What Does My Liver Function Test Report Indicate?

Question: Hi I underwent liver function test as a routine checkup as I was concerned about yellow stool. Report shows abnormal value of GGT (143) (report attached) . I am also attaching an earlier LFT done in August 2014. I am slightly obese on no medication . General fatigue and drowsiness are the major symptons. I have had Heb B and Hep C tested which are normal and USG abdomen was also normal.
What should be the next course of action?

Helllo XXXX,

I checked your reports.
The yellow color of the stool is common when the bilirubin results are normal.
GGT in your liver enzymes was increased and went over the previous year. It is mostly due to alcohol consumption or Obesity.
Since Hepatitis, Ultrasound abdomen are normal I would not suggest any further testing.
Instead if you are alcoholic you should rapidly go into teetotaler mode. I expect GGT to come back to normal after abstinence. If you are not alcoholic you should get on to exercise and diet modifications to reduce weight.
The other tests that would add value here is Serum Vitamin D3. I would like to see Fasting Blood sugar and HbA1c as well in such such cases.
I would not comment on General fatigue and drowsiness without any evidence on the labs.

Hi Doctor I am attaching reports of USG,HBsAg and D3. Fasting glucose is normal I dont have report as such. I have read that eleveated GGT is indicative of heart failure. How can I cofirm that?

Hello,

YOu have not mentioned about smoking and alcohol. I am interested in that first.
Isolated GGT raise has nothing to do with heart failure.
Generally we do not investigate for heart failure. There are obvious signs and symptoms of heart failure like breathless XXXXXXX swelling of feet, abdominal pain and infact all of them.
A doctor can check the signs and confirm heart failure.

Since there is insufficient Vitamin D3. I suggest a 2000IU per day CALCIFEROL tablet for 4 weeks. THis might correct GGT.

Hi Doctor,
I do not smoke or drink. I will take Vitamin D3 as prescribed by you. One more piece of information is that I had a GGT level of 250 in XXXXXXX 2013. Could it be Hemochromatosis or Wilson's disease?

Hemochromatosis and Wilsons disease are rare entitites, with other liver enzymes being normal and Ultrasound being normal I would not think so.

However I would refer to a gastroenterologist if a doubt exists. There are specific tests that are done to screen these diseases.
