What Does My MRI Scan Report Indicate?

Question: Hi Doctor,
Im 37 year old weighing 86 kg. Im very very tensed with my situation and relly do not know that how and who can help me.
1- in year 2010 i got a sudden attack and went to emergency at hospital. It was said that its panic attack.I got it frequently hence MRI's and everything was done. I was put on Nexito 20, petril 0.5 and othe rdeatives.
2- Then started nerve tingling, sometimes I had needles pinching in my hands and all, sometimes vibrations in leg along with gossebumps in right head. Left side was most affected. I changed many doctors and they had put me on Duloit 20 for 4 years.
3- Suddenly i had numbness in hands while sleeping do I went to hospital and they said its cervical. They prescribed Fluadc 20, Gabapentin NT, Pregablin 75, Voveran Plus .. I was ok so left these medines.
4- Now, the common thing is that anxiety never went away.. I always have to keep petril with me. Im sick of it.
5- Iam attaching my MRI report with this.
6- Now, I have stopped eating all medicines and I have got these anxiety attacks at least 6 times in last 1 month. Suddenly BP rises to 150/100 and heart rate becomes 120-130.
7- with these episodes i went to cardiologist. ECG, stress echo was done..everything came normal but i had high pulse and heart was not relaxing. So, he gave me Beta blocker earlier 25 mg but it redcued my BP too much. So now im on Ivabradine 5mg twice for 1 month. Despite that these attacks are very common.
8- I have so much pain in my left shoulder neck area and limbs. and I feel tingling in palms of right hand.
Now, Im very scared to go out so that I dnont feel embarrased. Im always thinking about that I should not get it again but nothing helps. I know I have to breathe, be positive...but nothing is helping me doctor. Can you please suggest something and help me live again? Why am I getting thee attacks?..
my thyroid is normal TSH 1.6..I have also done USG of neck where some nodules are seen.. I will go to thyroid specialist for that..Im hypothyroid since 12 years and taking 100mcg thyronorm. Alos taking 500 BD glycomet for insuin resistence.What wrong with me doctor?
Can I be cured?

I am Dr.Aay Panwar,a neurologist, here to answer your query.

I have gone through your detailed history and MRI report as well. Honestly speaking, the symptoms you had mentioned appear to fit closely to severe anxiety and panic attacks only.Symptoms are mostly non-specific which do not fit into a specific disorder.

However, neck pain and left shoulder pain can well be explained by your C5-C6 disc osteophyte complex.

Further, thyroid function if abnormal, can result in non-specific symptoms at times.So, its advisable to get a latest Thyroid function tests done.

Ok Thanks Doctor,
so for anxiety can you let me know what medications I can have? It will be good if you can let me know. It might help me to get out of it? Is there any treatment to my cervical issue as well? Please suggest some medication.

Brief Answer:
Clonazepam/Pregabalin/Duloxetin/Methylcobalamin

Detailed Answer:
Hi Shailly,
Thanks for being in follow-up.

Clonazepam can be used for anxiety.Initially it can be used in low dose(0.25 mg twice a day) which can later be increased to 0.5 mg twice a day,depending on the response.

Further for your cervical issues, the following typical prescription may help-
1)Tab. Maxmala forte once a day( Pregabalin-methylcobalamin combination
2)Tab. Sympta(Duloxetine) 20 mg once a day,which can be increased to 30 or even 60 mg once a day depending on the response.
3)Tab. Shelcal once a day

Along with medicines, cervical spine exercises are mandatory on a regular basis.You should learn them from a physiotherapist and do everyday.

I would further suggest to get 25-hydroxy-vitamin D levels checked in blood and if detected low,they should be corrected.

PLEASE TAKE MEDICINES UNDER A PHYSICIAN'S GUIDANCE ONLY.
