What Causes Painful Urination, White Flakes In Urine And Semen?

Question: So I've been having this pain when I urinate. After I urinate it does not itch. Sometimes while urination I have these white flakes come out with my urin. While all of that comes out it burns so bad, but no sores or discharge is present. I would have a streak of days when it doesn't burn but just today I masturbated and noticed that a couple hours after masturbation I got the same white flake like stuff coming out. I put it in a cup to examine it and it's very cloudy? If any doctor is interested in seeing what this cloudy urine looked like then please feel free to email me! I can't tell if it's a UTI or chlamydia or I don't know?

This is Dr.Riyanka and I am glad to be helping you with your query.

From the symptoms that you have described , it sounds like after masturbation there is still some semen left in the urethra due to which when you urinate these whitish flakes are first ejected out before the urine can pass .This process can lead to burning sensation or pain.

Kindly , gently squeeze out the left over semen by massaging n running your fingers from the bottom of the male organ or take care to urinate before masturbation.

Also to be reassured of any urinary tract infection , kindly visit a physician and get urine tests done:
1.Urine analysis
2.Urine microscopy.

Since you are complaining of cloudy appearance of urine it is important to:
1.Rule out albuminuria (Loss of proteins in the urine)
2.To check the number of puss cells to rule out infection.

This morning when I first urinated i had a burning sensation without any flakes? Could this be a UTI?

Hello Sir,

Thank you for wriing back to us.

It can be a UTI if there is burning sensation while micturition. Other symptoms of UTI is increased frequency of micturition, a constant feeling that you have not emptied your bladder fully after the micturition. It may or may not be associated with fever and chills.

Kindly get your urine examination done as advised in the previous answer. Also you can consult with a physician and start of with an antibiotic course like :

1. Tablet (Ciprofloxacin 500mg and tinidazole600mg) , twice daily for ten days.
2. Disodium hydrogen citrate syrup -urinary alkaliser , 15-30ml twice daily (diluted in a glass of
water).

I've had this burning sensation while urinating, with white flakes at the same time (sometimes, but not all the times) it's been 9 months already. I took amaxocillin (a couple pills I borrowed from my buddy) for 7 days + 2 days 'cause I had 2 pills left. It went away for a good two weeks? Then it came back yesterday (5/19/15) i saw my urethra (2 centimeters deep inside my meatus opening) swelling; I went to pee, and the same white flakes came out again? I masturbated at least 3 times two days before it came back ( 5/16/15, 5/17/15, and 5/18/15) the first and second days' of masturbation I used: "cocoa butter gel body oil" to masturbate with, and on the third day I used: "aveeno" body lotion to masturbate with (just pointing it out). I remember having these pains when I was in middle school, I am 18 years old now, but I'm not sure what it is? When I squeeze my penis head it stings a little. 1-10 I give it a .. 4-10 pain while squeezing. I have no discharge, no blood while urinating, no itch, no fever, no std type symptoms? I had a throat infection before hand. I believe it was pharyngitis of some sort? I don't know if that helps either? I have a set schedule to go to the doctors for a check up on XXXXXXX 16th. Please help. Today I did not experience any pain while urinating.

I have gone through your problems in details. It is important to rule out any urinary tract infection. Hence , kindly get a urine analysis report along with urine microscopy and culture n sensitivity.

The reports shall give a better picture of the eitiology.

Also you can get and ultrasound abdomen done to rule out any prostrate pathology or urethral stones.

It is normal to experience little pain on squeezing the male organ hence kindly don't worry about that.

Since you are due for check up , these investigations shall help us come into a conclusion.
