Get a gym membership if you're low on budget
Get a bow-flex and have a professinal install it if you're rich
Work out 3x a day, 4x a week, work on a different muscle type a day
1st day-chest, 2nd-arms, 3rd-legs, 4th-shoulders&back
and a little ab workouts and cardio(running) in the mix
you don't have do in that exact order, just switch it up everyday

eating diet- no fried foods, incorprate a lot of veggies in your diet
if you're gonna eat meats, try baked/boiled/grilled with non-fat oils like
virgin olive oil
when it comes to eating, don't eat a lot, you're only supposed eat till
you're SEMI-FULL, a lot of people eat till they're full and get obese that way.....

Breakfast-Eat light
Lunch-Eat 2-3 small servings
Dinner-2-3 small servings with rice or spaggetti so you have energy for tomarrow morning

and remember to work up to from where you feel comfortable, you can't start lifting 150 pounds in the beginning....

Hi
You need to understand that certain medical diseases can also cause weight loss and weakness. You need to consult with your Doctor for a thorough evaluation to rule out diseases. Eat a high protein diet rich in cereals, pulses, chicken, eggs, lean meat. Eat plenty of fresh fruits and green leafy vegetables. Drink plenty of water. Do daily exercise. Do Yoga to get stress relief. Try weight gain techniques at your Gym to help gain muscle mass as well.
