Hello Dr. Branch,
I’m hoping you can help me with a few questions I have.


7 days ago I felt like I was having UTI symptoms. I have constant burning “down there” and a constant feeling that I need to urinate. It’s not so much that it burns when I urinate. It just feel like it is burning all of the time. I constantly feel like I have to urinate. I will go and one minute later I feel like I have to go again. I went to the doctor last Tuesday. They checked my urine with a dipstick which showed possible infection. I was prescribed 1 dose of Fosfomycin. It didn’t work. I still had symptoms. The doctor (not my family doctor) seemed confused so she treated me for a yeast infection. That didn’t work either. I finally had a chance to see my family doctor yesterday. He prescribed Macrobid for me but had not sent the results of my urine sample from the lab. The results of my original urine sample (from Tuesday) came back yesterday afternoon. It showed UTI. I don’t know what bacteria I have. My doctor called to say the Macrobid was not as likely to work due to the sensitivity so he gave me Cefixime 400mg, 1 pill a day for 5 days and said the Cefixime was more likely to work. I took one pill yesterday afternoon and today I’ve had no change in my symptoms. I read online that some people find relief after being on it 24 hours but I didn’t feel any change. I took my second pill 3 hours ago. I still feel the same.

I have anxiety around my health so I may sound a bit paranoid here. First I am worried about having had this UTI for a week now and am worried it might be really bad. I am worried this antibiotic is not working. I am also VERY nervous that this may spread to my kidneys. I am not sure how many more days to give this before I see the doctor again. Am I expecting too much too soon from this antibiotic? I’ve bern told it’s strong but I don’t feel like it’s doing anything. Maybe I am a bit worried that I only have 3 pills left and nothing has changed.

My biggest fear is that this UTI has not been treated properly for a week and will spread to my kidney. Does it take very long for the bacteria to spread to your kidney?
How long should I wait to see the doctor again?

I am a healthy 42 year non-smoking female. I’ve had UTIs in the past that have always responded to treatment. I have no fever.

I read your story and I would say you may need to wait another day or two to really see if the antibiotic is going to work or not. It is true that many patients see a response within 24 hours, but I have seen many patients who still report symptoms after 2-3 days, and even longer, though you should be seeing some improvement by then. So I would suggest waiting another day or two, and see if you see any improvement. As you mentioned, it may be that since this has been going on for several days, it may take a little longer to resolve.

You asked a good question about what to look out for with a more serious kidney infection. Sometimes simple urinary tract infections can travel up the ureter and infect the kidney. If this happens, it most cases you will definitely know it: you will have spiking fevers, and pain in your back on the side. It doesn't sound like you have a kidney infection at this point, and hopefully now that you are on good antibiotics, you will not get one.

If after the 5 days of Cefixime you are still having the same symptoms, you should probably get your urine checked again and sent for culture. If you could find out what the bacteria was that grew before and let me know, that may help give me a better idea what is going on too. If you are still showing signs of infection, they may need to use another antibiotic. If you are sexually active and it is at all possible for you to have an STD, they should probably check for that as well. Finally, if it is shown that the infection is cleared, but you are still having symptoms, there is a condition known as Painful bladder syndrome, for which you may need to be referred to a urologist and have further testing for. But I would expect that this is an infection and that it will just take a while longer to resolve.

Hello Dr. Branch,
Thank you so much for your detailed answer.
Through the night I feel like my temperature is elevated but when I take my temperature, although it is higher than it usually is, it is still below 99.
I have found that when I am catching or fighting something that my temperature does rise but doesn’t always hit what would be considered a fever. I usually notice because my face and my mouth feel hot. I am having that through the night now.
When you said a kidney infection would cause a spike in temperature, about how high would that be?
I’m still kind of nervous that even though my temperature is not considered high that I am still running a slight fever.

That is a good question, we would consider a fever to be anything above 100.4 (even then it is considered a "mild" fever), so especially if you are below 100, I wouldn't be concerned. It is normal to feel hot when fighting off an infection, but as long as your temperature isn't high enough to be a fever, it is reassuring that the infection isn't becoming so severe that it is causing a systemic response from your immune system, such as would happen with a kidney infection.

