What Causes Difficulty In Ejaculation After Removal Of Sperm Cord Lesion?

Question: I just had removal of sperm cord lesion. This was done during a hernia surgery. I did not know about it until I got the XXXXXXX What was done? I am now unable to ejaculate . Is this perminant? what other complications can accour?

Thanks for a rare query.

There is no lesion of the spermatic cord, its surgery or surgery for hernia that can cause inability to ejaculate.

We, as surgeon excise the whole cords in some old patient (with written informed consent of course) to avoid recurrence of hernia and nothing of this sort has ever happened.

Ejaculation is altogether a different process.

Was your surgery done under spinal anesthesia?
Was there handling of the prostate or urethra?
Did you have fever, backache, headache after Surgery?

Is the erection of penis and functioning during sex/ masturbation normal ?

The probable cause I can think of is there is a possibility of retrograde ejaculation in the bladder so you do not get the usual ejaculation n the forward direction.

Do you get the Discharge Tickets from the hospital stating what surrgey/ procedure was done?

Here is the full story: When I went in for surgery as out patient I was told that it is loproscopic taking about 35 minutes and another 30 or 40 minutes in recovery. After I discovered a 6 inch incission in my belly. Wnen the pain med in hosp. wore off I had extreem pain. I used up the pills that were to last 10 days over the weekend. I was however able to get it renewed. When I went to have the cathider remover I was told blood in the urine is normal. and I did in fact bleed. Followup with the DR resulted in OK every thing looks fine. Afterward I did get headaches feaver and blood in the urine. I drank a lot of water and it cleared up by the end of the day.

Here is my concern. I received a XXXXXXX for " removal of sperm cord lesion " I paid the XXXXXXX and went to internet to check it out. The erection was not normal as expected. My sex life is history at my age. Lack of ejaculation is not my concern, I was just testing. When I called the Dr office I got a return call from DR nurse assistant. She told me that this sperm cord lesion is very common and poses no problem. The DR was just removing some fatty tissue. I tend to have
a question in my mind if this is true. My pain was so severe that I questioned why I could not be admitted. They won't admit a patient for hernia. I questioned why not the planned endoscopic proceedure and was told that This DR does not do that. Wow! tald about getting the run around. I did not get any notice of the extra work, just a surprize XXXXXXX

I am not still clear what was done?
Was the hernia operated ? If yes- was this by open approach/ laparoscopy was not attempted at all or was converted to an open approach for some reason , may be due to old age or so?

The Cather is always put in- in all patients of your age undergoing any surgery in the lower part of the abdomen. It i good that the fever and other symptoms resolved fast.

Your Concern:::
There must have been a sperm cord lesion and yes, there can be a lipoma of the cord/ Cyst of the cord. Yes , this is a small procedure and there may be a system of the Surgeon or the hospital to charge extra for extra work.

The severe pain could have been the nerve entrapment/ trauma during the surgery. It looks that only sperm cord lesion was operated. This does not need laparoscopy at all. ( and you were told that the Doctor does not do that)

How is the pain/ erection/ ejaculation now? Any other symptoms?
Is the pain gone now?

Well, this must be happening at some places.

If there is no fever/ red urine/ pain/ swelling, It is good.

You are unable to ejaculate >> There must have been due a trauma of the urethra by the catheter with swelling. This causes retrograde ejaculation in the urinary bladder.

Can this be permanent>>>This can be permanent if the internal sphincter is damaged.

I AM sorry for the harsh reply the last time. I had just completed a long reply and it went blank. This was very upseting to me. I hoped you did receive it, but apparantly not. Let's start up fresh.

Let's move away from Sex and ejaculation and review the following. I understand your responce can not be used in any form of complaint or lidigation.

Surgery was completed for hernia on the left side. It was to be laparospic but was changed to open approach. In addition to hernia repair there was a removal of sperm cord lesion. QUESTION: I was told that this is very common and is done
to remove fatty tissue. What is fatty tissue?, and how does it get there? In your opinion can this be true? Question: This was done without my knowlege or concent, and I was not told about it. QUESTION: Is this something that is done routinely? QUESTION: Should I have been at least been notified when discharged or seen with followup? You said that this was a small procedure and yet the DR charged $961.00. QUESTION: Under these conditions do you think more was done than scraping fatty tissue? Anyway, Let us discuss. I will be absolutely honest in replying your queries. Please feel free if there are certain technical words if you can not get them. In a way it may look a bit childish on my part , please do not laugh at it.

