Thanks for your query ,based on the facts that you have posted it appears that you have End Stage Renal Disease.

The main function of the kidney is to eliminate end products of protein metabolism (Urea and Creatinine)and produce urine .As there is significant irreversible damage to nephrons and glomeruli in ESRD the production of the urine is diminished gradually resulting into reduction of the urine output and constipation due to water imbalance which is the most important factor in formation of a soft stools.