I would keep checking your temperature whenever you feel hot, and if it did reach 100.4, that would be concerning at this point that the antibiotics aren't working, and you should see a doctor soon to be reevaluated. But I would hope and expect that you won't get a kidney infection and need to do that.

But if you took Tylenol (I would avoid using Ibuprofen, because sometimes it can hurt the kidneys), it would be fine, because with a kidney infection, as soon as the Tylenol wore off, the fever would come back, and they would check your blood at the ER which would show there was an infection.

It depends on the type of bacteria; sometimes it can happen quickly, within a few hours, and sometimes it takes several days, so there's not a typical amount of time that it takes to develop a kidney infection.

Thank you so much. I decided to be on the safe side and I came to the ER an hour ago. They took my blood and urine so far. Where I live we have a long wait to see a doctor so I could be quite a while.

My nerves are not too good. I am so worried.

So, will they be able to tell by my bloodwork if my kidney is infected? I didn’t know what they were looking for with my bloodwork. I would love to know the answer to that.

I still have no pain and my temperature is still not that high but I wanted to be better safe than sorry.

This might sound a little strange but I have a vomit phobia. I get waves of nausea but I am not sure if they are completely related to this or not. I think it’s possible this UTI might be upsetting my stomach but I wonder if my nerves are making it worse. I kind of get the sweats too but again I’m not sure if my nerves are involved. This is all causing me a lot of anxiety.

If I do have a kidney infection, how will they treat it?

Yes, nausea and/or vomiting can be a sign of kidney infection, but it could be from your nerves as well, so it is not a very specific sign, meaning it may be from other causes as well. But normally there will be pain with a kidney infection as well.

With the blood work they will mainly be focusing on your white blood cell (WBC) count. If you have a kidney infection, this will give a high WBC count, usually above 15-20 thousand. If it is normal, then a kidney infection would be less likely.

If you were worried it's always a good idea to get checked out in person, since there is only so much I can tell you by messaging here. Hopefully you can get a clear answer either way whether you have a kidney infection or not. If you do have a kidney infection, they often give IV antibiotics and keep you in the hospital, or, if they think it is not too bad, it can sometimes be treated with pill antibiotics such as ciprofloxacin at home.

She tapped my back where my kidneys are which felt fine. She also said my temperature is normal even though I feel like it is elevated compared to how I usually feel. I asked her about the waves of nausea I’ve had the last few days and why my temperature may feel a little high. She said she thinks it is unrelated and that I may have a viral infection that coincidentally came at the same time as this. That could be possible and I think with my nerves being bad, that may have made things feel worse.

She said she saw no indication that my kidney has been affected. She told me to follow up with my family doctor.

She additionally checked to see if I have a yeast infection and took a swab since I mentioned it feels like it is always burning in that area. She said it looked inflamed so she will have me use
Canesten.

I will take today’s dose of Cefixime and if I still feel like I have UTI symptoms tomorrow I will see my family doctor in the morning to see what he thinks and ask if 5 pills will be enough.

Also, I feel kind of achy, my stomach is still upset, although I do have moments where I feel hungry and I can eat, and my temperature still feels elevated. Would that be from the UTI? I know the doctor said she felt they are unrelated and that I may have a virus, which I can understand. There is a lot going around. I just wondered if it could be the UTI? I’ve never had symptoms like that with a UTI before. My stomach began feeling yucky on Monday, right before I started the Cefixime on Monday afternoon, so I am certain it’s not the meds causing it and the elevated temperature began yesterday. Maybe it is hard to say what it is for sure, but if it’s the UTI I hope it will feel better soon if the Cefixime is working.

I don’t know if you’d call it the chills or not, but I have had a couple of times today where I feel cold and I have to put on warm clothes and go under a blanket to try and warm up. Does that sound concerning to you? I just don’t know why I am getting all of these symptoms, especially today. They have really been the worst today.

