What Does This Blood Test Report Indicate?

Question: Hi Dr,
Can you review my blood test?
weight 89 kgs, height : 5Ft 8 inch, obese with high BP. Taking medicine for Hypothyroidism (Thyronorm 100).
I also have sensorineural hearing loss.
Please suggest on my reports.

1. Do you have any complaints regarding health?: Ans: Nothing serious till now.
2. Why were these reports done? Ans: Master Health check up / periodic check up.
No specific reason.
3. Have you also done a ultrasound abdomen along with these? Ans: Not now. But previous ultrasound reports (three years back) showed Fatty Liver and even before once I got Enlarged Spleen detected in USG. Do you suggest to get an USG done?
4. Do you consume alcohol? If yes then how frequently and since when. Ans: I consume moderately twice a month. I don't consume regularly, started since last 6/7 years.

Hello.
Thanks for following up and answering my questions.
From all the reports, it seems you have vitamin B12 deficiency. It is causing raised homocystein levels, the typical anemic picture, even raised AST and ALT to some extent.
What is not explainable at this moment is slightly raised amylase and lipase.
Also your vitamin D seems low.
You said in your answers that no serious complaints as such. So you do have some complaints. It will be helpful if you mention them too.
I would advice you following.
1. Start on multivitamin tablets that contains vitamin B12 and folic acid as main content. Available as mecobalamin capsules. That you need to take for another 1 month once a day after food. Taking injections once a week will also replace the deficiency faster but whatever you are ok with can be taken. Injections or orally is your choice.
2. Expose your self to sunlight especially from 11am to 3pm for alteast 10mins which contains the uv light to produce vit D by our skin. Your age is less hence not advising you vit D tablets. Doing this above should do the work.
Ultrasound is not required. I just asked because I though you mist have got done. No need to do as such.
Repeat your abnormal tests again after 1 month except for amylase and lipase. Repeat these 2 tests after a week to see for any changes in the values. They are not very high to say any pancreatic problem. Usually in pancreatitis the values goes in 1000+. Yours are just slightly above normal so nothing to be worried of it now. Just repeat these 2 after a week or so. And others after a month.
Get back with me with the reports or consult your doctor.

Hello.
Almost all multivitamin capsules contain b12. But we usually prescribe 'Becosules' capsule as it is cheap and easily available. It will contains almost all essential vitamins and folic acid too.
That should suffice.
