Question: Hi! Im 34 years old. I have a 5 years old daughter. I used to have period every 28 days normally. But since 2012 November I'm getting scanty period . This happens after the removal of copper t. My gynae dr told me to do usg pelvis, fsh ,lh,tsh ,t4 and bhcg test. All the lab test came out normal except the usg report says polycystic ovary and no free fluid cul de sac. I'm very worried because I would like to plan for another baby. Please help me. Thanks.

I understand your concern..

PCOS can cause irregular periods and anovulation. Are your periods regular when you are not on duphaston or meprate?

Since your reports are normal , you need not worry about scanty periods now. If your periods are regular, try for spontaneous conception. After 6 months of trial if you have difficulty in conceiving, you might require ovulation induction drug for ovulation to occur. Avoid delay in pregnancy since you are already 34 yrs.

If your periods are irregular, first regularize your period prior to pregnancy.

Yes my period are regular after I stopped duphaston and meprate. I got it every 28 days. First day is very light ,, it does not even soak 1 pad and 2nd day is like spotting, no pad is required . From 3rd day nothing. I do not have unbearable pain but I felt some kind of heaviness at my pelvis area, headache and slight backpain during my period. I can't sleep at night because of this.

You can take a cycle of OCP's from the first day of your periods for 21 days o see how the periods comes. It will be mostly normal flow. Take a course of antibiotics to treat for pelvic inflammatory disease which might be the cause of pelvic pain.

You can take mefenemic acid for pain relief during periods

After this cycle try for spontaneous conception for 6 months and then go for ovulation induction drug.

Dear doctor,
I would like to know that which report do the doctors normally follow to confirm polycystic ovary. My usg pelvis report says sonographic appearance of polycystic ovary, no free fluid cul de sac seen, kindly correlate with the lab report. But my fsh lh tsh t4 bhcg reports are normal. I'm not overweight also . I will be meeting my doctor on this coming Friday. I'm confused and worried.
Regarding the medicine ocp which have mentioned , please let me know how to mention it to the pharmacy. I'm residing in XXXXXXX Please help me out with this final question. Thank you for listening patiently to me.

Yes, what you said was right. PCOS was only finding in the scan , otherwise you have no symptoms. 70% people with just a finding of PCOS in scan needs no treatment. But a follicular study need to be done to see for ovulation. If that is there, no treatment is required..

I advised taking OCP because that might help to build up your endometrium to get a normal bleeding.. Please use it only with the prescription of your doctor..
