What Causes Recurring Fever, Nausea And Weakness Despite Having Normal Blood Reports?

default
Posted on Mon, 21 Sep 2015
Question: My wife got fever on saturday around 102 to 103. She did not had any throat infection or cough. I had to put wet cloth on her forehead to get the fever down. Then we consulted the doctor who diagnosed it as a viral fever and advised  the following medication

Tab Naprosyn twice
Tab Lorfast 10 mg twice
Tab sompraz on empty stomach twice
Cap Lycomerve once
Tab Dolo on SOS

The same day in the evening, the fever went down and remained in the range of 99.5 to 100 for the next 2 days. We again met the doctor and he advised to go for the Complete blood count, blood culture and sensitivity and dengue test. The reports are attached.

The dengue test came out yo be normal but the TLC and platelets were slight higher. The doctor then asked for a jaundice and liver test which we took today. The reports are attached.

The jaundice was negative but the liver was quite high (please check the reports). The doctor said that this usually increases with fever so nothing to worry and asked to take vomiting and nautia medicals for 4 days.

Now the fever most of the times is normal but some ting it's 99 or XXXXXXX 99.5. Also there is a feeling of nautia, vomiting and weekness. Can u please check the reports and let me know if u see something alarming ?

Thanks
XXXXX

Brief Answer:
Here are a few suggestions and views on reports

Detailed Answer:
Hello,
Thanks for your immediate response and uploading of lab reports XXXXXXX

It's a great set of reports every thing is perfect. Her platelets are on the lower side they need to be 150 + but they are 105 but as dengue and typhoid are negative so no need to be worried I will tell you what to do about that, later in the answer.

Her Alt is raised indeed and that is a significant increase, These can be because of fever but this can not be neglected , here are a few suggestions.

1) Don;t panic most of her labs are OK.
2) Ask her to drink loads of water.
3) Repeat a platelet count in 7 days and see if they are getting better, if they are going down,, talk to your doctor.
4) Your wife shouldn't take any pain meds they affect liver.
5) Discuss with your doctor if she recommends Hepatitis B-C done to rule out liver damage due to this cause.
6) Repeat Liver enzyme tests in 15 days and see the change if they are getting better then every thing is fine otherwise this is not a normal raise to ALT due to fever.
7) Keep taking recommended meds but read about their side effects if they do liver damage ask your doctor to consider changing it.

I hope your wife's next test reports would be much better.
Just encourage her to have good food and relax.
I Hope this answers your question, if your have more queries feel free to ask, otherwise close the discussion and rate it. Wish you the best of health.

Regards,
Dr.Muhammad Ahmad.

thanks for your reply doctor. just another questing, she is feeling better now and have no fever, also here felling of vomiting and nautia has subsided. I came to my home town for the checkup and was planning to leave her with my parents for a week for rest. Now when she is feeling fine, would it be ok to take her back with me and she can start going to office?

also, when I check her fever on a conventional mercury based thermometer vs on a digital thermometer, I see a huge difference. can u let me know which one is more accurate?

Brief Answer:
Yes things look great

Detailed Answer:
Hello,
Thanks for writing back,

In my experience mercury thermometer is more reliable, so I would advise you to trust that.

In the circumstances you have described and how she is improving in my opinion she can start her office though but if she can get medical rest easily without much problem and she is feeling weak and lethargic then she should take some days of rest if not for a week she can take rest for two to three days.

But if she feels ok to join office or getting medical leave is a tough thing in her job then she can join office in my opinion. Yes she should try to take as much rest in office as she can and don't do any extra exertion if not required, or if she can manage things without that extra exertion.

Do get the repeat of her tests as i told you and see if her labs are getting better or not.

I wish her a healthy brisk recovery,
Have a great safe weekend stay blessed.
Dr.Muhammad Ahmad
