What Does My Blood Test Report Indicate?

default
Posted on Tue, 24 Oct 2017
Question: Hi,

I am a 34 year old male. During my annual physical earlier this year, my doctor said lypmhocytes are marginally higher (50%) with total WBC being 5 K/mm3. Neutrophils was 39%. I had a repeat test 6 months later and now the result was WBC 5.7K/mm3 (lymphocytes 48%, Neutrophils 40%). So the doctor order for a blood smear and flow cytometry. I believe along with blood cytometry another CBC was done 3 days after the previous CBC. The result now was WBC 4.7K/mm3 (Lymphocytes 44%, Neutrophils 47%). Also my doctor said the blood smear test was normal.

I am still waiting for my flow cytometry results. But I am a person with health anxiety disorder and take zoloft for that for past 5 years. So this episode has sent me in partial panic mode. Here are my few questions

1) If it was some form of leukemia or lymphoma, would the blood smear have ideally flagged something?
2) I looked at my results for last 2 years and my lymphocytes have been around 2.6-2.8 K/mm3 (43-47%). Is that normal?
3)Also my MPV for the last 3 tests spanning 6 months has been 11.5, 11.7 and 11.6. Is it something to worry considering other values? Platelet count has been between 162-171 during this period.
4) I have B12 deficiency for sometime now. B12 values are ranging between 231-290. Have been bad at taking the supplements.

Long story short, I am worried of having Chronic Lymphocytic and Myeloid Leukimia, lymphoma and all forms of bad diseases. Please help.

Detailed Answer:
Hi and welcome to HealthcareMagic,

1. Yes, blood smear would show lymphoblasts or other atypical cells. You had normal WBC in both tests and slightly elevated lymph count which is not indicating lymphoma/leukemia

2. This is normal. Lymphocytes and WBC count may vary in a short period, even from day to day. ALso, some other conditions such as stress, infection or diet may affect these values.

3.These are normal values

4. The B12 deficiency may cause anemia, low red blood count. If there is normal RBC then there is no need to take supplements even if there is low b12 level.

So, all in all, there is not much of concerning result and your worries are probably the result of anxiety. Just try to focus your mind on something else, not on serious diseases.

Wish you good health.
Feel free to ask further.

Thanks Dr XXXXXXX I received my Flow Cytometry report and the doctor said it looks good. That makes me feel a lot better. Attaching my report here for reference. If you could take a glance that will be great.

F     Flow Cytometry Interpretation     See Below For Report
- Peripheral Blood
- Diagnosis:
- No diagnostic immunophenotypic abnormalities detected (see comments).
- Comments:
- No immunophenotypic evidence of a lymphoproliferative disorder, acute leukemia or circulating
- blasts is identified. Myeloproliferative neoplasms and myelodysplastic syndromes may not show
- antigenic abnormalities on myeloid cells and cannot be ruled out by flow cytometry. Please
- correlate the result with morphological findings, other pertinent laboratory data and clinical information.
- Flow Differential (%) and Population Analysis:
- Lymphocytes: 39.6%
- T-cells (71% of lymphoid cells) show a CD4/CD8 ratio of about 1.2 without overt
- phenotypic abnormality. NK-cells (11% of lymphoid cells) are unremarkable. Mature B-cells (15% of
- lymphoid cells) are polyclonal (kappa:lambda 1.6).
- Monocytes: 5.1%
- Monocytes co-express CD14 and CD64 without phenotypic abnormalities.
- Granulocytes: 51.7%
- Granulocytes are phenotypically mature and without aberrant antigen expression.
- Specimen Viability: 99.7%
- Cell Yield: 4.2 Million
