What Does Headache In A Diabetic Patient With These Blood Sugar Levels Indicate?

Question: My husband ( age 45 yrs ) is diabetic since 2007 .he is on Glycomet DT and dibecon DS. Today he experience shaking and headache due to sugar down. He immediately took electrol and ate one laddu. He felt better after eating. But he still have a strong headache near both ears. What can be done to get read of the headache of low sugar. He often gets low sugar and have to eat somethibg immediately. His current sugar is 105 fasting and 135 PP.

Hi,

Thanks for you question.

After going through your question, I can understand your concern.

Your husband's fasting and PP BSL shows good glycemic control.

Tab Glycomet contain metformin.
Metformin decreases insulin resistance and no action on insulin secretion hence rarely cause hypoglycemia when used alone.

Tab Diabecon DS is a herbal preparation it increases insulin release and cause hypoglycemia.

I would like to advise -
1) Document hypoglycemia by checking blood sugar level by glucometer when he feels symptoms of hypoglycemia.
2) Try to avoid skipping meals, follow medications timing and meal timings regularly.
3) Check HbA1c level.
4) Check his Renal and Liver function test as derangement in kidney and liver function can cause hypoglycemia.
5) If he has recurrent hypoglycemic episodes (documented with glucometer readings) despite of above measures there is need to reduce dose of medicine which causes insulin release ie. Diabecon DS. (After discussing with this diabetologist).

Head ache caused by hypoglycemia usually disappear after correction of hypoglycemia and rest.

Some time hypoglycemic symptoms disappear immediately after eating sweet for some time but patient can again go in hypoglycemia as medicine in blood is still working that's why after having load of glucose to relieve hypoglycemia immediately patient should take some complex carbohydrates like oats with milk, tea with marrie biscuits or any meal containing wheat bread so that there is continues slow absorption of glucose for longer time.

Best way to treat it is to avoid hypoglycemia.

In resistant case headache he can use paracetamol as pain killer.

Thank you Dr.Abhay. My husband is ok now. I went to eye specialist yesterday because he had a blur vision. She told his eye no.is increased but may be the blurr vision was due to hypoglocamia. His vision is cured now. But he is coughing now and while coughing headache reaches to मस्तक. headache stoped since morning but slightly started fron evening again. He also experience itching while urinating since 6 months., frequent urination at night. But his sugar never exceed than 200. But HbA1c on 26.08.2015 us 6.3 .....should he change medication completely ?

- HbA1c of 6.4 indicates very good blood sugar control.

- Considering good HbA1c and recurrent hypoglycemic episode, I may have advised to increase the dose of Metformin (Tab Glycomet) to 1.5 gram per day and reduce the dose of Diabecon DS to half of current dose with rechecking of blood glucose level (F & PP) after 4 to 6 weeks and if possible will stop Tab Diabecon.
Along with this he has to follow regular exercise (150 min per week), follow diabetic diet.

- His head ache may be due to his eye problem (refractory error).

- For itching of urine it is advisable to do urine examination. And to drink adequate water.
