What Do These CBC Rest And Abdominal X-ray Indicate?

Question: Hi

I am so very worried I have cancer, in particular leukemia. I am a mess right now. Can you please help and tell me what the chances are that I have leukemia.

I went to the doctor last week because of a feeling of fullness under the lower part of my left ribs.

He ordered an ultrasound, Complete Blood Count, and chest and abdominal xrays.

The report came back. It says I have borderline splenomegaly (measuring 12.9 cm in length).

The blood work came back I also have low platelets - result- 122- range 145-400 x 10E 9/L



The blood work also shows I have low WBC- result - 3.7 - range 4.0 - 11.0 x 10E/9L

My RBC was 4.69 - range 4.20 - 5.70 x 10E12/L

My MPV was 11.1 - range 7.4 - 11.3 fl

I have done blood work in the past that have also said I had low platelets (132 - on XXXXXXX 14, 2014). My platelets have been low since 2012 - they were normal in 2011.

On my blood test in 2014, There is a test I haven't seen on my other blood tests - it says RBC's appear - and the answer is normocytic, normochromic

and another test states. PLT's appear - adequate.

My red cell count in 2014 was 4.98.



I am a vegetarian which I hear my lower WBC and platelets.



What do you think of my results? Please I am so worried.

This blood report does not support any type of cancer!

Hi! thanks for your health query!

In short, you are suffering from deficiency of some vitamins/minerals or kind of low grade bone marrow depression but NOT BLOOD CANCER! There is also hardly any chance of other type of cancer, from this blood report...

Had you nucleated Red cells in your peripheral blood or differential count, there were chances of cancers....!

Your platelet count /WBC is on the lower side but fluctuating and not falling; therefore, it could be related to folate/Vitamin B12 deficiencies called a syndrome 'megaloblastosis' or some minor variant of hemoglobin abnormalities or some low grade inflammatory conditions as there is slight enlargement of the spleen also...never malignancy!

I've done more than 1000 bone marrow studies of such patients and all of them are completely normal after some supplements.

Please share the differential WBC count and if you have any kind of chronic health problem, for a better opinion.

Absolutely fine!

Hi! there is nothing to worry at all; in fact, some people may have this kind of parameters/values as their normal pattern!

Anyways, minor ailments should also be considered and treated accordingly.

One disease is there called hypersplenism where these symptoms may occur; please tell me your Hb%, MCV, RDW, MCH, MCHC if available and never forget to take commercially available folate/vitamin B12 supplements and give me feedback after 3 weeks!

Thank you so much. How do you diagnosis hypersplenism and is it serious?

APRIL 10 2015:

Hb% - NA, MCV - 94 , RDW - 12.9 , MCH - 32., MCHC - 338.

January 14 2014

Hb% - NA, MCV - 93.6 , RDW - 12.4 , MCH - 31.2 ., MCHC - 333

What do you think of these results?

I will take some folate, and b12. I just bought some b12, but should I buy folate as well?
Also should I be worried about Lymphoma?

You are not having hypersplenism!

Hi! Hb% is a vital marker however, as per other indices, you are fine and they do not support a possibility of hypersplenism!

Yes, you have to take folic acid 5mg /10 mg tablets one daily ..

Regarding lymphoma, you have to have some enlarged lymph node anywhere in your body like neck, groins, thorax, abdomen etc. and CT scan/ultrasonography are useful to detect them; but the features would be worse and rapidly progressing in that case!

Would you recommend a bone marrow biopsy if you were me?
I have started vitamin b-12 and folic acid. do you know how long until I can see an improvement?
And will the folic acid and b-12 make me feel less tired? how long until I feel better.
And just one more time, you really don't think I should be worried about leukemia?
I asked a another doctor on here. I'm really really worried. Here is what he said

Your spleen is moderately enlarged.
There are many different causes of splenomegaly.
Infections, portal hypertension, blood related disorders, disordered immune regulatory disorders are some of the common causes.

A liver function test, a peripheral blood smear study, a portal venous doppler scan and a bone marrow biopsy have to be done to find the cause.

Causes could be so many but we have to know the one for you!

Hi! there are hendreds of causes of an enlarged spleen in medical books but practically they are not that common; in fact, some people ma have enlarged spleen as there normal structure..

However, the cause must be detected.

I would like to suggest you to take these medicines for 3 weeks and then do a repeat test of blood hemogram and ultrasound of the spleen. Also note that if your spleen is tender!

If nothing is good or being worse then I would recommend a bone marrow examination.

Hi! if an enlarged spleen is tender, it would mean that there is infection, abscess, venous occlusion or infarction and antibiotics etc. could be needed.

It is just a sign that might help to narrow down the possibilities...