Also, I am still having some waves of nausea. I have a prescription for Zofran that I was prescribed for a different situation. I was wondering if I can use it if the nausea gets worse? I also have gravol but was told the Zofran works better.

He said my bladder may still have inflammation and my vagina may have yeast and irritation. He said to come back next week when I am done the pills and cream if I still have symptoms. I told him about my fear of the infection spreading to my kidney but he said Cefixime is used to treat kidney infections so it’s not likely.

I think your results are probably because the fosfomycin usually treats klebsiella pneumoniae (in one study, only 19% of klebsiella were resistant to fosfomycin), so it may have killed enough of them that it made the culture negative. However, the lab your doctor uses may not have checked to see if the bacteria was sensitive to fosfomycin, and I assume the lab did check Cefixime and the bacteria was sensitive to it, so your doctor switched to Cefixime just to be sure, which I agree is a good idea.

I would say the negative urine culture was a good sign that the bacteria is being cleared and it will not spread. Klebsiella can be difficult to treat but since they checked to see what antibiotics would be effective, I wouldn't worry, and Cefixime would normally work.

I would expect that this latest urine culture will also be negative. Hopefully you still start to notice improvement in the next couple of days.

I asked what bacteria it is and the nurse said it’s Enterococcus. My doctor has given me a prescription for Amoxicillin 500mg 1 a day for 5 days. The report didn’t mention anything about the Klebsiella so I am really hoping it’s gone.

Although I’m not excited to have a UTI, I am grateful that it showed up and that they are trying to treat me for it. I am really hoping I will get better soon.

Something like this happened to me when I was pregnant with my son. For 5 months I had back to back UTIs, most of them asymptotic. Do you think it would be advisable for me to be on a probiotic? I’m assuming it would not help with a UTI but since I have been on so many antibiotics maybe it would help restore my good bacteria and help prevent a yeast infection.

Hi Dr. Branch,
I saw my doctor and a student doctor who is training with him today. They used the dipstick in my urine sample. The nurse said there was a little blood but no white blood cells. They are hoping the Amoxicillin is killing off the enterococcus. My doctor said enterococcus is supposed to be susceptible to Amoxicillin. They will be sending my sample to the lab and will let me know about the results. The results of the vaginal swab they took at the ER came back negative.

My doctor seems a little unsure as to why I am having the burning feeling and continued feeling of having to go to the bathroom except that I may have inflammation persisting from the infection.

He also seemed unsure as to why I’m having these back to back infections. However, he did say that once you have one infection and the bladder gets inflamed, it makes it more susceptible to other bacteria invading.

He told me to drink a lot of water. He said to finish the Amoxicillin. I finish it tomorrow anyway. He said they will see what my results show. If the infection is gone, they said to give it a week to see if my symptoms settle down and to come back again if they don’t. The student doctor suggested I may need to see an OBGYN if the symptoms persist without infection showing.

Anyway, I told them I am so concerned of this spreading to my kidney but they said it doesn’t look like that is happening. I think they feel confident that the Amoxicillin is likely taking care of the enterococcus but they will know more when my results come in.

Do you agree with their advice? My doctor had offered to switch to another antibiotic but said it is probably best to stick with the rest of the Amoxicillin and see what the lab results say. We can go from there. He doesn’t like that I am having to take so many antibiotics.

I guess I am still nervous that something is wrong and that there is still bacterial growth happening since I still have symptoms, and it will spread to my kidneys. That is my biggest fear. I worry that waiting a week after I’m done the Amoxicillin puts me at risk of the infection spreading.

They are hoping the Amoxicillin is killing off the enterococcus. My doctor said enterococcus is supposed to be susceptible to Amoxicillin. They will be sending my sample to the lab and will let me know about the results. The results of the vaginal swab they took at the ER came back negative.

My doctor seems a little unsure as to why I am having the burning feeling and continued feeling of having to go to the bathroom except that I may have inflammation persisting from the infection.

