Suggest Treatment For Sporadically Spreading Folliculitis

Question: Hi doc, I hav come to XXXXXXX for medical checkup. My folliculitis is not getting over and now Shoulder is in control but it is sporadically spreading to other parts of back. This is more than 2 months now... Hope u remember me from past queries.

Saw skin specialist and he prescribed following drugs Clariwin 250 mg, Divon plus, Vizylac plus has given me muciprocin ointment to apply. What line of treatment is this, pl confirm, as he was indicating this as normal.

My test results have still not come in, have done all tests but hiv, am scared.

Thanks for your query.

It appears that you have some drug resistant infection. I would advise you to get pus collection done on swab and get gram staining and culture sensitivity testing done. Based on results take the medicine. Taking empirical treatment is not helping you. If you had unprotected intercourse in past get 4 th generation ELISA testing done at 12 weeks. Do not worry I would suggest you to take bath with chlorhexidine wash. Practice hand washing and maintain hygiene. Get hemogram and blood sugar levels checked. It could also be acne get evaluation done by skin specialist. It does not appears to be HIV related.

Thank you doctor. Will share my reports as they come between today and tomorrow for your evaluation.

Also did'nt have unprotected intercourse ever. Also my skin and very doctor here confused me that you can stds even his even with protected intercourse, how true is that.

He also gave me some stress that deep kissing or licking/ sucking the body parts not vagina or anus can also transfer stds. How true is that as I would have indulged in these activities.

Sure upload the test reports when available. Yes sometimes it is possible to get STD from high risk behavior even with protected intercourse. You can get VDRL, chlamydia, herpes and HIV testing (12 weeks) done for psychological advantage only. In your case it does not appear to be STD related but taking the tests is no harm.

Doc, got my vdrl medical reports, some bad news, got rubella igg, CMv igg, herpes simplex 1 igg as high or positive. All my igh results are negative.

Could'nt muster courage for hiv test though..

However the skin/vd specialist I saw seemed to diagnose my boils on shoulders as herpes linked and has asked me to take Herperax 400 and minolux 100 tablets for 10 days and come back for repeat meeting in December end. He suggested that my boils etc should cleAr up with this.

My questions are :

1. Is this correct line of treatment
2. Are these positive igg markers durables
3. Are any of above contagious and how can I protect my family against myself.
4. How do I recognise future outbreaks if any
5. Do I need to take life long anti viral medicines, my skin/ vd suggested no, and we may need viral support only when there is an outbreak. Is that the case.
6. Also what all can I do to strengthen my immune system, as that seems to be the answer to all above, and can reduce the frequency of outbreaks quite a bit.

If ther is any other advice you may want to extend to me, am really heart broken.Apart from above, had done all other tests and majority of the, are ok, some which are out of bound are thyroid, vit D, bit B12 borderline low, billurubin was 1.5 on XXXXXXX 1.0, ESR was 20 ( come down from 23 in October),

haemogram, hepatitis B, electrolytes, fasting sugar, Kft, urine culture were ok, chlorestrol levels have come down from oct, other vdrl tests were all ok, including ige etc.

Also to share my rubella and CMv igg were high 10 yrs back and same skin doctor asked me to ignore them, though he has given medicine for CMv now.

1. Yes even I would advise you to take the treatment of Herpes.
2. IgM is more reliable to detect acute infection. For proper reliability of IgG titers must be repeated twice at intervals of 2 weeks to demonstrate rise in titer. But if value is too high it can aid in diagnosis.
3. You have not uploaded your reports. To prevent family members ask them to practice Hand washing with chlorhexidine wash. Refrain from sexual activities till 2 weeks post treatment. Avoid too much care taking of children and avoid touching too much.
4. If you get recurrence. It probably would be painful vesicles or pustular lesion commonly in the same area.
5. No need of life long treatment. It is a simple treatable disease. Not to worry so much.
6.Don’t smoke, eat a diet high in fruits, vegetables, and whole grains, and low in saturated fat, exercise regularly, maintain a healthy weight, control your blood pressure, avoid alcohol and adequate sleep.
Take steps to avoid infection, such as washing your hands frequently and cooking meats thoroughly.Garlic and aloe vera will be beneficial. Avoid stress. Take multivitamin and calcium supplement. Additionally 1500 mg methycobalamine. Take probiotics if required.

