Suggest Treatment For Cough And Heartburn When Diagnosed With Glandular Fever

Question: I'm 42 years old Female. I've been diagnosed with Glandular Fever on the 27th of August. I started having symptoms on the 21st of August. At first the doctors thought i had tonsillitis...so i was given a course of antibiotics Tavanic 500mg (Levoflacine) for 5 days. I am very worried because i am old..and already a week has passed and no improvement whatsoever. I have high fever approx 100-101 F I take panadols for that every 4 hrs. My throat/tonsils are all cover in while pus? and very painful even when drinking water. I have aches everywhere...i cannot even sleep bec the fever is worse when i do. I also have a persistent cough. From yesterday i srated taking Zantac....because I had very strong and painful heartburn. although i am drinking lots of water....my urine is very dark...and i only do a bit. today i noticed that my eyes are a bit yellowish. I don't know what to do the doctors are telling me that there isn't a cure. Since i am old will it last long on me and will i have complications. Pls help me. I don't know if i left something out...because I am not thinking straight. Oh i really don't knoe what to eat. i am also taking Multivitamins and minerals to help me. Thanks

You said your tonsils are all covered with white pus? This is due to colonization of bacteria in the area and there is a specific name for that white material (maculopapular rash).

Will you have complications?
Yes, anybody with this disease may have complications like spleen issues, nervous system involvement, Jaundice etc. I have given details in my answer below for your knowledge as you were very curious looking.


Please don't worry as the disease you have is chronic and long going. EBV Infectious Mononucleosis is a disease in which we usually keep the patients on follow ups and close monitoring. So you need to muster up your courage.

I usually keep the complete blood counts with differentials checked serially to see how the lymphocytes are getting normalized in blood of my patients.

As far as the precautions are concerned, we have to keep an eye, with serial visits, on the enlargement of the cervical lymph nodes, tonsils that may obstruct the airways. If there is cervical lymphadenopathy, nervous system involvement, blood disorders like decreased platelet counts we may think for corticosteroids addition in management. You don't need that right now.


You will have months long to even a year of severe body aches and it is unfortunately the disease like this. You have to do as much rest as you can as this is the way to cope with the pains especially in the start months of this disease.

There is no effective antiviral treatment available for this disease. That's why they used the term no cure for you.
Speaking in general, some antivirals may reduce new viral counts but not so true at least in my clinical practice. There is nothing like that which may help benefit the patients more than the side effects of antivirals.

Precautions:-
*Avoid this contagious disease to get transferred through your body secretions to your loved ones around you.

*Take plenty of fluids and avoid unnecessary physical activity as it may harm you more and will make you weak. Keep normal healthy diet. No specifics.

*Take Tylenol or Paracetamol for fever and pain.

So as such there is nothing I can do. As to follow ups....ideally every when should I visit my doctor? Or the blood tests? Also will the maculopapular rash go on it's own....and will it take long?

Follow ups:
In the start it is recommended every weekly to see your physician for the blood work. Then he may give you further schedule according to the progression of disease. Usually visits decrease once the things settle down to a level a bit.


Regarding rash:
Your doctor is the person who is trained to do your physical examination and see the severity of the tonsil and throat colonization and white colored rash over throat area. If it is very bothersome, he will definitely go for prescription of Prednisone to treat it.
Will it take long??
Depends on severity of the rash and tonsils enlargement. May go away in a week or two.


Home remedies:
Brush and floss the teeth daily.
Gargles with any suitable mouthwash.
Plenty of fluids to drink.
Normal diet. No specifics.
