scan. Also during this time I started to get back pain, then X rays of the back, and another Upper GI with follow through. Many blood reports, but not as extensive as this one. My anxiety has become overwhelming and I think this new doctor is wanting to rule out as many things as possible. I also told him I had tingling in hands and feet. Also two things in this process that was misdiagnosed. One was an Organaxiel malrotation and Prostatitis. And sadly, I do another test like this one, and I get more anxiety. Would the tingling cause a concern for Myeloma, and what can you tell from these lab reports? Also, he ordered the SPEP at the same time as the other tests.

Tingling sensation in hands and feet are the MAJOR symptoms of anxiety with gastritis.

I do agree that peripheral tingling sensation is a symptom of multiple myeloma and is mostly seen during the half way of disease and is not the presenting symptom.

Back pain also occurs in multiple myeloma but with your age ligamental strain with spondylosis may be the cause and is more common , besides your ESR is totally normal.More than 90 % of Multiple myeloma patients presents with low heamoglobin and ESR level of 80+.

If you had MRI of abdomen , in that your back bone is clearly visible and the lesion of multiple myeloma are lytic meaning bone destruction.

MRI abdomen do creates false diagnosis and is common.

Your thyroid levels are normal with normal TSh , people do present with peripheral neuropathy meaning tingling , burning sensation in hands and feet in thyroid problems.

Multiple myeloma do have high calcium level , your is normal , besides the BIGGEST CLUE TO NOT HAVING MELOMA is that you have normal protein level and albumin.In myeloma total protein is same but albumin decreases in ALL cases. ( CRP is normal so no infection like HIV , Syphilis , typhoid).

I can clearly say that you donot have Multiple myeloma or any of its sisterns diseases like Walderstorms or smouldering myeloma which are less aggressive.YOU ARE A HEALTHY PERSON.

Hope this alleviates your anxiety and nothing to worry about as i have explained you with all the logic.

Dr Omer, I just received the SPEP and the Myeloma results. It is negative. Your knowledge regarding my query was exceptional. Before I got the results and with your explanation I did recognize that Myeloma was not realistic. The doctor does want to do one more test for Peripheral Neuropathy to determine if my tingling is related. We do know it is not related to Diabetes. Do you agree I should take this test?

Happy to hear that your SPEP came negative.As i told you in detail that Myeloma patients do not present with normal blood reports.

Yes i do agree that you are not diabetic and your tingling sensation is not because of diabetes as your HBA1c came totally normal.

The test for peripheral neuropathy is Nerve Conduction Studies with EMG.YES you should go for this test although it is not that accurate but do give US the hint that a person have neuropathy or not.IF it comes positive then there are other causes of peripheral neuropathy aswell.

I still think that your peripheral neuropathy is more related to your anxiety.