He also seemed unsure as to why I’m having these back to back infections. However, he did say that once you have one infection and the bladder gets inflamed, it makes it more susceptible to other bacteria invading.

He told me to drink a lot of water. He said to finish the Amoxicillin. I finish it tomorrow anyway. He said they will see what my results show. If the infection is gone, they said to give it a week to see if my symptoms settle down and to come back again if they don’t. The student doctor suggested I may need to see an OBGYN if the symptoms persist without infection showing.

Anyway, I told them I am so concerned of this spreading to my kidney but they said it doesn’t look like that is happening. I think they feel confident that the Amoxicillin is likely taking care of the enterococcus but they will know more when my results come in.

Do you agree with their advice? My doctor had offered to switch to another antibiotic but said it is probably best to stick with the rest of the Amoxicillin and see what the lab results say. We can go from there. He doesn’t like that I am having to take so many antibiotics.

I guess I am still nervous that something is wrong and that there is still bacterial growth happening since I still have symptoms, and it will spread to my kidneys. That is my biggest fear. I worry that waiting a week after I’m done the Amoxicillin puts me at risk of the infection spreading.

It occurred to me last night that I have had many UTIs over the years and several yeast infections (more UTIs though). I have never had a UTI that feels like this before. Usually when I have a UTI I feel like I have to urinate a lot and may have some feeling of burning but I have never had the burning this bad from a UTI before, where it is so constant and feels so sore to sit or even walk sometimes. I feel like I am on fire much of the time.

I have had yeast infections in the past that do not itch but burn and it’s been pretty sore and makes me feel like I need to urinate a lot. I would say how I feel lately is similar to that.

I know my vaginal swab showed no yeast infection, but when the ER doctor had a look she said it looked irritated. It certainly feels irritated!! I was wondering if it’s possible that all of this antibiotic usage is causing vaginal irritation? Is it possible for the area to be irritated or inflamed without a yeast infection? Could it also cause that feeling of having to urinate if it’s irritated?

I am not sure if I mentioned this in all of my messages but I was on Amoxicillin for 10 days at the end of May/beginning of XXXXXXX for strep throat. Two days after I completed the meds, the UTI symptoms (like the burning) started and I have taken 3 more antibiotics since then and the burning has not really stopped except for a couple of days after I stopped the Cefixime but then I was back on Amoxicillin.

Anyway, I know I have definitely had 2 UTIs in the last two weeks but I’m wondering if some of the burning could be from vaginal irritation?

However, if you do start to have severe diarrhea, fever, or severe abdominal pain, I would stop and let your doctor know. But maybe you are almost finished at this point.

An ovarian cyst big enough to push on your bladder and cause symptoms would be very rare. However, if these symptoms persist, getting a pelvic ultrasound may be appropriate anyways, so that would at least determine whether or not you had any ovarian cysts that could be doing this.

However, with your description of the burning being the major symptom, I would think that vaginitis could definitely be a possibility as well. It sounds like the initial tests from the ER were negative. The next thing I would want to look at is the pH of the vaginal swab they did, if you have those results. That can help determine whether it might be from vaginal atrophy or a skin condition such as lichen sclerosus, or a dermatitis. Is the burning you are having all the time, or mostly when you urinate? There is a condition called interstitial cystitis, or painful bladder syndrome, that may also be possible if the symptoms persist without a clear cause. But it is still too soon to start thinking about that.

Hopefully you are almost done with the antibiotics and seeing some progress by now. Please let me know how things go, and any other changes in your symptoms or test results.

I took the Zofran about 7 hours ago. 2 hours ago I started to notice the nausea coming back again. The Zofran bottle says to take 2 a day. Does that mean I need to take them 12 hours apart? If so, is it possible to have a gravol in the meantime?

I have not had any diarrhea since last night but sometimes I feel a little crampy. Should I take another Pepto bismol as well or just leave it alone? I don’t want to feel sick but I also don’t want to keep taking so many meds.

