Can HIV Be Contracted Through Non-penetrative Sexual Activity?

Question: Hi Doctor,
Sorry for my bad English. I sucked a prostitute's breast for 2-3 minutes(didn't felt lactating). No other contacts or intercourse. After 6 months i had strong flue, headache, 1.8kg weight loss and some small red spots in my arms and shoulder. Some red spots disappeared. some new spots came. i had two armpits and 2cm swelling on the back of my neck.burning sensation in palm and skin. I am very much scared of HIV. Am not getting sleep. I have frequent urination tendency. I depend on tablet for sleep. Doctor prescribed Nexito plus and zolsomo. I went for HIV test named ECLIA antibody. Result is non reactive with value 0.2675, Then again went for HIV ELISA antibody test ,Result is non reactive.(Tests are done after 6 months and 10 days).
Will HIV pass through breast sucking? Is there any chance of HIV through breast fluids?
Is these tests are effective(i think those tests are 3rd gen)?
Do i need to test it again and again?
Why I am having these symptoms if i don't have HIV? I didn't have such red spots earlier.

I understand your concern and situation.

If a adult is suffering from HIV then breast milk can contain very tiny amount of HIV. This type of infection is likely in baby who are breast Fed continuously but highly unlikely to be infectious to an adult individual. As your case you had no sexual intercourse so it is highly unlikely. Just one episode of short duration of breast sucking is unlikely to infect HIV. Your tests are highly reliable 99.9% done after 6 months post exposure even if it is antibody based. You relax and remove your fear of having HIV infection. As for your rashes and swelling it could be lymphadenopathy due to viral or tubercular infection. Get evaluated by a physician. Get complete haemogram, ESR, X RAY and sputum examination done. Weight loss and swelling of such type are seen commonly in tuberculosis. FNAC from swelling can be done to examine AFB bacilli and cytology under ultrasound guidance. Do meditation, eat healthy and protein rich balanced diet with multivitamins and calcium supplements. There is effective antitubercular drugs available which cures the disease.

I planning to get married soon. Thats why i am nervous and scared of this. I don't wana spoil a girl's life. May i trust this tests and get married? I can't concentrate on work.
Will any other kind of STD pass like this? with similar symptoms?
If amount of virus entered is very very low. will it cause late development of antibody(about 6 months or more)?
What is the difference between ECLIA and ELISA ? Which is more efficient?

Hello dear.
Don't worry at all. Yes you can certainly trust these tests and get married without any worries. You had no sexual contact so there is no possibility of HIV infection. Yes sometimes very rarely some STD like syphilis, herpes, skin infections can be contacted if the person was having active lesions in breast. But it doesn't seem to be in your case. So don't worry. These diseases have altogether different symptoms. HIV antibodies usually develop within 6 months post exposure. In very rare cases it gets delayed.ELISA is a 3rd Generation test detects antibodies, whereas ECLIA is a 4th Generation enzyme immunoassay that looks for both antibodies and p24 antigens.
ELISA can detect up to 99.9% infections at 6 months. ECLIA is an advanced test 4 th generation detects both antibodies and p24 antigens. It detects cases as early as 12 weeks post exposure. So in your case there is no possibility of HIV. Trust me no need to worry. Get happily married.

I had vaccination to prevent tuberculosis. Last question..In your answer you said..In very rare case it gets delayed. Why so? Any previous cases about delayed in detecting antibody. Is there any of other kind of armpits swell other than lymph gland swelling? how can I differentiate? There is a itchy small swelling about 2cm on backside of my neck. Is it a lymph gland swelling. ? Do i need to test after 3 months?

BCG vaccination is not 100% effective. It is important in preventing meningitis in neonates and children. Antibody detection can be delayed but not antigen. As you have got tested for p24 antigen so there is no possibility of infection in your case. No need of further tests required. As for swelling do not make diagnosis get yourself evaluated from a physician or dermatologist. Lesion associated with itching could be due to fungal infection or allergies. Swelling in armpit can be due to various reasons fat deposits lipoma , lymphadenopathy,bacterial infections, allergic reactions, adverse reactions to vaccinations
viral infections or rarely noncancerous fibrous tissue growth. Most lumps are harmless and are usually the result of abnormal tissue growth.

Thank you Doctor. I didnt take antigen test. Bothe Eclia and Elisa were anitbody tests. Do i need to take a antigen test. Is there any other tests to get a 100% accurate result? What is viral load test? is it effective?

ECLIA (Electrochemiluminescence immunoassay) test 3rd Generation tests that look for antibodies to the HIV virus is what you got tested. They have a similar window period (4 weeks to 3 months). 99.8% accurate after 12 weeks up to 95% of infections being detectable within 4-6 weeks. .The 4th generation tests are blood tests that looks for antibodies and p24 protein antigens.4th Generation tests have an accuracy rate of 99.9% after 12 weeks, and most HIV specialists consider test conclusive at 6 weeks.PCR viral load is the most sensitive test for detecting HIV infection. Qualitative PCR TEST detects viral DNA and RT PCR TEST detects amount of virus DNA called as viral load and detects as few as 50 copies/ ml. These tests are nearly100% sensitive and yes they are effective and accurate.These tests are positive as early as 4-6 weeks. In your case 6 months have passed so no need but to relieve your anxiety get tested by 4 th generation ELISA TEST.
