hi,
thanks for writing to HCM
regading your doubts
1) there no worry if the PPBS is maintained between 130-170. but ideal is every 3 months you get HbA1c done and see the reading is 2) the medicine you are giving is OK. but B1+B6+B12 combination is better
3)the following precautions he should take
wear shoes completely covering whole feet to avoid even trivial injuries
check blood sugar regularly
diet limiting mainly carbohydrate containing foods
take medicines regularly, even minor injuries need antibiotics consulting your doctor
consult endocrinologist at least once in 6 months
hope this answer serves your purpose

Hello.
1.-In a diabetic patient, sugar levels in blood post food must be lower than levels your father has.Besides measuring blood glucose is important measuring glycosylated hemoglobin (HbA1c).If blood sugar levels are high there is damage to heart, kidney, lung and other organs.
2.-Methylcobalamin with vitamine, effectivenes in nerve pain is moderate.
Gabapentine, for example,is widely used in the treatment of nerve pain.
3.-About Precautions required for him,say you that he is a person with various cardiovascular risk factors and that is dangerous. He should avoid eating sugary foods and drinks, avoid high-fat foods, avoid salt in foods and drink plenty of water. Also,he must walk 30 minutes every day.
I wish you good health.

General & Family Physician Dr. Pavan Kumar Gupta's  Response
Hello,
It is allright to keep PP blood sugar levels in an elderly patient to around 150-170.
However diabetics in long run can develop heart and kidney complications.
These can be diagnosed by cardiac assessment and getting the renal function tests and tests for microalbuminaria done.
Pain in lower foot is due to diabetic neuropathy and this can be taken care by taking methylcobalamine and also medicines like pregablin and DULOXETINE..
You must make sure that blood sugar, blood pressure and cholesterol levels are kept within normal range.
Ask your father to keep a control on diet and do some regular aerobic exercises like walking for 30 minutes a day.
Keep meeting your doctor regularly.
Thanks

DEAR SIR/MADAM

ACCORDING TO ADA THE POST PRANDIAL SUGARS SHOULD BE BETWEEN 140 - 180 MG/DL SO NO NEED TO WORRY

SECONDLY HE IS HAVING NERVE PAIN, METHYLCOBALAMINE IS ALONE NOT ENOUGH, PREGABALIN OR GABAPENTIN WILL HELP

OTHER PRECAUTIONS ARE TO START ACE INHIBITOR AND STATINS

THANKS

Diabetologist Dr. Pratap VGM's  Response
DEAR SIR/MADAM

WELCOME TO THE HEALTH CARE MAGIC.

IF THE DURATION OF THE DIABETES IS LONG, THEN HE WILL BE PRONE FOR THE LONG TERM COMPLICATIONS OF DIABETES, REASONABLE TIGHT CONTROL OF BLOOD SUGARS ARE VERY MUCH ESSENTIAL TO CONTROL THE COMPLICATIONS.
SECONDLY DID YOU DO FUNDUS EXAMINATION, BECAUSE IT WILL REVEAL LOT OF THINGS.
CHECK FOR URINE MICRO ALBUMIN CREATININE RATIO AND MICRO ALBUMIN ESTIMATION IN URINE. IF PATIENT IS HAVING MICROALBUMINURIA THEN ITS IDEAL TO START LOW DOSE OF ACE INHIBITOR OR ALPHA RECEPTOR BLOCKERS. IT WILL RELIEVE THE PAIN TOO TO SOME EXTENT
SECONDLY START PREGABALIN OR GABAPENTIN CONSULTING YOUR DOCTOR, IT WILL HELP TO RELIEVE LEG PAIN.
THIRDLY PRECAUTIONS THAT NEEDED TO BE TAKEN TO PREVENT FURTHER PROGRESSION OF COMPLICATIONS. PERIODICAL BLOOD TESTS NEEDED TO BE DONE.

THANKS
