Question: hi for the past 8 months i am having painful muscle spasm all over my body and my doctor cannot provide an answer

Hello,
I have gone through your question and understand your concerns.
Such painful spasm of muscles all over body may be due to metabolic causes.
You need to get tested for blood sugar, Thyroid profile, renal function test, liver function test, vitamin D, vitamin B 12,Calcium and potassium .
Is there any headache, neck pain?
Any weakness or thinning of limbs?
Any medications being taken?
For symptomatic relief I would suggest to start pregabalin with methylcobalamin.

Hi Dr. XXXXXXX thank you for your response..my doctor did a b12, calcium and potassium test which came back normal..i have been having some abnormal sensations over the last 2 years however these cramps only started after i visited an accpuncturist 8 months ago. the pattern of onset is always the same.. a rushing sensation starting in my neck then to the biceps fingers chest muscles, stomach and legs..my muscles gets so tight i am unable to move or use my hands which feels very heavy. my left arm is swollen at the elbow and my fingers feels like i am wearing a tight glove filled with sand. my doctor has tried 5 days intravenous solumedrol which did not provide any relief, I am currently trying Gabapentin 300mg 4 times a day. I am desperate for an answer as this is very debilitating. I do not have neck pain but a vibrating sensation when i bend my neck forward , weakness and tingling in the legs.. i am afraid the spasm, spasticity will get to my heart and cause an heart attack..

Hello,
You may be suffering from cervical myelopathy.
Is there any history of vision loss, headache, vomiting, seizures, loss of consciousness, double vision?
Any history of trauma to neck, urinary complains?
You need to get MRI of cervical spine region for possibility of myelopathy with or without syrinx.
Continue the medications and get back to me with MRI reports.

I had an MRI done on my brain and neck. my brain is clear. the MRI without contrast on my neck showed a syrinx, so you are right. however when it was repeated with contrast the 2nd radialogist said it is a legion on my neck and other scaring ,now my neurologist is talking about MS.I have no lesions on my brain and no vision problems. In fact I saw my ophthalmologist 2 months ago and there were no issues with my vision. I will try to get copies of the MRI reports. I was in a motor vehicle accident 30 years ago.. I cannot recall having any injuries at the time. I am not experiencing any vomiting, or loss of consciousness,, these muscle spasms, extreme tightness comes on ever 5 minutes and they are symmetrical on both sides involving the same set of muscles. no urinary issues at this time. Thank you.

Hello,
You need to upload Mri images for review.
There are three common possibilities.
1) Multiple sclerosis: Single episode without any brain lesions or another episode, less likely to be multiple sclerosis.
2) Neuromyelitis optica: Demyelinating lesions of cord with visual involvement occurs without any characteristic brain lesion.
In both above cases visual involvement may occur before, during or after the myelitis episodes. The onset of symptoms are acute. For assessment of vision involvement, VEP testing is advisable.
Third possibility may be cervical myelitis due to various secondary cause like infection, post infection, trauma. You need to get detailed work up like csf examination, blood tests, VEP for diagnosis.
For treatment you can add baclofen for releif in flexor spasm. Dose to be titrated and increased accordingly.
For syrinx surgical options are also present depending upon progression.