Your first concern was:: Laparoscopic surgery was planned for left sided Hernia. Changed to an open approach. And they removed fatty tissue from the spermatic cord.

Your first question is pertaining to fatty tissue and how does it get there. Yes Sir- in my opinion this can be true. The fatty tissue here are the same as they are over all the body.
In all cases of hernia, there is a pad of fat- called extra-peritoneal pad of fat and this is the lowest (proximal) limit of our dissection. In fact this is the pad of fat we always look for. Other than this- the fat is not there except some cases of fat in the spermatic cord. The spermatic cord is attached to the hernia sac and occasionally can have a collection of fat.

Yes, During the the closure of the hernia defect we do not want any such tissues which can delay the internal healing; and fat is one of the tissues, which we remove if it is coming in a way for proper closure and anticipated to delay healing. This is done routinely if thought to cause the problem as I mentioned.

Yes, Sir. You should have been notified after Surgery or at discharge what they did and they are charging for it.

Saying small means- smaller than the hernia surgery. I am sorry I can not really comment about the charges.

Yes I think more was done than mere scrapping of the fat tissues.

The mere bulge on the other (right) side is called Malgaigne's bulge (no need of surgery) and is a normal condition.. If there is actual protrusion which increases on coughing, standing or straining and reduces on manual pressure or on lying down position, this can be a hernia. And will need Surgery.

Lipoma: is a collection of fatty tissue in a circumscribed form ( which is NOT obeying body's commands yet not cancerous). This can occur anywhere in the body. The cause is not known why it is formed. Lipoma of the cord is a known entity.

Cyst of the Spermatic Cord :: While the testis descend down from inside of the abdomen to the scrotal position just before birth, it takes the peritoneal surface with it. This lining get absorbed in a normal condition. Sometimes a part of this does not get absorbed. Reactivation of the lining secretes the fluid to cause a collection of fluid. This is called a cyst. Cyst of the cord.

Lipoma is a fatty tissue
Cyst has nothing to do with the fatty tissue.

You did a great job of explaining the guestions. I am relieved to learn that the lump on the other side is normal. When I push on it it retracts back, and does not stay protruded as was the other side so you are probably correct in un necessary surgery.

You asked earlier if prostate was involved. I do have BPH it is mainly under control with Flomax and SawPalmetto to help urination. Now I find that urine flow has improved a lot, in fact the bladder is more empty and I have gone all night without getting up to pee. I am thinking that this is a plus side affect. What do you say about that?

Unrelated to all of the above: I have a grapefood tree, and have been eating quite a bit of the stuff, sometimes on an empty stomach. Now I ocasionally get tummy aches which after a time goes away. I think that is the cause, so I am stopping eating them for a while to see if that helps. You think so?

BPH: good to know it is under control with Flomax and SawPalmetto. And as rightly observed the flow is improved with inadvertent dilatation of the catheter or the bulb of catheter. Please see this continues on. Be watch full after 3 weeks as there is a very small chance of recurrence of reduced flow due to body's normal reaction to any injury. This may not occur. Just a precaution.

This is really a great indicator- not needed to get up in the night for pee.

Yes, Sir. Grapefood is a citrous fruit and can cause tummy aches. You have already stopped taking it now. Let us see the effects. Later on you may please take after food. This is the best source of Vit C- very natural and one of the best antioxidants and helps in healing too. ( since you have your own tree one tends to eat freely and a bit more too - just on a lighter note).

OK DR. you have done a good job with me and I will shurely recomend to firiends and family about your site. I will definatly refrain from eating the stuff on an empty stomach and 2 at a time. THe only thing I still have bugging me is that lump in my groin opposite to the hernia operation. It comes out when on the potty, and if I cough .I can press on it and it goes back again. I sure don't want to go through anymore surgery.

I think if the swelling is coming out when on the potty and on coughing on the right side it is hernia.
I would request you to consult another surgeon, get examined , ask Him specifically whether this is of ''direct type'' or ''indirect type''.
The direct type of hernia need not be operated if the neck of the sack is too big.

The only anticipated complication is strangulated hernia wherein the intestinal contents are not allowed to return back to abdomen. Strangulated hernia is an emergency and a bad complications. This usually does not occur if the ''neck'' of the hernia sac is wide.

If the neck is narrow, please get operated whenever you feel so. The only precautions are:: never allow the contents to remain outside of the abdomen, avoid anything which causes cough and constipation ( they are enemies of the hernia), Consult a Surgeon urgently only if the contents of the sac could not be returned back to the abdomen.
