Posted on Sat, 17 May 2014
Question: HI, I AM 50 YEARS OLD AND STOPPED HAVING REGULAR PERIODS TWO AND A HALF YEARS AGO, I LAST HAD VAGINAL BLOOD SPOTTING 7 MONTHS AGO. I HAVE HAD A RECURRENT DISCOMFORT IN THE RIGHT LOWER ABDOMEN FOR MANY YEARS. A VAGINAL ULTRASOUND APPEARED NORMAL. MY DOCTOR SAID THE PAIN WAS PROBABLY FROM ADHESIONS FROM AN APPENDIX REMOVAL WHEN I WAS 16 YEARS OLD.
I AM WORRIED AT THE MOMENT AS OVER THE PAST SIX WEEKS AFTER I HAVE SEX OVER HALF A DAY LATER I HAVE STARTED HAVING A SLIGHTLY YELLOWY BROWN WATERY VAGINAL DISCHARGE LAST LASTS FOR APPROX 24 HOURS. I DO NOT HAVE PAIN DURING SEX . THIS HAS NEVER HAPPENED IN THE PAST, WHAT COULD THIS BE?

Brief Answer:
There are many possibilities for your problem.

Detailed Answer:
Hi,

I am sorry to hear about your difficulty. If I understand correctly you still have your tubes and ovaries and uterus. The recent GYN ultrasound was normal. I assume that you also have recently had a Pap smear and if appropriate you have had STD testing.

Because the vaginal flora can sometimes shift and produce abnormal discharge with or without an odor, I think that treatment with an antibiotic may help your symptoms. It is important however that someone do an exam in make sure there's not a cervical polyp which can also produce the kind of symptoms that you describe. In other words, I hope that your gynecologist did more than just an ultrasound.

If all possibility of abnormal growth of bacteria has been ruled out and there's no sexually transmitted infection then this may have to do with the lack of estrogen that begins at menopause. Tissue thins. If you're having regular sexual activity it may not be painful but it may be enough friction to produce discharge that is brown or rust colored. Also some of the lubricants act by pulling moisture into the vagina and can sometimes produce some strange colorations. I would encourage you to use of over-the-counter water-based lubricant with intercourse and if that is not adequate to consider discussing with your gynecologist either local vaginal estrogen or the new selective estrogen receptor modulator that turns on the estrogen receptors in the vagina without actually being in estrogen.

I hope this gives you a place to start. I hope you feel better soon.

Thank you. I am due a smear test and had the ultrasound approx 2 years ago. It sounds like it would be a good idea to have other up to date tests.
The watery discharge has happened even without lubricant. Initially I thought it was urine but later realised it was not. Could this problem be due to uterine lining problems. The other strange symptom I have is calf discomfort (right leg), which I notice in bed and is similar to the sensation I had when I had a disc problem 4 years ago (sorry I forgot to mention this) though I am not presently having back problems.
I have had no other tests

Brief Answer:
It is definitely time to update the ultrasound.

Detailed Answer:
A combination of ultrasound and pipelle endometrial biopsy (in office procedure) should be able to answer the question regarding the health of the lining of the world. I misunderstood and thought the ultrasound have been recent. Two years ago does not apply to the current situation. It is also time to have a physical exam for all the reasons stated previously. Finally the calf pain may be related to disc disease even if you not are not currently experiencing back pain. It is in the distribution of the sciatic nerve. The other common reason for calf pain is lack of good circulation either because of atherosclerotic disease (hardening of the arteries) and it is also important to rule out a blood clot in the venous system. Please see your doctor and be thoroughly checked.
