Question: Hi Doctor,

I have fatty liver, high cholestrol (240) and gastritis problem. I am presently under lipagyln and stator F medication. For the past one month I have been drinking every day, and not continue stator F. For the past 3 days I have bloating issues, my stomach seems to be swollen, then is no pain or any serious issues.

My tummy seems to be swollen and I get discomfort in upper right quadrant in the stomach(liver) while sitting straight in a chair or in a car. I also have gas and constipation issue. I worried if there is something serious issues underneath. Please suggest any good medication for fatty liver and this bloating issue. Is there anything serious with my liver or intestine ?

Brief Answer:
You need to stop drinking.

Detailed Answer:
Hello ,welcome to the HCM,

Stator F can cause these symptoms ,Since you have mentioned that you have stopped taking this medicine one month before , so probably excess drinking (daily) causing these symptoms.

Drinking not only worsen fatty change in liver but it can cause inflammation and if continued can end up with liver cirrhosis.

I would suggest you to stop drinking first ,take meals with less fat (10 percent in a day) and get ultrasound abdomen done to rule out inflammation of liver .Repeat lipid profile and liver function test to estimate adverse effect of drinking. And follow accordingly.

Also you need to get checked up by a Physician in person to see whether it is hepatitis or ascitis (fluid collection in abdomen) causing right quadrant discomfort.

Hopefully my answer will be helpfull for you ,you may ask further if you have more queries to ask.

Brief Answer:
First physical examination needs to be done.

Detailed Answer:
Dear I understand your concern.First your thoroughly physical (body) examination by a Physician is must to rule out any sign of active liver disease. And also some investigations like ultrasound abdomen, lipid profile and liver function test and kidney profile to rule out any dangerous deranged blood levels.
As excess alcohol not only effect liver but it can cause adverse effects on kidney and lungs too.
Until that you stop drinking or if craving is increased you can make a goal and keep in mind some points.
1.keep a small amount or no alcohol at home.
2.limit of alcohol depends on condition of a person at point of time.
3.If all your tests and physical examination show no alarming signs then you can limit your alcohol level accordingly.
Lipagyln is used to treat dislipidemia (a drug to control cholestrol and lipid level).Liv52 is a herbal supplement for which I can not comment wether it will help to treat liver disease or alcohol effect.

Hopefully my answer will be helpfull for you.
Pls let me know if I need to clarify further.

I visited a Gastro Surgeon todya and showed my Lipid profile/LFT/Abdomen USG results taken two months back.

He told my Gamma GT level is high and a cause of concern. He told me that the liver may have significant damage due to alcohol. He prescribed me some medicines and told to abstain from Alcohol totally. Also he did a physical exam of my stomach and have not told anything. He suggested to have a endoscopy to check my liver varices when ever possible.

What is varices ?
How long it takes a fatty liver to turn cirrhossis?
Do I have cirohossis ?
Is there any treatment for cirhossis?

I am highly worried if the condition can be treated.

Report are attached.
Gastro surgeon wanted to have endoscope today to see if i have varices to confirm the damage to liver. Based on the test he told the veins are bit prominent and I have to stop drinking even a bit from now. He asked me to continue Livophill/liv52/esmoprozole/stator f/lipaglyn for 30 days and do a LFT after one month and come for review.

First he told the varices are not clear then he told could see the veins prominently .Can you please confirm based on the report attached that if he was exaggerating to make sure that I am scared enough and do not drinking at all ?

I made up my mind to quit the alcohol addiction but I would like to know that can't I have social drinking like once or twice a month any time. Will it effect my liver and I will have cirrhossis and die early. Please advice.

What does the endoscopic results actually say about my GI tract condition ?

Does the pain in the upper right abdomen is due to hiatus hernia or due to constipation/gas ?

How long should I be not touching drink till my liver comes back to normal and then have occasionally drinking. Please suggest.

Any other medications should I take ?

Brief Answer:
You need to stop drinking for at least 3 months.

Detailed Answer:
Dear I can understand that it must be difficult to fight with alcohol addiction, but right now you dont have any option.
Since your endoscopic results shows that there is fat deposits in liver, ulcer and inflamation of stomach, and inflamation in easopgagus.I would like to tell you that all these findings could be because of excess alcohol drinking.
As alcohol is broken in liver and its byproducts like acetaldehyde and free radicalls in excess are leading to fat deposits in liver, which if continued to be increased with alcohol drinking ,may lead to hepatitis (inflamation of liver) and cirhosis(fibrosis and scar). As your doctor said that some prominant veins are seen, that means it could be because of initial stage of varices(dilatation and bleeding of veins).Consequently which may lead to fluid accumulation(ascitis),internal bleeding and kidney failure.
Good news is that it is never late to reverse these symptoms by stopping drinking. So I woud suggest stop drinking for at least 3 months, follow your treating doctor timely, and take medicines on time. Also taking low fat meals and avoiding spicy and fried things would help to heal gastritis and easophagitis.
Let your liver damage to be reversed by stopping drinking then you can think about drinking what is permissible.
