What Does My Lab Test Report Indicate?

Question: Hello Doctor,
I have a serious Query for which I need your expert advice.
Problem : Have been experiencing severe pain in the whole abdomen area, and have loose stools since 8 - 9 months but not that bad , like after every meal I feel heavy and full and get the sensation o use the washroom, have got colonoscopy, endoscopy , USG done for the same.
Recently the pain has aggravated in the abdomen and I feel uneasiness in the abdomen . I thought it was gastric pain hence took Panta D and other gastric related medicines but did not get any relief .
Post that I did the USG in which the liver and sleep showed enlarged, post that got the Liver function test done in which the Values were high , lease find attached all the reports for your reference .
Post that I did the Fibroscan test to find out teh reasons for ALT and AST high values , but that report seems to be normal
I have recently done all the above tests and the Blood tests like Vitaman B12 and Vitamin D
My results of Vitaman B12 shows as + 2000 which is very high , I am really worried as to how the B12 results are so highly raised and I read over the internet it is due to liver disease, cirrhosis and other blood cancers and all. I am really worried can you please look into all the reports i have attached and Kindly share your expert opion please

Its very rare please get a second vitamin b12 level checked up.

Detailed Answer:
Hi,

Usually, excess vitamin B12 is removed in the urine. Conditions that can increase B12 level include liver disease (such as cirrhosis or hepatitis which is not your case as ultrasound is normal(mild enlargment or fatty changes are normal at your age.
Now what i suspect (as you already have read) is that it can be a sign of blood cancers known as myeloproliferative disorders, which includes myelocytic leukemia.

However first and the foremost suggestion is get a repeat test done.Your tests have been done at some hospital who usually have a temporary lab .Get your test done either at a good lab.
Yes there are some times pathological errors or sample problems.
As already said earlier high vitamin b12 is a very rare finding and before investigating the cause of it we should confirm the lab reults twice and that too at a good lab.

So before comming to descriptions of myeloproliferative disorder and panicking you by describing some rare possibilities i would suggest you to go for a recheck up of vitamin B12 level.
Get it done today and revert back with reports tomorrow.

Thanks for your Advice doctor, this really helps.
I will get the B 12 test done within a week's time as I shall get back on the same .

Last question, from the reports of colonoscopy, endoscopy do you suspect anything to be worried about and even with the LFT test, the hgh results of bilirubin means jaundice ? Request your opinion here.
Also, the fibroscan test is normal ?

Brief Answer:
Except B-12 nothing has clinical significance.

Detailed Answer:
Hello again my dear patient,
Have seen all your reports.
Mild to moderate derangement of liver function tests have no significance.Minimum three times elevation of liver enzymes should be there for having any clinical significance.
Secondly mildly elevated bilirubin levels have no significance again.Minimum 3mg/dl or more have significance.
Mild hepatomegaly can be due to infection in body as ESR is also elevated it could be a mild infection some where in gut.
Vitamin D deficiency is there in 90% of asians.

Rest you have microcytic hypochromic anemia.Most lokely iron deficiency anemia.Start taking iron polypeptides 6 mg inve daily for the same.


Fibroscan is ok!


At present the main concern is B12 levels.
I would refrain from making any further comment till we have a second B12 report.