Hello, I'm sorry you are now having to go through this. But usually diarrhea from antibiotics goes away within a couple of days, rarely up to a week. Did you ever end up getting those probiotics? That might be a good thing to try to help clear this up.

C. diff is always a good thing to think about whenever someone gets diarrhea with antibiotic use, but usually you will have several (at least 3) episodes of watery diarrhea in a day. If you do start to have more frequent diarrhea, I would suggest seeing your doctor to get your stool and blood checked to make sure it isn't C. diff; otherwise, I don't think you need to worry about it.

If you are taking the 4mg of Zofran, it would be safe to take it every 8 hours; if you are taking 8mg, then every 12 hours would be better. But yes, it would be safe to take gravol in between. Pepto-Bismal is also a safe medication to take and if you are having cramping, I think it would be fine to take it.

Hello, I'm sorry you are now having to go through this. But usually diarrhea from antibiotics goes away within a couple of days, rarely up to a week. Did you ever end up getting those probiotics? That might be a good thing to try to help clear this up.

C. diff is always a good thing to think about whenever someone gets diarrhea with antibiotic use, but usually you will have several (at least 3) episodes of watery diarrhea in a day. If you do start to have more frequent diarrhea, I would suggest seeing your doctor to get your stool and blood checked to make sure it isn't C. diff; otherwise, I don't think you need to worry about it.

If you are taking the 4mg of Zofran, it would be safe to take it every 8 hours; if you are taking 8mg, then every 12 hours would be better. But yes, it would be safe to take gravol in between. Pepto-Bismal is also a safe medication to take and if you are having cramping, I think it would be fine to take it.

I know that we are pretty sure the nausea and diarrhea I experienced are from either the antibiotics or a flu and not my kidney. I had no nausea until the diarrhea so I’m sure they are connected. Also my UTI symptoms have improved a little and are not getting worse.

However, I am a worrier (as I’m sure you noticed!). Since this may be a flu I know I could get a fever. If I get a low fever and have no pain, do you think it’s safe for me to assume it’s not likely my kidney? I know if I get a fever, even a low one, I will start to get panicky but I would like to keep things in perspective and try to avoid a trip to the ER. Would it be better to just wait and go to my clinic in the morning if I get a low fever (with no pain)?

Do you think it’s possible that not eating may have made the nausea worse? I was scared that if I ate, then the nausea would have got worse, but maybe the opposite was true.

When I woke up in the night initially, I thought I had reflux because I could feel something coming up but when I sat up it went back down. I had a nap this afternoon and I had some reflux where again something was coming up but went back down. If it was actually vomit and not reflux (particularly in the night), would it have just gone back down? I didn’t feel like I needed to throw up. If it was vomit, you would think I would have felt like I would be sick. Reflux doesn’t usually feel like that.

The last few days the nausea seems to come and go as it pleases though, whether I’ve eaten or not. Plus it basically started with that bout of diarrhea I had early Monday morning. I am assuming it is related to either the antibiotic usage or a stomach virus. My gut still seems a bit bubbly.

However, I am curious if it could be related to the gastritis. The Dexilant has worked very well for about 10 months. Is it possible that it may no longer work anymore? If so, is there anything my doctor will be able to do for me if it’s no longer working? It was awful having all of that nausea for weeks last year. I really don’t want to go back to that again.
Should I mention this to my doctor or wait to see if it settles down in the next few days? I figure if it’s viral it may last a week or so. I’m trying to be patient but I have to admit this is all getting on my nerves!!

Hi Dr. Branch,
Here I am again! I think it’s been about a week since I last wrote to you.

My nausea finally began easing about 2 days ago. It lasted a little over a week. I have a feeling it was viral.

My UTI symptoms had not fully gone away but had improved last week. I gave a urine sample on Monday last week (10 days ago - about 4 days after finishing the Amoxicillin) and a sample on Thursday of last week (7 days ago - about 7 days after finishing the Amoxicillin). The sample from Monday showed no infection but the sample from Thursday showed infection with Enterobacter cloacae complex (report attached). I am quite certain my previous infection was Enterococcus and the first infection was Klebsiella. I think I keep getting different infections.

