Question: Hi, I am 53 years old and I started on HRT's in 2018. The one I take is Activella o.5/01mg which is estrogen and progestin. Every year I have my mammogram and they have come back negative and my pap smear as well. On Oct 16, 2018 my breast were feeling very heavy and hurting so much. I went to the doctor and they did another breast ultrasound that showed nothing. I was so stressed out and worried and then 2 days later, I started bleeding. Not alot of blood but enough for someone who is 53 years old. The blood last for a like 3 days and then stopped completely. I went back to the doctor and she did a transvaginal ultrasound was done on 10/2/20, that showed: my uterus is anterverted and measures 8.9 x 4.7 x 4.8. the myometrium is homogeneous. a small solitary submucosal stable fibroid in the fundus measuring 1.7 x 1.6 x 1.5 cm previously in 2017 it was 1.5 x 1.3 x 1.5 cm. The endometrium measures 12.0 mm. The endometrium is unremarkable. the right ovary could not be visualized. The left ovary measures 2.4 x 1.5 x 1.9 cm. No ovary abnormalities are identified. There is no free fluid in the cul-de-sac. No adnexal masses are identified. Please keep in mind in 10/02/2018 I had another trans-abdominal exam done and then my endometrium was 4 mm but they claim to have seen both ovaries at that time of exam and measured them. The fibroid was shown then as well and measure 1.5 x 1.5 x 1.3. Of course I have a concern about cancer but my doctor is trying to tell me I should not be worried. He's thinks the thickness is coming from the HRT and not cancer. I don't know what to do and need some guidance??? Could it be correct what he is saying? If I need to get a biopsy done what type should it be? Meaning a hysteroscopy and myosure or just a DNC or what to cover both situations meaning the fibroid and the endometrium lining and do I need to consider a hysterectomy or what? Also, based on the removal procedure , do I need to worry if it is cancer once they cut it to remove it, will it spread? I am lost and need guidance... Thanks so much...

The increased thickness of the endometrium can be related to the hormonal therapy that you are having. The fibroid shows a minor change and is not suggestive of any malignant change. A D and C is sufficient to rule out any endometrial abnormality. To investigate fibroid, proper surgery to remove the fibroid or uterus is needed. Once they cut and remove the uterus then there is nothing to worry in future.

A D and C is sufficient to rule out any endometrial abnormality. To investigate fibroid, proper surgery to remove the fibroid or uterus is needed. Once they cut and remove the uterus then there is nothing to worry in future.

So, are you saying that I don't need a myosure procedure? You feel a D & C will be good for both procedures? Also, you agree that the hormones that I'm on now could be causing the thickness problem? If so, why did it not happen before since I've been on them since 2018. I'm very concerned about CANCER. Should I still be taking the hormone therapy until I get the biopsy or should I discontinue it now? What is the possibility that this could be cancer considering my age of 53 years old, on HRT since 2018 and my lining being at 12mm now? So, do you think I should automatically have my uterus removed?

Good Morning! I am not trying to be difficult by asking things that seem to be repeated. But, I am truly trying to get a clear understanding of things. See this is the problem that I go through here with some doctors. I truly believe that doctors forget that we as patients did not go to medical school so we are not as knowledge as you all are and need a little more clarification from time to time. You stated CANCER unlikely. I would like to know in your professional opinion how do you come to say or feel that way?? I'm truly stuck on endometrium lining of 12mm and 53 years old on HRT to me that just reaps of a red flag to me. But, once again, I'm not a doctor. Everything I have read online is saying just the opposite so this is why I need for you to please explain your response to me in layman terms. I will not be able to see the gynecologist here for another 3 to 4 weeks because of COVID19 so I don't know do I stop the Activella now or will it be safe to continue it until I do see him? I don't want to be creating any further unnecessary damage if I don't need to. I read CANCER feeds off estrogen and i know Activella has estrogen in it so is it ok to continue or it doesn't matter at this point? If I did stop the Activella now and had another ultrasound performed so you think my endometrium lining would go back within normal ranged? Do you have any ideal of why my Prolactin level would have come back a little elevated as well? Is there any particular diet I should be on now to help with this endometrium problem? I spoke with another doctor and he said i should do hysterscopy myosure instead of a D and C. So, I need to know which procedure is truly recommended for what I need done? Which removal process gives you more protection of being able to get a viable sample and also limits you from spreading the disease inside of you once they cut it should you have CANCER? I've always been told, once CANCER is cut open it spreads like wild fire. I know I have asked alot and I truly appreciate your knowledge and patience. Thank you so much and be safe!

Hello.
Endometrial thickening of 12mm is just borderline thickening. Less likely to be related to cancer as it can easily be caused by hormones you are taking.
It is best to avoid taking HRT for next 4-6weeks and get a repeat ultrasound scan done to see if endometrial thickening decreases.
Dietary changes are not going to help.

I'm now starting to understand what your saying. I a stopping the HRT today and I will plan to repeat the ultrasound in 4- 6 weeks to see if my endometrial lining has decreased. Is it going to be ok if I start bleeding from not having the HRT? Do you think I will start bleeding and if so, I thought that was bad especially since I'm post menopausal? Will that cause me to get cancer should that happen? Help me now to understand this: (if it decreases does that prove anything? Meaning does it rule out CANCER or not because I have read that some people have in range endometrium lining and still have endometrium cancer. I just don't want to either rush or delay the inevitable if that is the case. I had an opportunity to speak to a Dr. by telehealth on zoom and of course, he never mentioned to stop HRT and wait 4 - 6 weeks and retake ultrasound. You stated previously about me being anxious. It's not that I'm anxious, I just don't want to wait and it be that I have cancer and could have already been receiving treatment. That's all. I did think of doing the same thing that you said before but I just did not understand what were the possible outcomes. That what I wanted explained to me???? The DR. said that endometrium cancer is the #4 killer of women and you don't want to wait. He said I need to get a biopsy right now like as soon as possible. He said you could wait and the cancer progress to a higher stage quickly and you be dead sooner than you think. But I don't know if I agree with him or not. Tomorrow, I am scheduled to have my pap smear should I keep that appt and should I say or do anything differently with the Dr? It's being done by my primary care doctor instead of a OBGYN. Listen, I know your not my actual Dr but I like how informative you are and I'm going to follow your instructions even though this doctor is trying to immediately run me into the Operating Room without even allowing me time to come off the HRT and retake the ultrasound. I wish you were here and could be my doctor because the doctor here is truly not thinking clearly.....

Hello, I totally get it now. My plans are to go to visit the new gynecologist tomorrow and have a pap smear only. After 4 to 6 weeks, I will look into having an ultrasound and go from there. What would be the acceptable range when I take my next ultrasound? My stomach has been feeling really crampy today even though I'm still on the Activella.
