What Causes Frequent Urge To Urinate Along With Burning In Urethra?

Question: Hello Doctor, I am a 49 years old male... I have had frequent urge to urinate for over past two months. Burning and discomfort in urethral (tip of the penis) after the urination but not during urination. Moreover, at beginning I had some pelvic pain, pain between the genital anus, groin pain, low back pain. But I only have some discomfort in the pelvic area and still have the pain and discomfort in tip of the urethra in addition to frequent urination. During this two months, many other problems started to appear one after the other but not at the same, time. I started having joint/muscle pain a rash on the left side of my neck, then extend to the right side of the neck and eyes pain, both eyes.

I done urinalysis several times at ER, Urgent Care and urologist office and always come back negative. The ER also grew the urine culture for two days, and nothing grew. I had Cystoscopy and was told the bladder looks fine. I have done STD test, and all comeback negative. All the time I was on antibiotics, I took Cipro 500 mg every 12 hours for first 12 days, and then the doctor switched me to Sulfamethoxazole-TMP, twice a day for 30 days. I was told I might have prostatitis, and my prostate is irritated!. The day after I had the Cystiscopy done (last week) my primary doctor took a culture of my urethra and let it grow for five days. We got the result on Friday it showed grows of Enterococcus faecalis, he suggested taking Amoxicillin 500 mg three times per day for 10 days; I will start on that tomorrow.
Here are my questions:
1- Is it possible that I got the Enterococcus during the Cystoscopy?
2 - Did I have the Enterococcus the whole time, but was not detected by the urinalyses and by urine culture that was grown for two days. In other words, The Enterococcus was the cause of my problems the whole time.
3- I have done a lot of search online, and I wonder if I have the reactive arthritis?
4- Is oral Amoxicillin the right antibiotic for Entrococcus? And is it okay to take it, I don't know if I am allergic to penicillin or not. I don't know if it matter or not, but I know I am not allergic penicillium notatum (mold).

Thanks for your query,based on the facts that you have posted it appears that you have bacterial infection of Prostate (Prostatitis) due to Enterococcus Faecalis organisms.

I would state that this is not introduced from out side while doing Cystoscopy . The urine culture done earlier did not reveal the organisms as the organisms (Enterococcus Faecalis ) may not be presented in urine .It could have been detected had you have done Semen culture earlier.

They have been detected now in urethral discharge which comes directly from the prostate.

Your Dr has started Amoxicillin thrice daily .It is possible that he may be knowing culture and sensitivity report in detail and the sensitivity pattern of the organisms to Amoxicillin hence he has prescribed the same.

Please note Prostatitis takes long time to get cured hence you will need to take antibiotic and anti inflammatory drug like Diclofenac for 4-6 weeks.

I am sorry, Is Amoxicillin is the right choice of antibiotic for the Enterococcus Faecalis ? and for 10 days only? Thanks Doctor; I forgot to ask, about all the other problems that has popped up during this two months, such as the rash on the neck, eyes pain, and joint pain. Is it reactive arthritis due to my UTI infection? looking forward to hear your opinion.

Thanks for follow up.

The varied symptoms are most likely to be due to Viral infection or It may be considered as a coincidence.

Whenever we send any material for culture report it reveals causative organisms and antibiotics to which they are sensitive to and we prescribe appropriate antibiotics as per culture report .It is most likely that Enterococcus bacteria detected in your urethral discharge may be highly sensitive to Amoxicillin .Your treating physician might be knowing sensitivity pattern and hence has prescribed Amoxicillin.

Do not worry take it for 3 weeks .It will work definitely for you .If you have time post your review about my opinion on forum of Health Care Magic.

Sure, no problem, I will add a review.

I got hold of he lab report and says:
Specimen Quality: Adequate:
"Heavy Growth of Enterococcus Faecalis, skin Flora also present".
It suggest the two following antibiotics:
AMPICILLIN <=2 MIC
VANCOMYCIN 1 MIC
But the doctor prescribed the Amoxicilin 500 mg instead.

Would the Amoxcillin work instead of the Ampicilin?

Insist your Dr to prescribe Ampicilin.

In view of the sensitivity report insist your Dr to prescribe Ampicilin .There is no rational behind to prescribe Amoxicillin when organisms are reported to be sensitive to Ampicilin.