So, my doctor put me on Macrobid for 5 days from Saturday to yesterday. We were both surprised there was infection again since my symptoms had improved. The odd thing is that while I have been treated with the Macrobid, the symptoms started bothering me again over the last few days - the burning and feeling of needing to urinate. My doctor and I are perplexed!!

I gave another sample yesterday. Not sure what it will show since I was still on the Macrobid. I’m sure I’ll have to go again to give another sample next week.

My doctor is sending me for an ultrasound on Wednesday to have a look at my bladder, kidneys, etc. He wants to make sure I don’t have a kidney stone causing trouble (since I’ve had stones before), that my bladder is emptying properly, etc. He said we’ll wait to see those results. He said he might then have to send me to a urologist. He said some people have chronic infections that need to be treated on an ongoing basis. He also mentioned interstitial cystitis.

He said at this point we need to further investigate this to see what could be at the root of it. I just hope we get answers soon.

When I spoke with the receptionist at the ultrasound clinic, she asked if I have a history of ovarian cysts, which I do, so she booked me for extra time so they can also check for cysts.

For many years I took Marvelon and then switched to Micronor for a couple of years (between 2015-2017). While on the Micronor I had to have a couple of ultrasounds done for other reasons and they showed that I had ovarian cysts (which I had monitored and they would disappear after a couple of menstrual cycles). If I ever had cysts while on Marvelon they were never found at any of my other ultrasounds.

In 2017 I went back on Marvelon and in January 2019 I went back on Micronor (my doctor and I switched because we were not sure if the estrogen from the Marvelon was contributing to a flare up of PVCs I was having). I don’t really like being on Micronor and it seemed to make little difference with my PVCs so I switched back to Marvelon less than a month ago. I am now wondering if while on the Micronor from January to XXXXXXX I may have developed cysts.

Is it possible that a cyst could be putting pressure on my bladder and causing me to get UTIs?

About 3 years ago, I was seeing a walk in doctor about some abdominal issues (turned out to be constipation). While he was examining me he asked if I’ve had uterine fibroids. I said no and he said it felt like there was a lemon on either my ovary or my uterus (I can’t remember which one he said). I was asymptotic at the time. He sent me for an ultrasound. It showed nothing. My doctor examined me when the results came in and he said he felt nothing like a lemon. I’m not sure what that was all about but my doctor said if it had been a cyst, it must have gone away.

I wonder if I have a large cyst now?? Im not sure what to think.

This is causing me some stress and concern but I’m also wanting to get tested and get answers so hopefully I can get help.

I guess the saga continues... I will keep you posted.

Here is another update.
I finished the Macrobid on Wednesday. I received a call from my doctor’s office yesterday that the sample I provided on Wednesday showed no infection. I went back to give another sample today since I am still having UTI symptoms (burning, feeling the need to urinate) and I figure the Macrobid should be out of my system now.

Today I had to see a walk-in doctor. Apparently the dipstick of my sample today showed some white blood cells and some blood (I have my period so I’m not surprised by that). The doctor suspected I have an infection again. When I told the doctor everything that’s been going on over the last month and that I am still having symptoms, she seemed concerned and asked if I wanted her to treat me. I said I didn’t know if that was the best idea before we see the test results because I’ve been on so many antibiotics and I don’t want to take any that are unnecessary or incorrect. I said my urine results usually come back in 2-3 days so maybe it would be best to wait until then, when my doctor sees them. She said that was fine but still seemed rather concerned about it. Now I’m questioning whether I should have let her give me an antibiotic to take while I wait. It kind of bothers me having to wait for test results knowing I likely have another infection brewing but I also don’t like taking antibiotics if I don’t need to. There were 9 days between stopping the Amoxicillin and starting the Macrobid and I was fine. My symptoms had actually started to improve and I was surprised there was more infection. Anyway, hopefully I have not made a mistake in not taking treatment.