There is nothing to be worried about. You have no chronic ailment. Take the HIV test also. Trust God.

Thank you doctor, will upload reports and then u give me an assessment about his or not. Scared after these results.

Doc, how will I identify repeat outbreak and what kind of medication to take in case of repeat outbreak.

Also what can be reasons for herpes simplex 1 igg positive, how could I have acquired the same, as have not exchanged oral fluids beyond my wife.

Also my wife had Bartholin cyst 1.5 months back which got corrected by warm compress, is my diagnosed condition and Bartholdi cyst linked or I'm imagining too much.

Lastly, pl tell me, when to start prescribed medicines, as I have been asked to take weekly injections for bit d, mixture for b12, thyronorm 25, and one cholesterol tab every day. Can I start with simplex and CMv medicine alongside or delay these.

1. The herpes simplex infection can occur as STD or non STD related. It It is herpes genitalis which is usually related to STD. In your case it could be spread by sharing a cup, eating utensils, etc from strangers. In addition, if you or your partner gets cold sores on the mouth, the herpes simplex virus-1 can be transmitted during oral sex and cause herpes in the genital area.

2. Bartholin cyst is completely unrelated. Nothing to do with your disease. It is common disease in females which is not STD related.

3 Yes you can start the treatment together. No side effects.

Doc my worry is if I have passed these to my family already and if not how I can protect them.

Also how will I know in future that there is an outbreak of cmv or herpes simplex 1, so that can abstain any contact with my wife and other family members.

Immediate worry is to tell them to be away from me for next 30 days at least, not sure what reason can I give there. In case I wear full sleeve cloth and she hugs me, can she still contract these?

How often these outbreaks happen.

Shall I share this with my wife and mother or hide, this is the toughest one as am not sure of the consequences...

Hi

Herpes and CMV are common viral illness and nothing to hide. It is not necessarily sexual activities related and is treatable. Transmission of the virus via routes like sharing bed linen, clothing, towels, toilet seats, eating utensils, shared cups/glasses, and in public spas can also be the cause. The majority of herpes cases are spread through intimate (though not always sexual) contact. Usually sexual transmitted infections, herpes is passed on when someone with an active outbreak of the virus has sexual intercourse (whether vaginal, anal or oral) with a partner. But remember it may also be spread by contact with bodily fluids from an infected person, including blood, saliva, vaginal fluid, semen or any fluid expelled from a weeping herpes blister.Tell your family and take appropriate treatment. Avoid direct contact with family members till lesion subside. Abtain from sex and get your wife tested. I stress on hand washing. Taking treatment will reduce the risk by 75%. If you get recurrence in future it will be in form of rashes which can be pustular or vesicles.

Thanks for above doc, some more queries

1. In above you stated that taking treatment would reduce the risk of spread by 75%, what does that mean? Does that mean I cannot have any kind of kissing or sexual contact with my wife for lifetime now. My doctor who reviewed me stated that I can contact after 15 days of completing 10 days course...and as and when I feel lesions I stop again. Or I can transfer this to my family during normal days as well.

2. I also read that sometimes laboratories provide with incorrect results as well. Would it be worthwhile to repeat the test before starting to take the medicine? Or can start medicine from today and then can get the test done next week as that would complete 2 week gap between two testing saw suggested by you. My worry is whether in this case of second test medicine would suppress the results?

3. Also during my current state when there are lesions, can simple touch of my hand or there hand on any part of my body transfer this virus? Alternatively using same blankets, sleeping on same bed can that cause the transfer? As am banging very weirdly when someone is touching me for last two days and avoiding basic touch contact as well.

The risk decreases by 75% means risk of infection is drastically reduced. Only 1 out of 4 people are at risk with medicine without it the risk is 4/4. You just have to avoid intercourse and close contact till completing treatment or disappearance of lesions. Yes you may repeat the test if you have doubts. No need to panic too much just maintain general body hygiene and practice hand washing.
