What Does My Lipid Profile Test Report Indicate?

Question: I had my LFT and lipid profile done on 10 OCT 2016. Test results are as below :
LFT

Bilirubin Total - 0.35mg/dl
Bilirubin Direct - 0.17 mg/dl
Bilirubin Indirect - 0.18 mg/dl
AST - 39.6 U/L
ALT - 42.6 U/L
Alkaline Phospate :-103.0 IU/L
Lipid Profile
------------------
Total Cholesterol : 238 mg/dl
HDL Cholestrol :43 mg/dl
Triglycerides : 356 mg/dl
LDL Cholestrol : 159 mg/dl
VLDL : 71.2 mg/dl
I was put on medication for lowering cholestrol - Arbitel AV (atorvastatin 10mg) till 10 Dec 2016. I was taking alcohol everday (180ml) while on arbitel AV.
For the past two days I had very severe gastric problem and burning senstion in stomach just where the oesophagus ends . I had a nexpro injection but still today morning i was not able to pass the gas. In the morning I was throwing up and could see a slight blood in the vomit once. I got worried and went to a gastro . The LFT and lipid profiles were again taken and the results are below:

LDL Cholestrol : 61 mg/dl

1) Why I am seeing an increase in the LFT AST/ALT values as high. Is it because of the statin I have taken. Will the liver enzymes increase due to statins.

2) Why there is not much of a change in cholesterol level. Is it because of continuos drinking with statin. Why tryglycerides are high.

3) Is a slight bleeding a matter of concern. Is it because of stomatch ulcer or varices. Hope it is not varices. How do we differentiate between ulcer bleeding and varices bleeding.

4) I am going for an endoscopic test for stomach this saturday. What should we look forward in endoscopic. They told it will be under anasthesia. what is endoscopic under anasthesia means.

5) How to treat the burning sensation in stomach. Have taken zintac and was relieved rather than sompraz-D. Can we take ZINTAC 150mg

1. The abnormalities in lab values are directly related to alcohol intake and it's effect on the liver and cholesterol metabolism.

2. Cholesterol metabolism is long lasting and it takes time time to resolve with statins. Elevated triglycerides are directly due to long lasting drinking pattern.

3. Yes, slight bleeding can be a matter of concern in presence of alcoholism.In all such cases , varices have to be ruled out..Both bleeds can mimic each other, but in general variceal bleed is very heavy.

4. Endoscopy of the Upper GIT can clarify all the doubts.

5. Burning sensation can be well treated with Oral Pantoprazole rather than Zintac. Use Sompraz only when H pylori bacteria are demonstrated on this due examination obtained on Endoscopy.

Thanks for your response.

1) Are the LFT test values (AST/ALT/ALP) very high and a matter of concerns. How do we get it down. Can I use ayurvedic medicines like Keela Nelli (Phyllanthus amarus, or niruri)

2) Should I still continue with my statins. Will it bring down the tryglycerides and other values.

3) I had an experience of endoscopy without anasthesia, is it safe to have anasthesia for endoscopy. Any precaution to be taken before the test.

4) Should I have to stop alcohol totally or can I cut down on the quantity and frequency. Please explain.

Sorry for late reply as I was busy with my other professional obligations.

1) LFT values are mildly elevated. My best suggestion is to avoid any alcohol or medicine that can affect liver. Drink cleaner water and clean hygienic food. Syrup Liv 52 1 tbsp daily can be taken to protect liver.

2) Since statins can also affect liver, please avoid taking them until your liver tests are normal. Though they are helpful in bringing down the triglycerides.

3) Mostly some sedation is enough for endoscopy. But further recommendations are done by treating surgeon. Yes general precautions are applicable to you.

4) You should stop alcohol totally rightaway.

Thanks for the answers.

1) My symptoms : Heartburn, alternate diarrhea and constipation, severe gastritis, burning sensation in stomach, bloating, belching, feverish feeling after eating food etc.
These symptoms seem to be of H.pylori. Can you confirm that based on the symptoms?

2) For the past two days, I am on Sompraz L (40mg) once in the morning in empty stomach, Flagyl 200mg (twice daily) and Gaviscon. Can I start Clarithromycin too? What should be the dosage and strength?
How long should I continue these medications? List all side effects too.

3) I am reluctant to go for an endoscopy now. List another lab test (blood/stool etc) to confirm the existence of H.Pylori.

4) Sorry to say this, but I am really not able to stop alcohol at once. I have cut it down to 90ml per night . Will these have any adverse reaction to the above medications?

5) What should be my diet now? How long will it take to come out of H.pylori and what precautions need to be taken to avoid reoccurrence?Is this very serious ?

I will be addressing to your questions in detail-
1) Yes, these symptoms are consistent with H. Pylori, though its difficult to confirm it based on the symptoms alone. Even, acute gastroenteritis may present with the above symptoms.
2) Since you are already prescribed with Sompraz, Flagyl and Clarithromycin, you have been initiated with H. pylori treatment already. Yes, you may take Clarithromycin, which is an antibiotic, of which the dose and the strength should be prescribed by your treating doctor. It is normally taken for a period of 7-14 days.
3) Urea breath test and stool antigen test can be done to test for H. pylori.
4) It would be good to completely avoid drinking during the course of medicines.
5) You are required to stick on to a diet which is timed with an interval of 3 hours and not more than that, not taking main meals, rather taking small meals. You can take a diet comprising vegetables and fruits. Less spices, fats and oil. Also drink boiled and cooled water.