Do you think I should have taken an antibiotic from her in the meantime while I wait for the results or is it better to just wait for the results? It concerns me that she seemed concerned. My doctor is in tomorrow morning and I could go see him then if you think that would be a good idea.

Hi Dr. Branch,
I am still waiting for my urine test results from Sunday. I have my ultrasound tomorrow.

I just have a couple of questions. I have known that I have a kidney stone for at least the last 5 years. I was told it’s in an area of my kidney that’s not causing any trouble. If a kidney stone is causing my troubles, could this mean that this stone might be moving? If so, wouldn’t that normally be painful? I’m sure it’s possible I could have developed another stone though as well. Either way, if it’s causing all of these issues, does that mean it’s getting ready to pass? I am a bit panicked about that. Passing a stone is a very unpleasant event. We will be travelling to the US in less than 3 weeks and I terrified to pass it while on vacation. I have had to pass the last two stones I’ve had. Is there anything that can be done to remove it instead of me having to pass it?

Also, this has been going on for over a month. I’m worried that this could be dangerous for my kidney or could lead to bigger problems. I really hope my ultrasound results come back by Friday but it may take until Monday. Our healthcare system here is great for many reasons but it can be SO slow!!

I’m probably overthinking this and I know that I don’t actually know what’s going on for sure. I just like be informed.

Hello again. Kidney stones can get infected, which then becomes a place that bacteria live and can continue to cause recurrent urinary tract infections. So with your history of having a kidney stone in the past, I would definitely want to see if there was still a stone there, because that may be the cause of your infections. Even if the stone is staying in the same place and not causing any pain or other symptoms, it could be a nidus of infection.

A urologist will be the best person to talk to about this, but if there is a stone, and it is suspected to be infected, they may want to do a procedure to break up the stone, depending on how big it is, and hopefully allow it to pass. That would get rid of the source of infection and perhaps would solve these problems you've been having.

So it will be very interesting to see the results of the scan. Please let me know what it shows when you get the results, and what your doctor recommends doing next.

It has been a while since I have written but I had been waiting for a few test results to come back first.

I received my ultrasound results. Everything was normal. We have known that I have a small kidney stone in my left kidney for at least 5 years now which has not caused any problems. They saw it on the ultrasound and said it had shrunk a little since October 2018. They said it is not obstructing anything or causing any problems. The results showed that my bladder is emptying properly. I have no fibroids or cysts. I was glad to hear the ultrasound was normal. I was a little worried I could have had a kidney stone causing me trouble.

I finished my last antibiotic on July 3. I have had a few urine samples taken since then and they have all come back as normal. I gave my most recent sample 3 days ago and I saw a doctor at our walk-in clinic this evening who said the results were normal.

My UTI symptoms are not gone but they have improved. I am not feeling the burning constantly or a feeling of having to urinate constantly. It’s been nice to have some relief. Like I say, they’re not gone but they are not as bad as they were.

I have an appointment to see a urologist on September 10. My doctor said that maybe the urologist will want to look for inflammation.

We are leaving for a vacation in the U.S. tomorrow. I am really hoping I will be fine while I am there. We will be there for 9 days.

This evening I told the doctor that since I still have some lingering UTI symptoms, I am scared that if I get an infection while I am away that I will not know I have one and may not go to a doctor. She said that hopefully I will not get an infection while I’m away but she said that if I do I will likely start feeling more symptoms and that the burning will probably get worse again. She said to see a doctor if my symptoms get bad again.

While you're in the U.S., the easiest place to get treated if you have symptoms worsen would probably be an urgent care. They will do a urinalysis, and if that shows signs of infection, they would normally start antibiotics before the urine culture results are available. I would make sure to tell them about your history, as you would likely need to be started on a stronger antibiotic.

They can call you with the urine culture results, but you are right that if you are in a different state at the time, it might be difficult getting a different prescription if you need it. I think most clinics would be able to fax or email the results to you, and so, if you needed to, you could go to another urgent care clinic and show them those results for a different antibiotic.

