Suggest Treatment For Left Ventricular Dysfunction And Extreme Tiredness

Question: sir,
my husband suffered a heart attack myocardial infarction on 2010. he was 58 . he had angioplasty and 2 stants were implanted due to blockage in his heart. now he is 63 years. he is on regular check up, and medicines atocor 10, deplatA 75, sorbitrate, deriphyline retard 150 x2, and losartan 10 mg. his echo report shows left ventricular dysfunction, his EVF is 42. Blood reports are ok , is potassium 5,.2.but recently since 2 months if he takes losrtan5 mg , telma 10 mg or any other BP medicine, his BP is bvecoming lower and he feels extremely tired. he has less sleep. if the bp medicines are stopped , after 3-4 days his BP starts to go up and he feels uneasy. i hav consulted many cardiologist in XXXXXXX but not finding any relief.what to do , shall i stop the BP medicine ?

I had gone through your question very diligently and I really appreciate your concern for your husband
You should not stop his medicine abruptly it could be dangerous.
Looking at your description I would advise you to follow these guidelines
Get his Blood pressure recorded for consecutive 3 days at least 2 recording per day.
His Echocardiography is suggestive of left ventricular dysfunction which means his heart can't efficiently pump the blood to the blood vessels with poor ejection fraction ( 42%)

Here I would like to know more details like
Is he having shortness of breath or cough
Is he having any other associated disease like diabetes or high cholesterol
his weight and built

In my opinion a diuretic group of drug should be added to his treatment like hydrochlorthiazide or Natrilix SR.
He should take healthy diet and exercise as much he can comfortably do
He should take lots of fresh fruits and vegetables
He should take low salt or low fat diet
Please write back to me with these details so that I will be able to tell you more accurately

sir thanks a lot for quick response.my husband is not having any breathing trouble, he is not a diabetic,his body weight is 73 kg height is 5.5. he is not having any kind of swelling like legs or face, his cholestoral is 125,hdl 44,ldl62 vldl19, hb% is 11.5 ,urea 19 ,creatinine 1.2 sodium 142 potassium 5.2.
here doctor told him to stop fruits for a month, he cant tolerate any bp medicine with R , like olmezest R ,HIS BP BECOMES EXTREMELY LOW AND THERE IS A UNEASINESS. WE TRIED PLANE TELMA AND OLMEZEST BUT THAT ALSO IS GIVING HIM UNEASINESS NOW. BUT HE WAS ON BP MEDICINE SINCE 20 YEARS THERE WAS NO TROUBLE AT ALL , ONLY 3 MONTHS THE PROBLEM STARTED. KINNDLY ADVICE. HIS BP IS GENERALLY 128/82 BUT AFTER TAKING ANY BP MEDICINE NOW A DAYS AFTER 2 DAYS IT BECOMES 90/60.WHAT SHALL I DO

Welcome again and thanks for the details

In my opinion he needs moderation in doses of the BP medicines or change over to other group of drugs like diuretic or amlodipine group of drugs.

you should ask his doctor to reduce the doses of telmisartan to the half of the present dose and observe for a week
Since he is not diabetic and not to obese above all his all the reports are normal except postassium which is towards higher side of normal range.
High potassium level might be contributory factor for uneasiness but better BP control is advisable by dose moderation.
