Question: Spell of vertigo in November and lasted about a minute when I woke which left me off-balance and lightheaded. I went to a neurotologist yesterday cleared me from having anything related to my inner ear (he did rotary chair testing) and said its from my central nervous system... He referred me to neurologist... I feel like I'm moving but i am not... My neurologist doesn't know what it could be...do I have to worry if it is something bad

Brief Answer: Further tests are needed... Detailed Answer: Hi, I have gone through your medical history carefully. If MRI and MRA, and blood work resulted normal, then, there is nothing to be worried about. If there was anything bad, it should have been shown in the MRI or MRA. If peripheral causes (inner ear) have been excluded, then it might be related what your doctor said "its from your central nervous system". This refers to the vestibular sensory input which is abnormally processed by the central nervous system. Again, it is not something bad, but it interferes your daily activities. I also suggest to: - get your eyes checked - check your blood sugar levels - monitor your blood pressure - increase the water (fluids) intake - get a hormonal balance In such cases, I ask my patients to observe what might triggering an episode of vertigo. Try to avoid: - coffee, - chocolate, - cheese, - red wine Some of my patients have reported less vertigo episode when do not take such foods. I DONT HAVE VERTIGO!!!!! nothing is spinning on me.. i just feel lightheaded and feel like i am moving when i am not. i had one episode of vertigo in november of last year.

Detailed Answer: Hi back my dear, Feeling lightheaded might be related to low blood pressure/sugar, that's why it is important to monitor these parameters. Next, neck arthritis together with other local problems (possible hernia) of the spinal cord at the cervical level can also be a cause. If everything resulted normal when you got tested and the neurologist reported that it related to central nervous system (cerebellum); then, there is nothing you should be worried about. By the way, you can provide the reports of your MRI, MRA and blood work you did to let me analyze your health problems further. I usually advise my patients to lay down when they experience lightheaded, with limbs slightly elevated. Taking chamomile tea and any other tranquilizer drug can also help. Thank you! Dr.Albana

Brief Answer: Please provide the requested data... Detailed Answer: Thank you my dear, Unfortunately, I have received MR angiography only, which was normal. I did not receive any MRI report of cervical vertebrae. Can you please re-send them? What about any blood work you did? I want also to know if you feel lightheaded all the time or have episodes? If you are having episodes only, do your remember what has triggered it? As long as the reports are normal, it might be related to cerebellum dysfunction as cerebellum is responsible for the equilibrium of the body. This is not worrisome, it happens to some people. Unfortunately, it is happening to you too. It is a fastidious condition, however, fortunately, nothing serious has been found. Looking forward to have the requested data in! Dr.Albana

Brief Answer: Simple exercises help ease symptoms... Detailed Answer: Hi, Please bare in mind that my opinion is based on the data provided by you. In your reports, you are mentioning that your doctor is referring to central nervous system problems related to lightheaded. As long as no organic damage has been showed in your brain (as per MRA report), then, this is a good sign of not getting worried. Usually, the symptoms get improved, but can be repeated again. By the way, only a part of your blood work has been uploaded; unfortunately could not access other tests reports. To my personal opinion, in lack of other tests results, other problems should be investigated and ruled out: - possible migraine aura - uncontrolled stress/anxiety - cardiac-vascular problems If everything is ok, then, I repeat again, the symptoms will get improved, but will show off from time in time (unpredictable). Furthermore, you can do some simple exercises to ease your symptoms. Your neurologist can help too (Epley maneuver, Brandt-Daroff exercises, Semont maneuver). Thank you and all the best!

See other reports..thanks for helping.. Epley doesn't do anything I don't get dizzy from it.. My inner ear is perfect as per neurotologist.. He thinks it's the processing part that is not working.. But need to see a neuro.. My reg neuro says it's bppv but it's nit.. Confirmed by 5 specialists

Brief Answer: Following answers to your queries... Detailed Answer: Hi back my dear, I have gone through your MRI of cervical spine and noticed only herniation without stenosis. That means that the nerves passing through the C5-6 and C6-7 where herniation and bulging are located respectively. At this point, I should think it over again: - At the beginning, my first thought was over BPPV, however, you are saying that 5 specialists excluded as diagnosis. - Next, you are having disc herniation and bulging but no stenosis; so no compression to spinal cord or nerve root is visibly observed. - You are 45 years old, the menopause stage, hormonal imbalances should be considered. - although I do not have any blood work report, I should think over metabolic disorders (oscillations in blood sugar, cholesterol levels, blood pressure, anemia) If everything else is ok, then, it is due to problems with Cerebellum functioning. They are not serious, but they alter your quality of life. Physio-therapy is the permanent solution I advise to my patients together with occasional use of anti-inflammatory/analgesic drugs. I also advise to - cover your neck with a scarf. Although no stenosis is visibly seen; however, my personal opinion is that there might be a light compression to nerve roots passing by that might cause muscle contraction These are associated with a feeling of lightheaded. Easing the contractions through scarf-heating can help ease or prevent the symptoms. - observe what is triggering the symptoms Hope it helped! Let me know if you have further questions! Dr.Albana