I have been in the U.S. since Saturday. Right before I left I saw a doctor Friday night who said my previous urine sample results were negative. On Saturday morning I took a urine sample to the lab just in case. I called my doctor’s office yesterday. They said they received the results. So far they have not called to let me know if I need an antibiotic so I assume the results were also negative.

We have taken a long road trip so I had to do a lot of sitting and could not use the washroom as frequently as I probably should have. I have been feeling the burning and need to urinate frequently again for the last couple of days. I decided to go to an urgent care clinic here just in case. I told the doctor my situation and showed him my reports. He said my urine sample showed no sign of infection - no white blood cells, no bacteria just some red blood cells. I’m not surprised about the blood because my period is coming soon and I have had a little spotting. At home they cannot tell if we have bacteria in our urine so I was surprised to hear him say they didn’t see any bacteria. Or did he just mean any signs of it by looking at white blood cells, etc? That’s all that they can see back home.

Anyway, he said he didn’t suspect that I actually have an infection (so far I have been infection free for a month now). However, since I am having symptoms and I am away from home and cannot see my doctor until Monday he decided to give me Macrobid just in case to cover me until I get home.

My doctor has mentioned to me several times (and I think you mentioned this too) that he feels I may have interstitial cystitis. I didn’t say that to the doctor yesterday but he brought it up. He said that he believes that is likely what I may have. I don’t think he would have treated me for infection yesterday except that I am out of the country. He said he is not even going to send the sample to the lab because he doubts that I actually have an infection. When he heard that I had infection with 3 different species of bacteria in a month he was very surprised. He said he has never seen that before and he has been a doctor for a long time. He said when someone has multiple UTIs they are usually caused by the same bacteria. He said he wonders if I live with some of these bacteria all of the time and they may not even cause me a problem. Or he said that maybe my body is just strange because he has never seen this before.

I am a little nervous to take the antibiotics since I may not have an infection and I have been on so many antibiotics in the last couple of months. However, I am also nervous not to take them just in case I actually do have an infection and I am always terrified it will spread to my kidneys. I am not sure what is the best thing to do. I was wondering if I could get your opinion? Should I take the Macrobid while I am away and take a sample to the lab next week when I return home? I have constant burning “down there” and a constant feeling that I need to urinate. It’s not so much that it burns when I urinate. It just feel like it is burning all of the time. I constantly feel like I have to urinate. I will go and one minute later I feel like I have to go again. I went to the doctor last Tuesday. They checked my urine with a dipstick which showed possible infection. I was prescribed 1 dose of Fosfomycin. It didn’t work. I still had symptoms. The doctor (not my family doctor) seemed confused so she treated me for a yeast infection. That didn’t work either. I finally had a chance to see my family doctor yesterday. He prescribed Macrobid for me but had not sent the results of my urine sample from the lab. The results of my original urine sample (from Tuesday) came back yesterday afternoon. It showed UTI. I don’t know what bacteria I have. My doctor called to say the Macrobid was not as likely to work due to the sensitivity so he gave me Cefixime 400mg, 1 pill a day for 5 days and said the Cefixime was more likely to work. I took one pill yesterday afternoon and today I’ve had no change in my symptoms. I read online that some people find relief after being on it 24 hours but I didn’t feel any change. I took my second pill 3 hours ago. I still feel the same. I have anxiety around my health so I may sound a bit paranoid here. First I am worried about having had this UTI for a week now and am worried it might be really bad. I am worried this antibiotic is not working. I am also VERY nervous that this may spread to my kidneys. I am not sure how many more days to give this before I see the doctor again. Am I expecting too much too soon from this antibiotic? I’ve bern told it’s strong but I don’t feel like it’s doing anything. Maybe I am a bit worried that I only have 3 pills left and nothing has changed. My biggest fear is that this UTI has not been treated properly for a week and will spread to my kidney. Does it take very long for the bacteria to spread to your kidney? How long should I wait to see the doctor again? I am a healthy 42 year non-smoking female. I’ve had UTIs in the past that have always responded to treatment. I have no fever.
