What Causes Blood Spots, Soreness And Enlarged Breasts After Cervical Surgery?

Question: I recently had my mirena coil out after 5.5years.
I experienced light periods with it in for the last year, previously to that no periods at all.
I didn't have any side effects while it was in.
While having it removed I had part of my cervix quaterised due to some cell changes.
I had a ultra sound scan just before having the mirena removed to check it was in place.
The doctor advised I had a very healthy supply of eggs and fertility looked good.
Then my mirena was removed on the 17th of XXXXXXX I had some spotting after the removal for around a week, most likely from the cauterisation to my cervix.
It's now the 8th of March, I have not had a period, I have put on 4kg. I am what feels like depressed, lack of concentration, extremely tired and burst into tears. I have increased excerise and eating healthily but I am still putting on weight. I have very sore and enlarged breasts, I asked my doctor to do a blood test and it showed I have a increase in prolactin, just above the line of what's accepted. He's suggested I come back for testing in 3 weeks. I have done and pregnancy test which is negative.
I am feeling very low and not sure which direction to turn. I have started to worry that my periods won't start and I have messed up my hormones with this coil and will suffer ongoing problems. Any advise would be appreciated

Brief Answer:
Do not worry. It can happen after removal. You need progesterone support

Detailed Answer:
Hello,

I have gone through your query and I understand your concerns about changes you have observed after removal of Mirena coil.

Following is my reply:

1. These changes are expected after removal of mirena coil after a long time of 5 and half years.

2. Mirena keeps producing progesterone hormone. Removal causes decrease in progesterone hormone leading to these symptoms you have mentioned. Body take time to adjust to these sudden changes in hormones.

3. Periods are also delayed due to decreased progesterone hormones.

4. I suggest you start on oral contraceptive pills for next 3 months. These pills will help you overcome the hormonal changes which happened due to mirena removal.

5. After 3 month you can stop pills and expect everything to be fine.

I hope your doubt is clarified. Let me know if you wish any other information.

Thank you for your reply.

I asked my GP to give me a blood test, this is when it showed up I have increase in prolactin. Makes sense as my breasts are large and painful.

I did ask him to test my hormones so surely he would have notified me at the same time if there was a decrease in progesterone? Please advise? He only mentions the prolactin.

One of my main concerns is the weight gain, I spent many years struggling with my weight and finally got it under control. A increase in weight dramatically effects my moods on top of how I am already feeling, tired, tearful and depressed.

That's why after the mirena came out I decided Not to go on the pill. To avoid any weight gain. My doctor gave me the XXXXXXX pill at the time of removal, however I decided not to take this either.

I am reluctant to go on any pill for fear of putting on weight gain. Please advise?

Does the XXXXXXX pill regulate progesterone as a normal pill you suggest going on would? Please advise.

Many thanks
XXXXXXX

Brief Answer:
Start on oral contraceptive pills

Detailed Answer:
Hi,

Estrogen and progesterone are the main hormones responsible for this withdrawal of progesterone symptoms. These hormones are not accurate to measure and hence your doctor has not commented on them unlike prolactin.

You should have taken minipill as per your doctor's advice. As these symptoms were expected.

Pills wont cause considerable weight gain. Take low dose pills like XXXXXXX Regular pills are better than minipills.

Be under exercise and diet regimen for next 3 months to fight weight gain.

Let me know if you need any more clarifications.

Dr. Soumya
