Is A Surgery Needed For Cysts In Ovary And Pelvic Pain?

Hi I just completed an ultrasound today that results in both my ovaries covered in cysts and 80mls of loculated fluid in the pouch of Douglas. I have lower pelvic pain and sharp pains on the right side of my abdomen where a year ago I had my gall bladder removed.

Is this fluid cancerous? And will it prevent me from conceiving children?

I had endometriosis removed twice about 12 years ago and have been suffering with pelvic pain alot lately.
Can this be treated with antibiotics? Or do I need surgery ASAP?

Hello and welcome to HCM,     
I am Dr Nilajkumar a consultant gynecologist and I will be helping you in your queries.
There are three possibilities in your case.

1. recurrence of endometriosis. This is most likely considering the history and ovarian affections and will need surgery.
2. pelvic infection: since the collection is loculated may be an infections. Please see if you had fever and foul discharge at any time in the near past.
3. ovarian cancer: very rare. It may have produced a bulging abdomen and mass fluid collection by now.

The best way out now is to get a course of antibiotics and do a Ultrasound guided tapping of the loculated fluid if possible.

If it is pus, then continue antibiotics according to culture report. If it is blood then go for surgery as endometriosis is more likely. In the second case I advise a hysterectomy if family is completed as it will help preventing recurrence and relieve you of further trouble.
